5. All the necessary requirements for the security personnel or bodyguard are
complied with and the licenses/permits applicable to them and the firearms that

they carry are valid, updated and will not expire during the election period
from January 9, 2022 to June 8, 2022;
6. The 4” x 5” colored picture with full description of the authorized uniform for
the security personnel is in CBFSC Form No. 2022B-06 which is attached hereto;
7. In support of this application, the following documents are submitted:
a. Threat assessment or such other document to support the existence of threat;

and
b. Copies of all the valid licenses and permits including the LESPS and DDOs of
the private security personnel;
8. I acknowledge that payment of corresponding administrative fees is required

and non-refundable;
9. I acknowledge that the CBFSC has the discretion to grant or deny authority to
employ/avail/engage security personnel/body guard;
10. I understand that in case the CBFSC approves my application for engaging the
services of security personnel, each security personnel shall be allowed to carry
only one (1) small firearm.
11. I understand that any misrepresentation or false statement in this Undertaking or

its attachments or any violation of the conditions stated in Comelec Resolution
No. 10728 shall be a ground for the denial of the application or the revocation of
an approved Certificate of Authority. This is without prejudice to the filing of the
appropriate criminal and/or administrative charge/s;
12. The statements stated herein are all willingly, freely and voluntarily executed to
attest to the truth of the foregoing facts in connection with this application.
