On 8/14/21, at approximately 9:40 PM, I received a call that the Kalamazoo Co Sheriff’s Department had been
involved in an officer involved shooting. MSP was requested by KCSD to investigate this incident. While
heading to the scene at the below venue, I also learned that Dep. PROXMIRE had possibly been shot and was
headed to the hospital. On Sunday, 8/15/21, I learned that Deputy PROXMIRE had been pronounced dead at
1500 hours. Due to the nature of this incident, there were 2 complaint numbers pulled. 50-249-21 will be taken
for the officer involved shooting, and 50-254-21 was taken for the homicide of Deputy PROXMIRE.
It was also learned during this investigation, that the suspect, KYLE GOIDOSIK had pulled a gun on deputies
at the Shell gas station in Galesburg and shortly after, a pursuit ensued. During the pursuit, the suspect fired
several gunshots at deputies who were engaged in the pursuit. One of the shots stuck Dep. PROXMIRE killing
him. The deputies involved in the chase, at the time were not aware that Dep. PROXMIRE had been hit, so
they continued the pursuit. The suspect continued to shoot at the pursuing deputies until the chase ended at
the below venue, at which point, deputies returned fire, killing GOIDOSIK.

SCENE:
Upon arrival at the scene, at approximately 10:30 PM, there were firefighters who had the street blocked off at
44th and Q Ave East. There was an EMS Ambulance on scene as well as other firefighters inside the
perimeter. Additionally, Sgt. James Harrison from Kalamazoo Co. Sheriff’s Department was on scene and
While on scene, I obtained more details about Dep. PROXMIRE and the location of his vehicle. The location
where Dep. PROXMIRE was shot was on East MN Ave, just East of 11681 East MN Ave. This is an additional
scene reference the 50-254-21 complaint number.

While at the scene, I notified the Kalamazoo County Prosecutor, Jeff Getting of this incident.
Due to the nature of this call, the MSP Grand Rapids Lab was called to process these two scenes.
WEATHER:
The weather, according to the National Weather Service, was clear, the temperature was approximately 63
degrees, the winds were light, and road conditions were dry.

I was advised by Sgt. HARRISON on scene that the following vehicles were assigned to the following deputies:
D/Sgt. Gorham was tasked with obtaining a search warrant for GOIDOSIK’s van. See his supp for details.

GOIDOSIK sustained fatal injuries and was pronounced dead at the scene.
EMS PERSONNEL:
While on scene, I spoke with EMS Personnel who were the first EMS on scene. They were identified as
RACHAEL KOVALCIK and KALSIE VANLINDER. They advised that when they arrived, there were firemen
performing CPR on GOIDOSIK, however, he was DOA. They observed “gross deformities” to the face of
GOIDOSIK as well. I asked them if they observed any guns, however, they did not. They advised they only
saw a gun magazine by a deputy’s car. Their RUN #: Medic 41 – 3115354.
PRONOUNCED DECEASED:
The above EMS personnel advised me that GOIDOSIK was pronounced deceased at 2159 hours by Dr.

contacted me right upon arrival, as he is also on the Fire Department and was on scene prior to my
arrival. He stated that he and
. He said that they heard the sirens from the chase, and actually heard gunshots while the chase
ensued. He advised he did not see anyone shoot, but he heard a volley of 3 gunshots, then a pause, then
another 3 gunshots.
K9 SEARCH of AREA:
MSP K9 Tpr. CARNS was called to the scene to assist in looking for casings where the suspect has
supposedly shot at officers. He was successful in locating multiple casings. See K9 report Can-4027-21 and
Can-4078-21 for details.
ACCIDENT RECONSTRUCTIONIST:
Spl. Sgt. Davis was called to the scene to process the scene with the crime lab. His complaint number is: 01-
407-21.
SUSPECT WEAPON:
As noted above, there was a firearm located between GOIDOSIK and his vehicle. I was advised by the MSP
crime lab that this was a Smith and Wesson 9MM with serial # , which .I
was also advised that his weapon had an empty magazine in it, however, there was another magazine with 2
live rounds in it located on the ground near this one as well. The lab will detail this in their report.

IN CAR VIDEO:
I was advised by Sgt. HARRISON on scene that the following was the information available at that time,
reference in-car and body worn cameras:
Dep. and did not have in-car camera video available, however, they did have body worn
cameras available.
Dep. did have in-car camera footage available and he believed he had body camera footage.
Dep. did not have in-car video, however, should have body camera footage.
I tasked D/Sgt. Gorham to collect all in-car and body camera footage from this incident. It should be noted that
there may be footage recovered that was not listed above. See his supplemental report for details.
On 8/15/21, I met with D/Sgt. MISNER of KCSD who had the deputies’ departmental issued firearms. I was
advised on scene that possibly 2 of the deputies on scene used rifles and those were left in their respective
vehicles. The lab seized those weapons on scene, so the firearms D/Sgt. MISNER had were the deputies’
pistols. The following guns were seized and the ammunition counts are as follows:
Dep. – 9mm, S/N – , had 1 magazine in the weapon with 17 rounds and one round in
the chamber. He also turned over 1 full magazine from his duty belt with 17 rounds of ammunition.

and one in the chamber. He also turned over 2 magazines with 17 rounds of ammunition each in them.

in the chamber. He also turned over 2 magazines with 17 rounds of ammunition each in them.
Sgt. – 9mm, S/N – had 1 magazine with 17 rounds of ammunition in it and 1 in the
chamber. He also turned over 2 magazines, one with 17 rounds and one with 16 rounds in it.
These weapons were entered into property for this complaint, then taken to the Grand Rapids lab for analysis.

STOLEN:
I was advised by D/Sgt. Gorham that the KCSD had sent something out about a stolen boat out of Vicksburg.
He advised me that on the Shell Gas Station video, he thought the boat trailer being pulled by GOIDOSIK
matched that of the stolen one. Based on this information, I asked D/Sgt. Petersen to go to McDonalds towing
and attempt to identify the boat that GOIDOSIK was pulling. See D/Sgt. Petersen’s supp for details.
INVENTORY VAN:
After the MSP Lab was done processing GOIDOSIK’s van, it was towed back down to McDonalds Towing. I
asked D/Sgt. Petersen to inventory the contents for anything pertinent to this investigation. See D/Sgt.
Petersen’s supp for details.
MEDICAL EXAMINER:
RICH ELSMAN arrived on scene as the medical examiner. The ME number for this complaint is: 0815-21-RE-
787-KC.
DRUGS LOCATED:
While loading GOIDOSIK into the body bag, I noticed a bulge in his sock. The ME, ELSMAN pulled out a small
plastic container containing suspected drugs. I seized this on scene and entered it into property under this
complaint number.

On 8/18/21, I received an email from JESSICA RAMIREZ, MSP Grand Rapids Lab reference the firearm
located near GOIDOSIK on scene. Below is the email:
Below is the additional information you requested for the suspect’s firearm:
• Barrel length is 3 inches
• The pistol did not appear to have any alterations/modifications
• The magazine in the gun was empty and the chamber was empty. If fully loaded, the magazine holds 8
cartridges (tested with dummy rounds). So chamber + magazine would mean a maximum of 9 rounds
could be fired from the gun if fully loaded.
• Additionally, a 9mm pistol magazine was found near the pistol that was loaded with 2 cartridges. If
fully loaded, this magazine holds 7 cartridges (also tested). So, 5 cartridges were missing.
• Combined with both magazines and one possible round in the chamber to start, a maximum of 14 rounds
could be missing.
o 1 in chamber + empty magazine that contains 8 at full capacity and other magazine missing 5
cartridges if loaded at full capacity
While I arrived on scene, I was advised that GOIDOSI possibly ran from a traffic stop in Portage on this date,
as well as possibly a couple of days prior to this. Additionally, I was advised that Portage PD had possibly sent
out a BOL for GOIDOSIK due to the traffic stop.
On 8/16/21 I received an email from Portage PD that contained 2 police reports reverence GOIDOSIK. The
first was complaint number 2021-22434 where on 8/13/21, GOIDOSIK fled from a traffic stop after being
identified by Ofc. Reynhout. It was noted in this report that he was in a van with Co license plate: 835QFV,
which is the same as this incident.
The second police report, 2021-22575 was on 8/14/21 at approximately 1324 hours. Ofc. Stapert observed
GOIDOSIK’s van pulling a boat and recognized him and the boat from the previous day’s pursuit (information
passed on from Ofc. REYNHOUT). Ofc. STAPERT attempted to stop him, however, as he approached
GOIDOSIK, he fled and Ofc. STAPERT was not able to pursue. Additionally, the report notes that dispatch put
out a BOL to county agencies for GOIDOSIK, with information that he may be armed. These report will be
placed into external documents.

