Integrative Documentary Portfolio

1. Cover Page
(Follow the format I sent to you. You can add background design that suits to the subject.)

3. Introduction
Must answer the following questions:
 What is a portfolio?
 What are the things that can be found in your portfolio?
 What is the relevance of this portfolio?

It must contain 4-6 sentences.


4. Program Objectives
 State the program objective/s that addressed the relevance of making a portfolio.
 What is the relevance of the portfolio to the program objectives?

At least 3-4 sentences.


5. Learning Tasks/Performance Summary
 Choose only one (1) Task in the entire module (the best task for you). Choose between Activity,
Analysis or Application and fill out the table below.
 Put the Module number, Lesson number, Task (Activity/Analysis/Application). If you choose
application, make sure to indicate the Learning Task number if there is.
 Refer to the syllabus the Program Outcomes and Program Objectives that addressed the Task that
you have chosen. Find the Program Outcome and Program Objective that are aligned with your
chosen task.

1. What is the artifact? (summary of the evidence that document the experience)
2. How does this artifact relate to the program objective? (reveals on the learning that leads towards
meeting the program objective)

 The rationale should be in a paragraph form.

e. Reflection (150 words)

Details, pictures, graphs, texts, images


Should answer questions like:
1. How do these artifacts reflect learning?
2. What elements of program outcome/s are achieved?
3. What behavioral indicators are manifested during and after doing the tasks?

 Reflection should also be in a paragraph form.

Format:

 Use A4 size bondpaper.


 Font size should be 11.
 Font style is either Cambria/Arial.
 It should be Justified.
