Activity 1 Checking of Prescription Orders
Check if the prescription orders are correct or incorrect. If the order is accurate, leave the space blank. If
not, write the appropriate prescription order.
Rx Orders
2. Spiriva (Tiotropium Br) 18 mcg cap. 1 cap inhaled TID with handihaler.
4. Augmentin 375 mg cap. Dosage: Adult with mild to moderate infections 375 mg OD.
Co-amoxiclav (Augmentin) 375 mg tab. Dosage: Adult with mild to moderate infections 375 mg
TID.
Roxithromycin (Roximed) 150 mg tab BID or 300 mg tab OD. Take 15 minutes before meals.
6. Sodium alginate (Gaviscon) 500 mg/267 mg/160 mg tab. 2 y/o child: take 20 mL.
ISMN (Isoket 5) 5 mg tab. 2 tabs sublingually. Take 30 minutes before or 2 hours after meals.
9. Felodipine (Plendil ER) 2.5 mg tab. Swallow whole, do not crush.

10. Seroquel XR (Quetiapine) 50 mg tab. Adult with schizophrenia: daily dose of 300 mg (Day 1), 600
mg (Day 2), up to 1500 mg after Day 2.
Quetiapine fumarate (Seroquel XR) 800 mg tab. Adult with schizophrenia: daily dose of 300 mg
(Day 1), 600 mg (Day 2), up to 800 mg after Day 2.
11. Gabapentin (Neurontin) 100 mg cap. Patients undergoing dialysis: loading dose of 400-500 mg
then 300-400 mg following each 4 hr hemodialysis.

12. Tramadol + Paracetamol (Dolcet) 37. 5 mg/325 mg tab. Adult: 1-2 tab 4-6 hrly.

13. Parecoxib Na (Dynastat) 40 mg/ 5mL vial, 40 mg 4-6 hrly followed by 20 mg or 40 mg.
14. Methylprednisolone acetate (Depo-Medrol) 40 mg/mL vial. Multiple sclerosis: 200 mg for 1 week
followed by 80 mg every other day for 1 month.

1. The pharmacy department compounded 30 pieces of cough syrups. Total cost of the ingredient
per syrup is ₱60.00. The pharmacist used bottles which cost ₱15.00 each. Each bottle has its
own label - ₱10.00/label. If a 20% mark-up was added, compute for the dispensing price of the
products with % mark-up and minimum fee.

Dispense the needed medications to complete the medication order. Compute for the
dispensing price if each capsule costs ₱48.00 and a mark-up of 25%.
3. A prescription order calls for 35 pieces paper tablets. All the items will be placed in one paper
envelope. Compute for the dispensing price given the following data:
Anti-hypertensive drug/pptab: ₱25.00
Paper envelope: ₱12.75/envelope
Glassine Paper: ₱3.50 /paper
+ 50% mark-up
4. The prescription calls for 500 mg of Drug B per capsule, 1 capsule four times a day for 3 days.
The standard dispensing fee for pharmacy is ₱5.50 per capsule. What is the dispensing price of
the Drug B capsules if one capsule of Drug B 1000 mg costs ₱10.75?