The above vehicle and boat, as well as Dep. PROXMIRE’s vehicle were towed to McDonalds’
Towing.
PROPERTY RELEASED:
Property items 13, 14, 23, 24, 26, and 27 were personal affects of Dep. PROXMIRE. Pros. Getting
authorized me to release them to the KCSD who will give them to his family. Property item # 55
should have been entered under 50-254-21, so it was released to me and will be entered under that
number.
SUSPECT’S FATHER:
I was able to speak with in reference to this incident. He advised that the Friday
before this occurred, he left for up North and when he returned Sunday, he noticed his home had
been broken into. He said he knew is was his son, KYLE. He made mention of a cooler with a bunch
of food gone and also that KYLE has had issues with paranoia lately. It should be noted that inside
KYLE’s van was a large white cooler with a bunch of food inside. was very apologetic and
expressed deep sympathy for the deputy that was killed.

When unloaded, it contained 6 rounds of ball ammunition.

At approximately 2145 hours on 8/14/21, I received a phone call from D/Lt Criger from 5th District
Headquarters. He advised that members of the Kalamazoo County Sheriff’s Department had been in an officer
involved shooting and they were requesting that we investigate the incident. D/Lt Criger requested that I
respond to the scene at 44th St/East Q ave, south of Climax, MI.
ARRIVAL ON SCENE:
Upon arrival, I made contact with D/Lt Criger and was assigned to make contact at the Shell gas station in
Galesburg to locate/interview any possible witnesses and to obtain surveillance footage. At that time I was also
informed that in addition to the officer involved shooting, Deputy Proxmire had been hit with gunfire during the
vehicle pursuit and was in the hospital in critical condition.

and was aware that someone may be coming to talk with her. I asked her to explain. She stated that
earlier tonight the police were in the parking lot and were talking to a male and the police chased him out of the

parking lot with their lights and sirens on. She stated at one point she had heard the subject yelling something
but she wasn’t sure what he said. stated that the male had entered the store earlier and bought a pack
of cigarettes. He was in the store for a few minutes and then returned to his vehicle. advised that
was aware of what happened and was willing to come in to assist in obtaining video of
the encounter.

the incident in the parking lot. She provided a copy of two video files, one showing the events in the parking lot
and one showing the inside of the store. advised that she believed two customers had exited a black
SUV that was parked by the curb and purchased some items in the store. She recognized one of the
individuals as who had recently applied for a cashier position at the store. At the time this

information seemed relevant because the Deputies who responded to the parking lot initially made contact with
the occupants of the black SUV. It was later determined that had not been in the black SUV but had
walked to the store from that direction, which is why had thought she came from the vehicle.

INTERVIEW ORIGINAL 911 CALLER:
At approximately 1420 on 8/15/21, I received a return phone call from . I advised that I would like
to speak to her regarding what she saw in the parking lot at the Shell last night. She stated, “you guys didn’t
have my back so I’m not helping you out either.” She advised that she was upset because her family was in
danger last night and the police took cover behind her vehicle instead of protecting them. I asked if she
would be willing to talk to me and I tried to explain my role and she advised that she knows what I am
investigating and that she does not care. She hung up the phone.
21:08:16 – A black crossover SUV pulled into the lot and parked by the curb on the southwest side of the
parking lot. The occupants stayed in the vehicle for the duration of their time in the parking lot.
21:15:07 – A black full-size van towing a boat covered with a blue tarp pulled into the parking lot, slowly
passed by the black SUV without stopping, looped around all of the pumps, and pulled into pump #13 facing
towards the gas station. Pump #13 is approximately 30-40 feet from where the black SUV was parked at the
curb.
21:17:01 – The driver of the black van exited the vehicle and entered the gas station to buy a pack of
cigarettes.
21:19:04 – The driver of the black van returned to the vehicle and appeared to pump gas into the van from
pump #13. The male is standing on the north side of the van near the gas pump and is out of sight of the
camera.
21:21:03 – A KCSD patrol vehicle pulls into the lot and parks behind the black SUV by the curb. Deputy
Proxmire makes contact with the driver of the black SUV. Another KCSD patrol vehicle arrives shortly after and
two Deputies emerge and walk up to the black SUV to assist Deputy Proxmire.
21:22:10 – Deputies turn their attention towards the black van parked at pump #13 and move towards the rear
of the van. They cannot be seen on the surveillance footage because the black van is blocking the view. It is
unclear from the surveillance footage what drew the attention of the Deputies.
21:22:38 – Deputies can be seen drawing their service pistols and taking positions of cover and concealment.
There was no indication from the video that any shots were fired at the gas station.

The following is a summary of video file “00000004759000000,” which depicts the events that occurred inside
of the store:
21:13:13 – Two females can be seen purchasing items at the register.( advised she believed
these two females had gotten out of the black SUV parked by the curb, however that was later determined to
be incorrect. The females are unrelated to this incident).
21:17:18 – The driver of the black van entered the store, waited in line and then purchased a pack of Lucky
Strike cigarettes with cash, then left the store and walked back to his van.
21:22:38 – and several customers can be seen turning their attention towards
the parking lot. They went back to business as usual in the store after a few quick glances outside.
I left a business card with and requested that if any customers told her they witnessed what happened
in the parking lot to have them give me a call.
SCENE SEARCH:
I searched the area around pump #13 where the black van had been parked and also along the curb where the
black SUV had been parked. I did not locate anything of evidentiary value.
I placed a request with Kalamazoo County Dispatch for all 911 audio, radio traffic and call for service reports
related to this incident. I was advised that the requested files and documents would be complied and sent to
me as soon as possible. Kalamazoo Dispatch Deputy Director Victoria Rose sent me multiple files which are
detailed below.
On 8/16, I received the below listed audio files from Kalamazoo Dispatch. Each of the files is summarized
below and was entered into property as part of item #61.
Portage Department of Public Safety unit 6202 attempted a traffic stop on a white van that was spray
painted black. The van was towing a boat. Unit 6202 advised over the air that he knew who the driver of
the vehicle was and he was recently arrested with a loaded gun in the vehicle. The black van fled from
the traffic stop and PDPS did not pursue due to the fact the vehicle was passing other motorists in the
turn lane.
2. “CFS 2124 - KCSO Radio Traffic for PDPS Fleeing” - (8/14/21 at 13:25:51):
Kalamazoo County Dispatch put out a BOL reference the fleeing vehicle in Portage. Dispatch also
advised the suspect’s history with firearms.

The caller contacted Kalamazoo Dispatch to request if officers could help get someone out of her car.
She advised that she was traveling down the highway. Dispatch requested that she pull off at 35th st
and park at the McDonalds parking lot. She identified herself as . She stated that the
father of her children wouldn’t get out of the car. The call disconnected.

Kalamazoo County Dispatch dispatched Deputy Proxmire(unit 3466) to make contact with ” at
the Shell gas station at 10305 Miller Road. had called 911 to report that her child’s father would
not get out of the vehicle. Deputy and Deputy (unit ) are dispatched as a backup.
Shortly after arrival, Deputy Proxmire called out over the radio that there was a “male with a gun” at the
gas station.
Deputy Proxmire advised he is in pursuit of a black Econoline van with a Colorado plate, towing a boat.
He advised they were pursuing the van southbound on 35th st and were approaching MN Ave.
Dep Proxmire then advised they turned eastbound on MN ave. As they passed 36th street Deputy
Proxmire advised he was going to back off slightly because there was debris falling from the trailer and

hitting his patrol vehicle. That was the last known transmission Deputy Proxmire made.
Dep takes over radio communication and advised the pursuit was still eastbound on MN ave

passing 39th st. He advised debris was still falling from the trailer.
Dep (unit ) assumed the second position behind Dep and advised that he
believed Deputy Proxmire “is out.”(there is no indication that anyone realized at this point in time that
-
Deputy Proxmire had been wounded.)
As they were approaching 44th st Dep advised that the driver may have just shot a round off at
-
them.
The pursuit turned southbound onto 44th st and into the town of Climax. Dep advised the driver
may have just shot 4 more rounds off in town. Dep confirmed that shots had been fired from
-
inside the van.
After they passed through “the curves” on southbound 44th Dep and Dep confirmed
the driver shot 3 more times and that the shots were fired in their direction.
The pursuit continued southbound on 44th past Q ave, which is a dead-end. An unknown Deputy
advised over the air that the driver is continuing to shoot at them.

