PHYSICAL ACTIVITY READINESS QUESTIONNAIRE (PAR-Q)

The questionnaire will indicate whether you should check with your doctor before you start a
programmed of physical activity. If you are over 69 years of age and you are not used to being very active you
should certainly check with your doctor. We will treat all information confidentially.

Talk to your doctor by phone or in person before participating in physical activity. Tell
your doctor about the question(s) you have answered yes to. You may still be able to do any
activity you want - as long as you begin slowly and build up gradually - or you may need to
limit your activities to those which are safe for you. Talk with your doctor about the kind of
activity you wish to participate in and follow their advice.

If you have answered no to all questions

You can be reasonably confident that you can start to increase your physical activity and
take part in a progressive exercise programmed. Remember - begin slowly and build up
gradually.

What role do you think dance will play in your life after this course?  How do
you believe participating in dance processes will benefit you physically,
intellectually, socially, and emotionally?

When I saw my COE (certificate of Enrollment) and I see those subject, one
thing catches my attention is this subject rhythm and dance because Dancing is my
habit since I was a young and until now, I’m still into to it and I think the role of
dance in my life after this course is gain more experiences even were in this kind
of situation, also gain more learning on another style of dancing and more. since I
didn’t do an exercise and for me the best exchange to not doing an exercise is
dancing, to be honest I would prefer doing dance instead doing exercise and on my
experienced dancing helps me to improve more not just my gesture but also in my
body because physically I feel that every time, I doing dancing in my school I no
longer feel any pain in my back, in my waist and in short body pain. I noticed on
myself while participating the dances in our school if dance avoiding the negative
vibes came from my body like anxiety like that and also for example while in
competition, I already gained some of my classmate into my friends that is not my
totally closed in our room to make it summary I gained some of friends and
socialize with other.
