Stakeholders’ Core Values Questionnaire

Direction: Below are some statements that describe each core values. Read them
carefully and respond on how each statement resonates your values. Please answer
your responses on a scale of 1 – 5, where ‘5’ means ‘always true,’ and ‘1’ implies
‘never true.’