“Shots fired” is called out over the air by an unknown Deputy and EMS is requested at 44th st, south of
Q ave.
Units on-scene advise that the scene is secure and disregard any incoming units.
Dispatch advises they have a caller on the line stating a Deputy is slumped over the wheel at MN
ave/38th St.
Dispatch then advises that officers are out and beginning CPR and they begin coordinating a response
to the scene at MN ave/38th st.
KDPS units are dispatched to assist with the KCSD pursuit and are advised the driver of the pursued
vehicle is shooting at County units. Units are enroute to assist when they are disregarded by KCSD
command on-scene.
Unit B30 arrived on scene at E MN Ave and 38th st and began CPR on Deputy Proxmire. She reported
he had a gunshot wound to his head and requested transport immediately.
Several KDPS units transported Dep Proxmire to Bronson Hospital. Additional KDPS units coordinated
with the transport unit to predict the fastest route to the hospital, block traffic points and prepare staff at
Bronson Hospital to intake Deputy Proxmire.
The caller contacted Kalamazoo Dispatch and advised she was on MN ave in Climax. She advised
there was a police car with lights and sirens on that was stopped in the middle of the lane. The caller
advised it was in the middle of nowhere so she didn’t know what was going on. Dispatch advised the
officer was blocking the roadway.
The caller contacted Kalamazoo Dispatch and advised she was on MN ave in Charleston Township
and there was a police officer stopped in the roadway with lights and sirens on. The caller advised they
were concerned for the officer because there was nobody around. Dispatch advised that the officer was
blocking traffic. The caller stated there was no traffic in the area to block. Dispatch advised he was in
the area for an incident and the call was disconnected.
10. “CFS 2518 - Citizen call in Regarding Officer in car4” (8/14/21 at 21:49:25)
The caller contacted Kalamazoo Dispatch and advised he was at MN ave between 36th st and 38th St.
He advised there was a KCSD vehicle stopped in the roadway with lights and sirens on and he was
concerned for the officer because he couldn’t see anyone. The caller was advised that the officer is ok.
11. “CFS 2518 - Citizen call in Regarding Officer in car5” (8/14/21 at 21:51:17)
The caller contacted Kalamazoo Dispatch and advised he was on MN ave next to a KCSD vehicle. He
advised he stopped because the sirens were going off. He stated that the Deputy was inside the vehicle
and was shot in the head and slumped over in the vehicle. The sirens were audible during the call and
distorted some of the audio. The Dispatcher asked for clarification and stated that he had multiple
officers on scene and asked again what was going on? The caller stated there were no other officers on
scene and that the Deputy was all by himself. As the caller is on the phone, another officer arrives on
scene and begins CPR.
Kalamazoo County Dispatch received five (5) calls from concerned citizens regarding Deputy Proxmire’s
vehicle being parked in the roadway with lights and sirens activated. Dispatch was unaware at that time, as
were all law enforcement personnel, that Deputy Proxmire had been shot. All of the calls are saved as part of
property item #61.

something was not right so they called 911. advised the dispatcher told her that the vehicle was
blocking traffic. stated that she did not believe that to be accurate because there was no traffic to block
and the location didn’t make sense. stated that she almost stopped and turned around to make contact
with the officer but then didn’t because she believed dispatch had probably checked the status of the officer.
She left the area and returned to her residence.
I interviewed via telephone reference what had led to her contacting Kalamazoo Dispatch on
the night of 8/14. She advised that she and her family were driving home from Grand Rapids around 2150
hours when they were traveling EB on MN ave. They approached a police vehicle that was stopped in the
roadway in the EB lane and the lights and sirens were activated. advised this seemed very odd
because there were no cars in front of the police car and they couldn’t see anything down the road so they
slowly passed by in the WB lane. She advised that as they passed she looked in the car and couldn’t see
anything so she decided to call 911 because something seemed wrong. She advised the dispatcher told her
the car was blocking traffic and that everything was fine. and her family were not convinced so they
turned around and drove by slowly again to see if they could see in the vehicle and they could not.
began to cry and stated that she wishes they had gotten out but she was afraid for her family at that time
because something seemed wrong. I explained to that they had done all they could and had alerted the
dispatch center of their concerns. stated as they turned back around to go past the vehicle again and
return home, other vehicles had stopped so they went a different way home. once again said that she
felt guilty for not stopping earlier and I assured her that she had done what she could. Our conversation ended
a short time later.

night of 8/14. stated that he had just left the Galesburg Speedway and was going S/B on 38th St. He

he noticed the vehicle wasn’t moving he drove past it slowly and looked inside but could not see anyone in the
vehicle. stated the patrol car was straddling the fog line and was sitting still. All things considered he
thought this was very strange so he contacted 911. He was advised that the officer is ok, so he continued
home.
INTERVIEW:
I interviewed via telephone reference what had led to him contacting Kalamazoo Dispatch on the
night of 8/14. stated that he had just left the Galesburg racetrack. He was driving home on MN ave
when he and several other vehicles came across a patrol car that was stopped in the roadway with lights and
sirens on.

stated he was a couple cars back and he decided that if he hadn’t seen an officer by the time
he got up there he was going to get out and check if everything was ok. stated that he got out of his car
and walked up to the driver’s door and saw the officer slumped over. He opened the door and then observed
that the officer had a gunshot to his head. He advised the officer was slumped forward and to the right, against

his middle console. Steven called 911 and shortly after a female KDPS officer arrived on scene. stated
that when the KDPS officer pulled the injured officer out of the car, the vehicle rolled forwards and off the
roadway. Another bystander put the vehicle in park. stated that the vehicle had not been moving prior
to that so the injured officer must have had his foot on the brake. stated that other officers arrived on
scene and quickly loaded Dep. Proxmire up and left in a hurry. I thanked for what he did that night. No
further information was obtained.
CASING SEARCH:
After reviewing available body cam footage on the morning of 8/17/21, it was determined that there were two
additional locations where shots may have been fired by the suspect at pursuing police vehicles. Tpr Carns
and his police K9 Bo assisted with the shell casing search at approximately 1300 hours on 8/17/21. Bo is
specifically trained in the detection of fired shell casings.
We searched location #1 which was on MN ave, near 42nd st. No shell casings were located.
We searched location #2 which was in the town of Climax, just south of the railroad tracks on the north side of
-
town. One spent casing was located in the driveway of Peace Community Christian Church(item #58). Based
on radio traffic called out during the pursuit it was evident there were several shots fired in that location. While
on scene, rode up on his bike and advised that and
had found some “bullets” in the roadway on Sunday, August 15th and they picked them up as souvenirs. I
made contact with and , and recovered three additional spent
casings. See their interviews below for details regarding the original location of the casings.

On 8/18/21, D/Sgt Misner from KCSD emailed me a photograph of a boat that was stolen in Vicksburg on
8/5/21. He inquired whether the boat that was being towed by the suspect was one and the same. I informed
him that D/Sgt Petersen from MSP Marshall was going to process the boat today and he would advise. I
forwarded the email to D/Sgt Petersen for his reference. See supplemental report 6 for additional details.
On 8/26/21, I was contacted by Victoria Rose, Deputy Director of Kalamazoo Consolidated Dispatch. She
advised that she recently realized we had not yet received the AVL report related to this incident which shows
the location and speed of any vehicles involved. I met with Rose and Director Jeff Troyer on 8/26 and obtained
the report. The digital file is saved to a zip drive and entered as property item #62. Rose also provided me with
a printout which is saved as an external document.
The report and digital file indicate that Deputy Proxmire’s patrol vehicle began pursuing the fleeing van at
21:23:38 hours. He reached speeds of 86 miles per hour during the pursuit. The system pings the vehicle
location approximately every 10 seconds. At 21:26:24, Dep. Proxmire’s vehicle was traveling 70 mph. At
21:26:34 his vehicle was traveling 0mph at a heading of 90 degrees and the nearest location was
. The vehicle remained at 0 mph and a heading of 90 degrees until 21:53:22 when the it
changed direction to 91 degrees. This is likely when Dep Proxmire was extracted from the vehicle by KDPS
and the vehicle rolled forward off the roadway.

Kalamazoo Dispatch Unit AVL Report – 3466

INFORMATION:
On 8/14/21 at approximately 10:00 p.m. I was contacted by D/Lt. Criger and asked to respond to the Climax
area in Kalamazoo County. Upon responding to the scene, I was advised that there were two separate
scenes. One scene was on MN Ave near 38th St. and one scene was on 44th St south of Q Ave. Upon
responding to the scene, I arrived at MN Ave. first and then I was requested by D/Lt. Criger to respond to 44th
St.
Upon arriving on scene at 44th St. just south of Q Ave., I contacted D/Lt. Criger who requested me to obtain a
search warrant for the suspect vehicle. A search warrant was typed and sworn to over the phone by myself to
Judge Santoni. It was signed by myself as the affiant and faxed to Judge Santoni. Upon receiving the signed
copy from the judge, I proceeded back to the scene and provided a copy of the search warrant and tabulation
to Sgt. Jason Sinke.

