Dear Richard,

I am pleased to extend this conditional offer of employment to you for the position of PART-
TIME Caregiver/CAN (PM SHIFT), reporting to Hope Langston RN, Director of Health Services.
You will report to work at Silverado Thousand Oaks Memory Care Community at 980 Warwick
Ave, Thousand Oaks, 91360. Based on the expected completion of construction and licensure of
our new community, your anticipated start date with Silverado will be determined. Silverado will
contact you two weeks prior to this date to confirm your actual start date.

Your starting hourly rate will be $16.00 and your overtime rate will be determined as required
by applicable state law. Silverado’s dates of pay are the 10th and 25th of each month.

Information about benefits can be found at www.silveradobenefits.com. If you are a full-time


benefits eligible Associate, you have 30 days from your start date to enroll in benefits. If you
decide not to participate, you must go online and decline benefits. If you fail to enroll on time,
you will NOT have coverage (except for Company-paid benefits). The next opportunity to enroll
in benefits is during open enrollment or if you have a qualifying life event. The effective date of
benefits for Nurses is the first day of the month following your start date. For all other full-time
hourly associates, the benefits effective date is the first day of the month after 60 days of
service, following a 30-day orientation period.

This conditional offer of employment and your ability to begin or continue working with us are
contingent upon the results of your reference check, employment verification, and all other
background checks and testing, which may include but are not limited to: a physical/medical
examination, tuberculosis test, drug panel screen, and state and federal criminal history check,
in accordance with any applicable federal, state, or local laws.

By signing this offer letter, you agree that your employment with Silverado is on an "at-will"
basis and can be terminated by you or the Company at any time. You also acknowledge that you
have reviewed the terms and conditions contained herein and there are no other terms,
agreements or promises, verbal or written, outside this letter. Additionally, you agree to follow
Silverado’s policies, procedures and directives in effect during your employment.

To indicate your acceptance of this offer letter, please print and return a signed copy of page
