    AFFIDAVIT

    I, Priyanka Gandhi Varada spouse of varadha aged 27, residing at 32, kamaraj nagar, erode
    (p.o) Chennai, do hereby solemnly affirm and declare as under:

    1. That my earlier name according to the records was Priyanka Gandhi.


    2. That I have changed my name after marriage as Priyanka Gandhi Varadha on
    (12.06.2020)
    3. At present all the records have my new name Priyanka Gandhi Varadha.
    4. I have not published in the Gazette for the change of name.
    5. I state that Priyanka Gandhi and Priyanka Gandhi Varadha is the name of one and the
    same person and that is myself.
    I am executing this declaration to be submitted to the concerned authorities for the
    change name.