D/Lt. Criger asked me to obtain the body cams and all in-car cameras from the Kalamazoo County Sheriff
-
Department units involved. Upon doing so, it was found that only one MVR (in-car camera) unit was working.
This would have been in Deputy vehicle. This was unit . A download of the hard drive from
unit# was completed by Lt. Sandlin and a CD was provided to me. This CD was downloaded onto the
-
thumb drive to maintain one article of evidence.
The units for Deputy Proxmire, Deputy , and , and Sgt. had no MVR’s containing any
footage that was retrievable at this time pertaining to this incident. It should be noted that Sgt. MVR
did have traffic stops on I-94 on his MVR however there was no information pertaining to this incident provided
by Lt. Sandlin.

location the suspect traveled south on 35th St. and to MN Ave. and continued on east bound on MN Ave. to

ended on 44th St. just south of Q Ave. 44th St.

INFORMATION:
On 08/15/2021 at approximately 8:30 A.M., I was contacted by D/Lt. Shane Criger regarding an officer involved
shooting where the suspect had been shot and killed. I was also informed that during the incident a
Kalamazoo County deputy had been shot by the suspect and was listed as critically injured. I was requested
to attend the autopsy that had been scheduled for 10:00 A.M. this date.
While at WMED, attending the autopsy of Goidosik, I was informed that the injured KCSD deputy, RYAN
PROXMIRE had succumbed to his injuries and had been pronounced deceased at Bronson Hospital. I was
requested to attend the autopsy of Deputy PROXMIRE – This will be contained under the complaint 50-254-21.
AUTOPSY – GOIDOSIK:
On 08/15/21 at approximately 09:30 A.M., I arrived at WMED for the prearranged autopsy of Goidosik. I
observed the autopsy and spoke with Dr. Douglas during and after the autopsy. Dr. Douglas located three
gunshot wounds to the left leg of the suspect, Goidosik; all of which were through and through the leg with no
projectile found. Dr. Douglas located one gunshot wound to the left side of Goidosik’s face. She indicated the
projectile appeared to be from a high caliber round, shot from a lengthy distance and exited through the front of
Goidosik’s face. Dr. Douglas was able to tell the wound was not self-inflected, however, there was stippling on
Goidosik’s left forearm and left side of his face. Four gunshot wounds in all were located and all left no
projectile to be retained. No other injuries were noted, and the fatal wound was the gunshot wound to the left
side of the head.

Police Officer Kite and Deputy Chief John Blue were on scene in two separate patrol units upon my arrival. I
discussed this matter with them and advised that there was no reason to believe that there was anything
explosive in the unit at this time.

the unit as this unit was in default at the time. She advised that this was the unit for Kyle Goidosik. I then
proceeded to cut off the padlock using a pair of bolt cutters and access was gained to unit 6127.
CONTENTS:
The contents in unit 6127 was various metal objects, a wooden bed frame, books, magazines, a dryer drum,
and various parts of microwaves taken apart.
The unit was 5 X10 and was stacked all the way to the door with various personal belongings. I proceeded
through the storage unit the best I could without removing every item from the storage unit, assisted by Police
Officer Kite. There were no weapons or ammunition of any kind located. The property that was removed from
unit 6127 was placed back into 6127 by myself and Police Officer Kite. It should be noted that there were no
items of value observed in this unit.
Police Officer Kite and I secured unit 6127 with a default lock that was provided by
manager.

On 08-18-2021 D/Lt. Shane Criger requested me to search the suspect vehicle that had been returned to
McDonalds towing after being processed by the forensic laboratory. I was also requested to inspect the boat
and trailer that the suspect was towing during the incident, it was also being held by McDonald’s Towing.
During the search of the suspect vehicle, 2003 Ford Econoline Van, two cell phones and a pistol were seized
and placed in property. The van was filled with miscellaneous items of clothing, tools, power tools, household
items and food. No other items were seized during the search, the van is being held until this case is
reviewed.
It should be noted that the boat and trailer were found to be stolen, the originating agency that handled the
investigation was Kalamazoo County Sheriff’s Department. I spoke with D/Sgt. Misner reference the
investigation and advised the boat was filled with miscellaneous hunting and sporting equipment. The boat’s
registration number had been defaced in an attempt to conceal the boats identifiers. The boat and trailer will
remain at the tow yard until the case can be reviewed.

I first want to extend my condolences to your department. I am not sure if you will be obtaining any
surveillance footage from the Shell gas station, but my husband and I were inside Subway in the Shell station
on Saturday August 14. During this time is when the gun was pulled on one of your deputies. I am unsure of
who the deputy was, but I wanted to reach out and explain what I witnessed during this situation.
The deputy came in and warned us to get down for our safety because the suspect had just pulled a gun on
him. Even during this highly stressful moment, he did what most couldn’t and kept a level head as he backed
in, never taking his eyes off the door towards the suspect’s position to insure we were down in the safest
position we could be. He then ran right back out to the situation and that’s when the suspect took off and he
pursued.
My heart is heavy, and I hope our community knows how much pressure these officers are under daily. In this
situation most wouldn’t be able to handle a gun being pointed towards them, he did. Then he came inside to
help protect others and went back outside to ensure the suspect was caught.
I haven’t been able to clearly identify him as it happened so quickly. I am not sure of formality for security
footage, but I would like to know if it was indeed the same heroic deputy that unfortunately had his life taken

too soon. If it was, I wanted to inform you of his heroic actions leading up to the pursuit.

the stolen boat(KCSD incident 2021-119537) and advised that there was no mention in the original of a stolen
gun. Misner reached out to the responding deputy who advised that had told him that a gun may have

been stolen during the theft and had sent him the information about the gun a few days later so the gun had
not yet been entered into LEIN or the narrative of the report. D/Sgt Misner advised they would document the
theft and recovery of the gun in a supplemental report. He also advised that he had spoken to and they
were going to have the KCSD VSU make contact with him to provide services because he is having difficulty.

“SUMMARY: On the above date and time, Fire Fighters Juday (1492) and C. Audette (1493)
responded in unit 1451. Upon arrival Fire Fighters were met be Kalamazoo County Sheriff
deputies who were actively doing CPR. Fire Fighters were told that the patient suffered from
multiple gun shot wounds. Fire Fighters, with the assistance of Deputies removed the
patients shirt to look for GSW's and trauma. Capt. Juday applied the AED to patient, while
Lieutenant C. Audette took over Continuous Chest compression's.
OBSERVATIONS: The patient had an apparent GSW to head with a major deformity, patient
had no pulse and was not breathing. Once the AED was applied, Officers continued with
CPR until the AED advised to stop and check the patient. The AED never had a shock-able
rhythm. CPR continued until Life EMS Medic 41 arrived on scene applied 12 lead heart

INTERVIEW:
I contacted Assistant Chief Sean Perrin from the Climax Township Fire Department(CTFD) and interviewed
him reference this incident. He advised that he had responded to the scene of the officer involved shooting on
44th st. Perrin advised he was the 4th CTFD responder on scene and he arrived in his personal vehicle. When
he arrived, he observed Capt Juday and Lt Audette performing CPR on a subject who was laying prone on the
side of the roadway. Perrin advised that the Deputies were standing nearby but he did not have any interaction
with them. Perrin advised that EMS arrived a short time after he did. He quickly briefed them on the injuries
when they arrived and then assisted while they took over medical care. Perrin had no further information to
provide.
INTERVIEW:
I contacted Captain David Juday from the Climax Township Fire Department(CTFD) and interviewed him
reference this incident. He advised that he had responded to the scene of the officer involved shooting on 44th
st. Juday advised he was driving home from his family farm on the night of the incident when he saw the police
pursuit go by him S/B on 44th St. Shortly after he heard the 911 tones for a medical call so he drove to the
station and met Lt Audette there. They left for the scene in their rescue unit 1451. When they arrived he
observed Deputies performing CPR on a prone subject who was laying on or near the roadway. He and Lt
Audette took over CPR from the deputies. Juday noticed a severe injury to the subject’s head and face and
knew that he had been shot. They hooked up an AED and no shocks were advised. Juday informed me that
their AED has an audio recording mechanism so they can speak into it and keep track of any medical
interventions they took. I asked if the device was recording on the night of this incident. Juday advised he
believes it was and that I could request the audio from the Chief if it is needed. Juday advised he did not have
any interactions with the police officers on scene and to his knowledge there were no statements made by the
involved officers that would have been recorded by the device.
INTERVIEW:
I contacted Firefighter Mark Wilson from the Climax Township Fire Department(CTFD) and interviewed him
reference this incident. He advised that he had responded to the scene of the officer involved shooting on 44th
st. Mark advised he was the 3rd CTFD responder to arrive on scene. When he arrived Audette and Juday were
performing CPR. He took turns assisting with chest compressions until the patient was turned over to EMS.
Wilson advised he had no contact with the police officers on scene. No further information.
INTERVIEW:
I contacted Lieutenant Chris Audette from the Climax Township Fire Department(CTFD) and interviewed him
reference this incident. He advised that he had responded to the scene of the officer involved shooting on 44th
st. Audette advised that he arrived on scene along with Capt. Juday in fire truck 1451. He advised that when
they arrived officers were performing CPR on the subject and they observed “major deformity” to his head. The
hooked up an AED and began CPR. No shocks were advised. Audette advised they continued care until EMS
arrived and they turned the patient over to them. He had no contact with the officers on scene.

