    PHYSICAL EXAMINATION:-

    1. General Appearance:-

     Body Built: Moderately Nourished


     Look: Very Anxious
     Height: 152 cm
     Weight: 62 kg

    2. Vital Signs:-

     Temperature: 98.8 F
     Pulse: 68 beats per minute
     Respiration: 24 breaths per minute
     B.P.: 140/80 mm of Hg

    3. Head and Neck:-

     HAIR: Thick, Normal hair distribution


     SCALP: No dandruff or no any mark of injury
     FACE: Oedema is present on face.

     EYES: Eyes are symmetrical in size and shape.

    Visual activity normal

    Pupils are reacting to light

    Conjunctive is pale in colour

    Eyebrows and eyelashes normal No discharge from the eye.

     NOSE: No septal deviation, No abnormal discharge from


    the nose

    External nares normal Sense of smell normal

     EARS: Earwax is not present in


    external auditory canal in both ears
    No any abnormal discharge from ears

    No any readness or inflammation, Hearing activity normal

     MOUTH: Lips are pale, Teeth are normal

    Gums are weak, Tongue is pale.

    No any signs of any kind of infection in mouth

    Halitosis is not present

     THROAT: No
    tonsillitis

    No dysphagia

     NECK: Full range of motion

    No any palpable lymphnodes

    No jugular vein distention

    4. Chest:-

     Symmetrical in size and shape

     Equally chest expansion


     No any abnormal lung sound heard during auscultation
     Respiration rate is 24/ min

     Breast:- Breast are large in size. Pigmentation over areola present. colostrum can
    be expressed from the breast. No tenderness found and no cracked nipple found.

    5. Abdomen:-

     Inspection:-
     Linea nigra and striae graviderum
    and striae
    albikans is present
     Size and shape is symetrical
     Palpation:-
     Lie is longitudinal
     Presentation:- vertex
     Attitude: general flexion
     Position: L.O.A
     Fundal height 30 cm
     Abdominal size 66 cm
     Uterus is firm and tender
     Auscultation:-
     FHR:- 142 beats per minute

    6. Extrimities:-

     Full range of motion of all joints with full strength.


     Edema and numbnessis present

    7. Back:-

     Normal curvature of spine


     No kyphosis, scoliosis or lordosis
    8. Genitalia:-

     No redness or itching.
     No any signs of infection seen.
     No Blood stained discharge is seen per vagina.
