Approval Chit (Work)

This document is an approval chit form used for construction projects. It contains sections for the contractor to request inspection and approval to proceed with work. The superintendent officer's representative would then inspect prior work, certify materials, and indicate pass/fail results. Finally, the engineer or quality assurance engineer would approve or not approve the contractor's request to proceed based on the inspection results.