On 8/14/21, at approximately 9:40 PM, I received a call that the Kalamazoo Co Sheriff’s Department had been
involved in an officer involved shooting. MSP was requested by KCSD to investigate this incident. While
heading to the scene at the below venue, I also learned that Dep. PROXMIRE had possibly been shot and was
headed to the hospital. I later learned that he had in fact been shot in the head. On Sunday, 8/15/21, I learned
that Deputy PROXMIRE had been pronounced dead at 1500 hours. Due to the nature of this incident, there
were 2 complaint numbers pulled. 50-249-21 will be taken for the officer involved shooting, and 50-254-21 was
taken for the homicide of Deputy PROXMIRE. This complaint will be close Exceptional Clearance after all
supps and after all property has been disposed.
It was also learned during this investigation, that the suspect, KYLE GOIDOSIK had pulled a gun on deputies
at the Shell gas station in Galesburg and shortly after, a pursuit ensued which started this incident. During the
pursuit, the suspect fired several gunshots at deputies who were engaged in the pursuit. One of the shots
stuck Dep. PROXMIRE killing him. The deputies involved in the chase, at the time were not aware that Dep.
PROXMIRE had been hit, so they continued the pursuit. The suspect continued to shoot at the pursuing
deputies until the chase ended, at which point, deputies returned fire, killing GOIDOSIK. This will be carried
under the 50-249-21 complaint.

The weather, according to the National Weather Service, was clear, the temperature was approximately 63
degrees, the winds were light, and road conditions were dry.

These officers are witnesses only to the events at the Shell station and to the pursuit up until the time Dep.
PROXMIRE was killed. At the time of this report, the officers are unsure of whether or not they wish to make a
statement.

EVIDENCE:
There was 1 shell casing that was located near the scene of Dep. PROXMIRE’s vehicle, which was collected
by the MSP crime lab.
The weapon used by GOIDOSIK was collected by the crime lab under the OIS complaint, but will be compared
to evidence located at both scenes. There was a firearm located between GOIDOSIK and his vehicle at the
location where the chase ended. I was advised by the MSP crime lab that this was a Smith and Wesson 9MM
with serial # I was also advised that his weapon had an empty
magazine in it, however, there was another magazine with 2 live rounds in it located on the ground near this
one as well. The lab will detail this in their report.
On 8/18/21, I received an email from JESSICA RAMIREZ, MSP Grand Rapids Lab reference the firearm
located near GOIDOSIK on scene. Below is the email:
Below is the additional information you requested for the suspect’s firearm:
• Barrel length is 3 inches
• The pistol did not appear to have any alterations/modifications
• The magazine in the gun was empty and the chamber was empty. If fully loaded, the magazine holds 8
cartridges (tested with dummy rounds). So chamber + magazine would mean a maximum of 9 rounds
could be fired from the gun if fully loaded.
• Additionally, a 9mm pistol magazine was found near the pistol that was loaded with 2 cartridges. If
fully loaded, this magazine holds 7 cartridges (also tested). So, 5 cartridges were missing.
• Combined with both magazines and one possible round in the chamber to start, a maximum of 14 rounds
could be missing.
o 1 in chamber + empty magazine that contains 8 at full capacity and other magazine missing 5
cartridges if loaded at full capacity
While I arrived on scene, I was advised that GOIDOSI possibly ran from a traffic stop in Portage on this date,
as well as possibly a couple of days prior to this. Additionally, I was advised that Portage PD had possibly sent
out a BOL for GOIDOSIK due to the traffic stop.
On 8/16/21 I received an email from Portage PD that contained 2 police reports reverence GOIDOSIK. The
first was complaint number 2021-22434 where on 8/13/21, GOIDOSIK fled from a traffic stop after being
identified by Ofc. Reynhout. It was noted in this report that he was in a van with Co license plate: 835QFV,
which is the same as this incident.
The second police report, 2021-22575 was on 8/14/21 at approximately 1324 hours. Ofc. Stapert observed
GOIDOSIK’s van pulling a boat and recognized him and the boat from the previous day’s pursuit (information
passed on from Ofc. REYNHOUT). Ofc. STAPERT attempted to stop him, however, as he approached
GOIDOSIK, he fled and Ofc. STAPERT was not able to pursue. Additionally, the report notes that dispatch put
out a BOL to county agencies for GOIDOSIK, with information that he may be armed. These report will be
placed into external documents.

INFORMATION:
On 08/15/2021 at approximately 8:30 A.M., I was contacted by D/Lt. Shane Criger who requested my
assistance in attending an autopsy Kyle Goidosik, a suspect who had been shot and killed by law enforcement
(50-249-21). The autopsy was scheduled at WMED – Kalamazoo this date at 10:00 A.M. I was also advised
that during the incident, Goidosik had shot and injured Deputy PROXMIRE, who was currently at Bronson
Hospital in critical condition.
While attending the autopsy for Goidosik, I was informed that Deputy PROXMIRE had succumbed to his
injuries and was pronounced deceased. I was requested to attend the autopsy of Deputy PROXMIRE, identify
cause of death and collect any evidence discovered.
On 08/15/21 at 1800 hrs, I attended the autopsy of Deputy PROXMIRE which was conducted by Dr. DeJong at
WMED.
Dr. DeJong was able to determine that Deputy PROXMIRE had sustained a single gunshot wound to the
colliding after the GSW or from CPR being performed during recitation attempts. Nonetheless, the fatal injury
was the single GSW to the head of Deputy PROXMIRE.
Capture during the autopsy was the projectile that was fired from an unknown caliber firearm, that entered
Deputy PROXMIRE’S . The projectile was collected as
evidence (Item #1), and it will be sent to the MSP Forensic Lab for comparison.
PHOTOGRAPHS:
Photographs of the autopsy and evidence collected was documented by WMED staff and copied to an external
storage drive, which was turned over to me. I entered the external drive into property as item # 2.
We have received an investigative report from WMED that was completed by Investigator James Matteson

On 08/14/21 at approximately 2200 hours I was contacted by Lt. Shane Criger and requested to respond to
44th St and Q Ave for an officer involved shooting (50-249-21), while enroute I was redirected to E MN Ave
near 38th St to the scene where Deputy Proxmire was shot. I took initial photographs of the Kalamazoo County
Sheriff Department (KCSD) patrol vehicle while emergency lights were still activated, oversaw the security of
the scene, and interviewed the citizen who first found Deputy Proxmire deceased.
KCSD Deputy James Kristy: had E MN Ave blocked west of Deputy Proxmire’s vehicle.
KCSD Deputy Chad Deem: had E MN Ave blocked east of Deputy Proxmire’s vehicle, located shell casing on

Michigan State Police, Grand Rapids Lab responded to the process the scene and collect evidence.

INFORMATION:
On 08/16/21 at approximately 11:30am I, Tpr. Desch, was called by Lt. Dunlap, requesting my assistance as
an evidence technician. I was available and was advised to contact D/Lt. Criger for my assignment. D/Lt.
Criger asked that I process the Kalamazoo Department of Public Safety (KDPS) police vehicle that transported
Deputy Proxmire from the shooting scene to the hospital. He specifically requested that a sample of the blood
be taken and to look for any other evidence of the deputy’s shooting. He told me that the vehicle was at KDPS
headquarters.
VEHICLE:
I arrived at KDPS headquarters at 1:23pm. Lt. Reiber gave me access to the vehicle, secured in a sallyport.
He used a key to unlock the vehicle doors. He also advised that Deputy Proxmire was transported in the rear
passenger compartment (usually used for prisoner transport). A KDPS officer was driving, with an additional
KDPS officer in the back with Deputy Proxmire.
The vehicle is a KDPS fully marked patrol vehicle, unknown year Ford Explorer, VIN 1FM5K8AW1LGC83128,
MI/082X020, labelled as unit 1123.
PHOTOGRAPHS:
Overall photographs of the exterior and interior of the patrol vehicle were taken as it was parked in the narrow
sallyport (garage). Photographs were then taken to specifically document the blood located on several areas
of the interior. The rubber glove and license plate (mentioned below) were also photographically documented.
Due to the dark interior of the rear passenger compartment while using flash, I had to adjust the exposure
compensation, eventually settling on +3 to more easily notice the blood stains.
OBSERVATIONS:
No evidence or blood was visualized on the exterior of the vehicle. A single key was in the ignition (not the key
used by Lt. Reiber to unlock the vehicle). A WatchGuard camera system was installed, including a camera in
the rear passenger compartment. A license plate (MI/525D01) was loose on the front/passenger floorboard.
The rear passenger compartment is normally used to securely transport prisoners. As such, it has a single
piece, plastic bench seat. A rubber material is installed on the floorboard. The front partition is made of metal
When I opened the right/rear door, I noticed a single, purple rubber glove next to the door and bench seat.
Once I picked this glove up, it appeared unused (clean and still compressed in places). Other than blood
stains (documented below), no other items or evidence was located in the rear passenger compartment.
Nothing believed to be related to this incident was observed in the rear storage area of the Explorer. It held
equipment I assume is standard in KDPS vehicles.
AREAS OF BLOOD:
Blood was observed at several locations along the entire width of the rear, plastic seat, both horizontal and
vertical surfaces. A greater quantity was on the right side. Smaller amounts of blood were also seen on the
left and right rear floorboards, and metal partition.
A small amount of blood was observed to the right of the rear seat, where the plastic seat is contoured to the
floorboard. This blood continued to the “seam” the right/rear door makes with the floorboard. Some blood was
transferred to the metal and rubber seal of the door at this “seam.”
Two very small blood spots were located on the interior of the left/rear passenger door.
A sample of the blood from the rear bench seat was taken by moist swab (property #4). I selected an area on
the right side, on the horizontal surface a couple of inches in front of the curve to the vertical surface.
A small blood stain was noticed on the front passenger seat. It was on the horizontal cloth surface, towards
the front and center. It was dull and approximately the size of a half-dollar (a scale was used for this
photograph). A sample of this blood was taken by moist swab (property #5)
I did not chemically confirm that any of these stains were blood. Based on my observations and knowledge of
this investigation, I believed it all to be human blood. All of it was dry.
RECOVERED:
I donned new rubber gloves prior to collecting each swab. After completing my work on the KDPS vehicle, I
met with D/Lt. Criger at the Kalamazoo Expo Center and turned these two swabs over to him at approximately
3:05pm. I had already packaged and sealed them completely.

