Information requirements cover’s the clients information and how they are managed and
collected manually and what information needed. Also included the information of the staff
needed from the client. Health Record System develop for the health center do not have yet
an existing system and also for the staff who are the one recording the information of the
client. The hardware requirements needed should be a computer unit with the specs of Core i3
or equivalent processor with atleast 8 Gigabytes of memory for better system performance.
The software requirements needed to develop the system included the Eclipse, used for the
front end, is a free and open source for web application. With such framework, the researcher
worked either with the front end and back end independently at first without defining the at
the start database structure. However, once both were done, they were integrated with the use
of an Application Program Interface (API). This API integrated these two, front end and
backend. An API is a set of routines, protocols, and tools for building the proposed system.
It specifies how software components will interact with one another. For every sprint, this
cycle is being followed. PHP programming language was used in coding process with
MySQL, an open source relational database management system, for defining the database
structure. MySQL Workbench, a unified visual tool used, provided data modeling, SQL
The process box shows the stages undergone during the conduct of the study. These
include its development and assessment. The development stage included a series of
iterations composed of analysis, design, development, testing and then evaluation based on an

the system assessment and evaluation, the developed system were evaluated by the IT experts

Feedbacks from IT experts and end-users were collected and they were incorporated to
