    REORGANIZATION OF THE MUNICIPAL RAPID/POST DAMAGE ASSESSMENT


    AND NEEDS ANALYSIS TEAM (R/PDANA)

    WHEREAS, hydro meteorological and geologic hazards occurred during every seasons
    and on different scales throughout the year;

    WHEREAS, early and immediate identification of the needs of the affected population is
    highly important;

    WHEREAS, recovery planning after the disaster occurs to is assess effects and impacts
    to determine set of strategies as the blueprint for reconstruction of the community;

    WHEREAS, the accuracy of the data pertaining to the emerging need and recovery
    planning greatly affects and influence the response and rehabilitation/recovery
    measures and recommend actions which are mandated to be delivered and performed
    RDANA Report within the first 72 hours after occurrence of the incidence and PDANA
    during the transition from response to early recovery and rehabilitation period;

    NOW THEREFORE, I, JANE V. PLAZA, Municipal Mayor of Carmen, Surigao del Sur,
    by virtue of the power vested in me by law, do hereby order;

    Section 1: Composition: Carmen Rapid/Post Damage Assessment and Needs Analysis


    Teams (R/PDANA) shall be composed of:

    LAURY P. LAURON - Agriculturist II/OIC Team Leader


    ENGR. JHONAMIE S. MANAWATAO - Municipal Engineer Asst. Team Leader
    SHARON FE A. TISANG - Municipal Social Welfare Development
    Officer- Member
    DR. MARY MICHELLE R. PAGANPAN - Municipal Health Officer- Member
    BELINDA R. DICO - Municipal Assessor- Member
    ALAIN P. CABANILLA - Municipal Environment and Natural
    Resources Officer- Member
    MARILOU A. ASPRIL - Municipal Treasurer- Member
    ENGR. JULBERTO C. TUPAS - General Manager, Carmen Water
    District- Member
    ENGR. BERNARDO REYES - Board of Director, SURSECO II-
    Member

    Section 2: Functions-

    1. The team shall immediately conduct rapid damage assessment and needs
    analysis of the impact of disaster within 72 hours after the occurrence;
    2. The team shall prepared a post rapid assessment damage assessment and
    needs analysis during the transition from response to early recovery and
    rehabilitation phase and submit to the council for identification of strategies;
    3. The team shall accurate gather and collect data regarding any damages
    sustained from the impact of the disaster using the scientific methods and use of
    the prescribed templates as provided;
    4. The reports shall be submitted to the Chairperson of the council thru its
    secretariat the LDRRM Officer.

    Section 3: All issuances inconsistent with this Executive Order are hereby repealed,
    amended, and/or modified accordingly.

    Section 4: This order shall take effect immediately.