Kalamazoo patrol officers Kelly Pittelkow, Nicole Corstange, and Ryan Roddie arrive on scene, begin CPR on
Deputy Proxmire, and transport him to the hospital. The times below will provide documentation regarding the
review of the body camera and patrol car in-car camera recordings.
2152:00 PSO Kelly Pittelkow arrives on scene, removes Deputy Proxmire from the vehicle and begins
CPR.
2154:55 PSO Ryan Roddie arrives on scene. PSO Roddie drives next to Deputy Proxmire’s vehicle and
it is at this time that PSO Roddie, Pittelkow, and Corstange are able to get Deputy Proxmire
into the rear seat of PSO Roddie’s vehicle.
PSO Pittelkow drives to the hospital as PSO Roddie and Corstange provide medical treatment
to Deputy Proxmire. PSO Roddie performed CPR and PSO Corstange provides direct pressure
to the forehead of Deputy Proxmire.
Video footage is in the form of both body cameras as well as in-car camera. Each officer, PSO Roddie, PSO
Pittelkow, and PSO Corstange has a CD from the in-car camera footage for their vehicle and an additional CD
for the body camera footage from their body camera. A total of six CD’s were secured from the Kalamazoo
Department of Public Safety. These will be placed into evidence under this complaint number.

INITIAL RESPONSE:
At approximately 2145 hours on 8/14/21, I received a phone call from D/Lt Criger from 5th District
Headquarters. He advised that members of the Kalamazoo County Sheriff’s Department had been in an officer
involved shooting and they were requesting that we investigate the incident. D/Lt Criger requested that I
respond to the scene at 44th St/East Q ave, south of Climax, MI.
ARRIVAL ON SCENE:
Upon arrival, I made contact with D/Lt Criger and was assigned to make contact at the Shell gas station in
Galesburg to locate/interview any possible witnesses and to obtain surveillance footage. At that time I was also
informed that in addition to the officer involved shooting, Deputy Proxmire had been hit with gunfire during the
vehicle pursuit and was in the hospital in critical condition.

pack of cigarettes. He was in the store for a few minutes and then returned to his vehicle. advised that
was aware of what happened and was willing to come in to assist in obtaining
video of the encounter.

21:08:16 – A black crossover SUV pulled into the lot and parked by the curb on the southwest side of the
parking lot. The occupants stayed in the vehicle for the duration of their time in the parking lot.
21:15:07 – A black full-size van towing a boat covered with a blue tarp pulled into the parking lot, slowly
passed by the black SUV without stopping, looped around all of the pumps, and pulled into pump #13 facing
towards the gas station. Pump #13 is approximately 30-40 feet from where the black SUV was parked at the
curb.
21:17:01 – The driver of the black van exited the vehicle and entered the gas station to buy a pack of
cigarettes.
21:19:04 – The driver of the black van returned to the vehicle and appeared to pump gas into the van from
pump #13. The male is standing on the north side of the van near the gas pump and is out of sight of the
camera.
21:21:03 – A KCSD patrol vehicle pulls into the lot and parks behind the black SUV by the curb. Deputy
Proxmire makes contact with the driver of the black SUV. Another KCSD patrol vehicle arrives shortly after and
two Deputies emerge and walk up to the black SUV to assist Deputy Proxmire.
21:22:10 – Deputies turn their attention towards the black van parked at pump #13 and move towards the rear
of the van. They cannot be seen on the surveillance footage because the black van is blocking the view. It is
unclear from the surveillance footage what drew the attention of the Deputies.
21:22:38 – Deputies can be seen drawing their service pistols and taking positions of cover and concealment.
There was no indication from the video that any shots were fired at the gas station.
21:22:51 – The suspect moved to the driver’s door of the black van and can be seen holding a black object in
his right hand.

21:13:13 – Two females can be seen purchasing items at the register. advised she believed
these two females had gotten out of the black SUV parked by the curb, however that was later determined to
be incorrect. The females are unrelated to this incident).
21:17:18 – The driver of the black van entered the store, waited in line and then purchased a pack of Lucky
Strike cigarettes with cash, then left the store and walked back to his van.

SCENE SEARCH:
I searched the area around pump #13 where the black van had been parked and also along the curb where the
black SUV had been parked. I did not locate anything of evidentiary value.
Portage Department of Public Safety unit 6202 attempted a traffic stop on a white van that was spray
painted black. The van was towing a boat. Unit 6202 advised over the air that he knew who the driver of
the vehicle was and he was recently arrested with a loaded gun in the vehicle. The black van fled from
the traffic stop and PDPS did not pursue due to the fact the vehicle was passing other motorists in the
turn lane.
2. “CFS 2124 - KCSO Radio Traffic for PDPS Fleeing” - (8/14/21 at 13:25:51):
Kalamazoo County Dispatch put out a BOL reference the fleeing vehicle in Portage. Dispatch also
advised the suspect’s history with firearms.

The caller contacted Kalamazoo Dispatch to request if officers could help get someone out of her car.
She advised that she was traveling down the highway. Dispatch requested that she pull off at 35th st
and park at the McDonalds parking lot. She identified herself as . She stated that the
father of her children wouldn’t get out of the car. The call disconnected.

Kalamazoo County Dispatch dispatched Deputy Proxmire(unit 3466) to make contact with “ ” at
the Shell gas station at 10305 Miller Road. had called 911 to report that her child’s father would
not get out of the vehicle. Deputy and Deputy (unit ) are dispatched as a backup.
Shortly after arrival, Deputy Proxmire called out over the radio that there was a “male with a gun” at the
gas station.
Deputy Proxmire advised he is in pursuit of a black Econoline van with a Colorado plate, towing a boat.
He advised they were pursuing the van southbound on 35th st and were approaching MN Ave.
Dep Proxmire then advised they turned eastbound on MN ave. As they passed 36th street Deputy
Proxmire advised he was going to back off slightly because there was debris falling from the trailer and

hitting his patrol vehicle. That was the last known transmission Deputy Proxmire made.
Dep takes over radio communication and advised the pursuit was still eastbound on MN ave

passing 39th st. He advised debris was still falling from the trailer.
Dep (unit ) assumed the second position behind Dep and advised that he
believed Deputy Proxmire “is out.”(there is no indication that anyone realized at this point in time that
-
Deputy Proxmire had been wounded.)
As they were approaching 44th st Dep advised that the driver may have just shot a round off at
-
them.
The pursuit turned southbound onto 44th st and into the town of Climax. Dep advised the driver
may have just shot 4 more rounds off in town. Dep confirmed that shots had been fired from
-
inside the van.
After they passed through “the curves” on southbound 44th Dep and Dep confirmed
the driver shot 3 more times and that the shots were fired in their direction.
The pursuit continued southbound on 44th past Q ave, which is a dead-end. An unknown Deputy
advised over the air that the driver is continuing to shoot at them.
“Shots fired” is called out over the air by an unknown Deputy and EMS is requested at 44th st, south of
Q ave.
Units on-scene advise that the scene is secure and disregard any incoming units.
Dispatch advises they have a caller on the line stating a Deputy is slumped over the wheel at MN
ave/38th St.
Dispatch then advises that officers are out and beginning CPR and they begin coordinating a response
to the scene at MN ave/38th st.
KDPS units are dispatched to assist with the KCSD pursuit and are advised the driver of the pursued
vehicle is shooting at County units. Units are enroute to assist when they are disregarded by KCSD
command on-scene.
Unit B30 arrived on scene at E MN Ave and 38th st and began CPR on Deputy Proxmire. She reported
he had a gunshot wound to his head and requested transport immediately.
Several KDPS units transported Dep Proxmire to Bronson Hospital. Additional KDPS units coordinated
with the transport unit to predict the fastest route to the hospital, block traffic points and prepare staff at
Bronson Hospital to intake Deputy Proxmire.
The caller contacted Kalamazoo Dispatch and advised she was on MN ave in Climax. She advised
there was a police car with lights and sirens on that was stopped in the middle of the lane. The caller
advised it was in the middle of nowhere so she didn’t know what was going on. Dispatch advised the
officer was blocking the roadway.
The caller contacted Kalamazoo Dispatch and advised she was on MN ave in Charleston Township
and there was a police officer stopped in the roadway with lights and sirens on. The caller advised they
were concerned for the officer because there was nobody around. Dispatch advised that the officer was
blocking traffic. The caller stated there was no traffic in the area to block. Dispatch advised he was in
the area for an incident and the call was disconnected.
The caller contacted Kalamazoo Dispatch and advised he was at MN ave between 36th st and 38th St.
He advised there was a KCSD vehicle stopped in the roadway with lights and sirens on and he was
concerned for the officer because he couldn’t see anyone. The caller was advised that the officer is ok.
11. “CFS 2518 - Citizen call in Regarding Officer in car5” (8/14/21 at 21:51:17)
The caller contacted Kalamazoo Dispatch and advised he was on MN ave next to a KCSD vehicle. He
advised he stopped because the sirens were going off. He stated that the Deputy was inside the vehicle
and was shot in the head and slumped over in the vehicle. The sirens were audible during the call and
distorted some of the audio. The Dispatcher asked for clarification and stated that he had multiple
officers on scene and asked again what was going on? The caller stated there were no other officers on
scene and that the Deputy was all by himself. As the caller is on the phone, another officer arrives on
scene and begins CPR.

advised he was coming home from dinner on Saturday night and saw several police cars
pursuing a vehicle SB on 35th st, and then EB on MN ave.

and pulled into his driveway. stated there was a police vehicle parked in the roadway just down the road
from his house, that seemed like it had been a part of the pursuit. stated that he thought it was odd
because the vehicle’s lights and sirens were on and it was stopped in the roadway. advised he contacted
Kalamazoo Dispatch and a short time later he saw other police vehicles arrive on scene and he later heard
what happened.

I interviewed via telephone reference what had led to her contacting Kalamazoo Dispatch on the
night of 8/14. advised that she and her fiancé were picking up their child from a friend’s house when
they encountered a police vehicle that was stopped in the roadway with lights and sirens on. The vehicle was
parked in the EB lane of MN ave. advised she is a Corrections Deputy for Cass County and knew
something was not right so they called 911. advised the dispatcher told her that the vehicle was
blocking traffic. stated that she did not believe that to be accurate because there was no traffic to block
and the location didn’t make sense. stated that she almost stopped and turned around to make contact
with the officer but then didn’t because she believed dispatch had probably checked the status of the officer.
She left the area and returned to her residence.
INTERVIEW:
I interviewed via telephone reference what had led to her contacting Kalamazoo Dispatch on
the night of 8/14. She advised that she and her family were driving home from Grand Rapids around 2150
hours when they were traveling EB on MN ave. They approached a police vehicle that was stopped in the
roadway in the EB lane and the lights and sirens were activated. advised this seemed very odd
because there were no cars in front of the police car and they couldn’t see anything down the road so they
slowly passed by in the WB lane. She advised that as they passed she looked in the car and couldn’t see
anything so she decided to call 911 because something seemed wrong. She advised the dispatcher told her
the car was blocking traffic and that everything was fine. and her family were not convinced so they
turned around and drove by slowly again to see if they could see in the vehicle and they could not.

scene and quickly loaded Dep Proxmire up and left in a hurry. I thanked for what he did that night. No
further information was obtained.
CASING SEARCH:
After reviewing available body cam footage on the morning of 8/17/21, it was determined that there were two
additional locations where shots may have been fired by the suspect at pursuing police vehicles. Tpr Carns
and his police K9 Bo assisted with the shell casing search at approximately 1300 hours on 8/17/21. Bo is
specifically trained in the detection of fired shell casings.
We searched location #1 which was on MN ave, near 42nd st. No shell casings were located.
We searched location #2 which was in the town of Climax, just south of the railroad tracks on the north side of
town. One spent casing was located in the driveway of Peace Community Christian Church(item #58 under
incident 50-249-21). Based on radio traffic called out during the pursuit it was evident there were several shots
that

The report and digital file indicate that Deputy Proxmire’s patrol vehicle began pursuing the fleeing van at
21:23:38 hours. He reached speeds of 86 miles per hour during the pursuit. The system pings the vehicle
location approximately every 10 seconds. At 21:26:24, Dep. Proxmire’s vehicle was traveling 70 mph. At
21:26:34 his vehicle was traveling 0mph at a heading of 90 degrees and the nearest location was
. The vehicle remained at 0 mph and a heading of 90 degrees until 21:53:22 when the it
changed direction to 91 degrees. This is likely when Dep Proxmire was extracted from the vehicle by KDPS
and the vehicle rolled forward off the roadway.

“EVENTS LEADING TO ILLNESS/INJURY: EMS WAS ADVISED THAT THE PT HAD MULTIPLE GUN SHOT
WOUNDS, CPR INITIATED BY MFR'S. EMS ARRIVED ON SCENE AND NOTED INJURIES AS STATED IN
FLOW. UNABLE TO FULL ASSESS OTHER INJURIES DUE TO CRIME SCENE. PT WAS PULSELESS AND
APENIC. POSITION PT FOUND/INITIAL SCENE FINDINGS: SUPINE ON SIDE OF ROAD NEAR VEHICLE.”
“SUMMARY: On the above date and time, Fire Fighters Juday (1492) and C. Audette (1493)
responded in unit 1451. Upon arrival Fire Fighters were met be Kalamazoo County Sheriff
deputies who were actively doing CPR. Fire Fighters were told that the patient suffered from
multiple gun shot wounds. Fire Fighters, with the assistance of Deputies removed the
patients shirt to look for GSW's and trauma. Capt. Juday applied the AED to patient, while
Lieutenant C. Audette took over Continuous Chest compression's.
OBSERVATIONS: The patient had an apparent GSW to head with a major deformity, patient
had no pulse and was not breathing. Once the AED was applied, Officers continued with
CPR until the AED advised to stop and check the patient. The AED never had a shock-able
rhythm. CPR continued until Life EMS Medic 41 arrived on scene applied 12 lead heart
monitor and ran a strip checking further heart rhythm. Life EMS had Fire Fighters continue
CPR and contacted Doctor Doyle at Medical Control. Once a patient update was given, Life
EMS paramedics advised us we could stop compression's. CPR was stopped at 21:59. Dr.
Doyle at Bronson hospital gave TOD.”
INTERVIEW:
I contacted Assistant Chief Sean Perrin from the Climax Township Fire Department(CTFD) and interviewed
him reference this incident. He advised that he had responded to the scene of the officer involved shooting on
44th st. Perrin advised he was the 4th CTFD responder on scene and he arrived in his personal vehicle. When
he arrived, he observed Capt Juday and Lt Audette performing CPR on a subject who was laying prone on the
side of the roadway. Perrin advised that the Deputies were standing nearby but he did not have any interaction
with them. Perrin advised that EMS arrived a short time after he did. He quickly briefed them on the injuries
when they arrived and then assisted while they took over medical care. Perrin had no further information to
provide.
INTERVIEW:
I contacted Captain David Juday from the Climax Township Fire Department(CTFD) and interviewed him
reference this incident. He advised that he had responded to the scene of the officer involved shooting on 44th
st. Juday advised he was driving home from his family farm on the night of the incident when he saw the police
pursuit go by him S/B on 44th St. Shortly after he heard the 911 tones for a medical call so he drove to the
station and met Lt Audette there. They left for the scene in their rescue unit 1451. When they arrived he
observed Deputies performing CPR on a prone subject who was laying on or near the roadway. He and Lt
Audette took over CPR from the deputies. Juday noticed a severe injury to the subject’s head and face and

I contacted Firefighter Mark Wilson from the Climax Township Fire Department(CTFD) and interviewed him
reference this incident. He advised that he had responded to the scene of the officer involved shooting on 44th
st. Mark advised he was the 3rd CTFD responder to arrive on scene. When he arrived Audette and Juday were
performing CPR. He took turns assisting with chest compressions until the patient was turned over to EMS.
Wilson advised he had no contact with the police officers on scene. No further information.
INTERVIEW:
I contacted Lieutenant Chris Audette from the Climax Township Fire Department(CTFD) and interviewed him
reference this incident. He advised that he had responded to the scene of the officer involved shooting on 44th
st. Audette advised that he arrived on scene along with Capt. Juday in fire truck 1451. He advised that when
they arrived officers were performing CPR on the subject and they observed “major deformity” to his head. The
hooked up an AED and began CPR. No shocks were advised. Audette advised they continued care until EMS
arrived and they turned the patient over to them. He had no contact with the officers on scene.

I responded to Bronson Methodist Hospital on 8/14/21 to collect evidence and to act as liason. The evidence
collected is entered in the officer involved shooting complaint, 50-249-21.
MEDICAL INFORMATION:
Sgt. Proxmire was transported by fellow officers to the emergency department. The attending physician in the
emergency department on the evening of 8/14/2021 was Brian Devetter, MD. The trauma attending physician
was Henry Scott Bjerke, MD.
The time of death was recorded at 15:54 on 8/15/2, per autopsy report. Sgt. Proxmire was pronounced dead
by Sandra Medinilla, MD, MPH, FACS.
The assigned medical examiner investigator (MEI) was James Matteson. The medical examiner and forensic
pathologist that performed the postmortem examination was Joyce L. deJong, DO.
AUTOPSY REPORTS:
Autopsy reports were received on 9/8/21. The report indicates the manner of death was homicide and
describes the gun shot wound to the head of Sgt. Proxmire. The toxicology findings were negative for any of
the screened substances.

SUMMARY:
On 9/23/21 at approximately 10:00 a.m. I contacted who is employed with McDonald Towing. I
was asked by D/Lt. Criger to verify whether or not the safety belt was worn by Deputy Proxmire at the time of
this incident.
I contacted Officer Kelly Pittelkow, a public safety officer with Kalamazoo Department of Public Safety and was
first on scene to assist the injured Deputy Proxmire. Upon speaking with Officer Pittelkow, she advised that
she believed that Deputy Proxmire was wearing his safety belt, as she had attempted to remove Deputy
Proxmire from the vehicle and was unable to do so. She states she believed he did have it on, but she was not
sure. PSO Pittelkow advised she does not believe she turned on her body camera until after Deputy Proxmire
was removed from the vehicle.
On 9/23/21 at 10:00 a.m. I met who is employed with McDonald Towing. He allowed me to have
access to the secure building where Deputy Proxmire’s vehicle was being secured and stored. It was at this
time I examined the seatbelt.
Upon examination of the safety belt, it is apparent that the safety belt was in use at the time of this incident as
the safety belt was pulled out to what would have been the worn position of the driver (Proxmire). The lap belt
portion of the safety belt was saturated in blood. It should be noted that directly to the right of this location on
the seat was also a large pool of blood.
I asked , who is employed with the McDonald’s Towing agency, to step over to the vehicle and
witness what I was examining in the vehicle. 1111 did observe the blood on the safety belt.

Narrative
08/14/2021 the Climax Township service reported on incident 2021-00000127, call number 2557. The
incident occurrence was at the Street or Highway located at address S 44TH ST in the city of CLIMAX
within the county of Kalamazoo of the state of Ml. PSAP received the call at 21:38. The unit was
notified at 21 :38, responded at 21 :42, arrived at the scene at 21 :46, left the scene at 23:38, arrived at
the destination at Not Recorded and completed the call at 08:30.

The EMS Crew consisted of JU DAY, DAVID (1520499) who was the Primary Patient Caregiver-At Scene,
AUDETTE, CHRISTOPHER (516964) who was the Primary Patient Caregiver-At Scene and WILSON,

On the above date and time, Fire Fighters Juday (1492) and C. Audette (1493) responded in unit 1451.
Upon arrival Fire Fighters were met be Kalamazoo County Sheriff deputies who were actively doing
CPR. Fire Fighters were told that the patient suffered from multiple gun shot wounds. Fire Fighters,
with the assistance of Deputies removed the patients shirt to look for GSW's and trauma. Capt. Juday
applied the AED to patient, while Lieutenant C. Audette took over Continuous Chest compression's.
OBSERVATIONS:
The patient had an apparent GSW to head with a major deformity, patient had no pulse and was not
breathing. Once the AED was applied, Officers continued with CPR until the AED advised to stop and
check the patient. The AED never had a shock-able rhythm. CPR continued until Life EMS Medic 41
arrived on scene applied 12 lead heart monitor and ran a strip checking further heart rhythm. Life EMS
had Fire Fighters continue CPR and contacted Doctor Doyle at Medical Control. Once a patient update
was given, Life EMS paramedics advised us we could stop compression's. CPR was stopped at 21 :59. Dr.
Doyle at Bronson hospital gave TOD.
UNITS INVOLVED:
On Scene Command- Unit 1492
Chest compression's were performed by Units 1493, 1417, 1409.
Ventilation's were performed by Units 1417, 1493.
Life EMS Medics were Rachael Kovalcik and Kelsie Van-Linder
OTHER:
Units were on scene from 21 :46 hrs until 08:30 on 08-15-21.

NARRATIVE
SUMMARY:
On 08/17/2021 I responded with several other units to assist KCSD officers who were in pursuit of a subject actively shooting at them during the
pursuit. Prior to catching up to the pursuit I was told the suspect was no longer a threat and city units were needed only to assist with in progress
calls. While returning to the city, travelling westbound on MN Ave near 38th St I happened upon a county cruiser with the lights and sirens
turned on. I checked the status of the deputy and observed a gun shot wound to his forehead I immediately removed the deputy from his vehicle
and began life saving efforts. I requested additional units to my location to transport the deputy to Bronson Hospital. PSO Corstange and PSO
Roddie arrived, the deputy was loaded into vehicle 1123 and transported to Bronson hospital.
OBSERVATIONS:
As I approached the county cruiser I noticed the lights and sirens were still on. A male citizen was standing near the cruiser flagging me down on
the phone. There was a deputy in the driver's seat slumped over the steering wheel. I approached the driver's door. I could see a gunshot wound
the deputy's head. He was unresponsive. I opened the door and pulled the deputy out. After removing the deputy the vehicle began to roll
forward. A citizen ran up to it and put it in park as I was pulling the deputy out of the way so he was not run over by his own car. I opened his
shirt and vest and began CPR. I did not observe any other injuries on the deputy. I continued CPR until a transport vehicle arrived. PSO
Roddie and I loaded the deputy into the back seat where PSO Corstange was waiting to continue CPR. PSO Roddie also got into the back seat to
assist PSO Corstange with life saving efforts during the transport.
TRANSPORT:
I drove vehicle 1123 to Bronson Hospital Ambulance bay where staff was waiting for our arrival The deputy was transferred from the cruiser to a
stretcher and into Bronson Trauma Unit.

SUMMARY:
On August 14th, 2021 I assisted with an officer involved shooting. The original call for service was at 10305 Miller Rd in Comstock, County of
Kalamazoo, and State of Michigan. I took command of managing police personal at Bronson hospital as the Kalamazoo County Sheriff Deputy
was being transported for medical treatment due to his gun shot wound.
DISPATCH:
Kalamazoo County Sheriff Office was in pursuit of a suspect who pointed a firearm at a KCSO Deputy, The pursuit ended when the suspect
crashed in a field near 44th Street and Q Ave.' Sgt. Pittelkow came upon a Deputy Ryan Proxmire's vehicle on a few miles away from the crash
scene. Sgt. Pittelkow observed a gun shot wound to Deputy Proxmire's head. Sgt. Pittelkow, PSO Roddie and PSO Corstange loaded Deputy
Proxmire into vehicle 1123 and headed to Bronson Hospital.
BRONSON HOSPITAL:
I was assigned as B10 during the shift. I drove to Bronson Hospital once I heard Sgt. Pittelkow enroute with Deputy Proxmire. I entered the
emergency room entrance and contacted the nurse at the registration window. I informed her I needed medical staff on standby at the ambulance
entrance for a Deputy who was shot. I assigned two PSOs to the main entrance of Bronson Hospital in case the suspect's family arrived. I
assigned two PSOs to the ambulance entrance. I assigned two PSOs to go with me into Bronson Hospital when Deputy Proxmire arrived.
SCENE SECURITY:
PSO Prichard and I stood outside of Trauma room C until additional Kalamazoo County Sheriff Deputies and command arrived to take over. The
point of contact was turned over to Sgt. Noble for the remainder of the shift.
DVR/BODY CAM:
I used DVR equipment from vehicle 1170 and department issued body cam in this incident. The DVR and body cam videos were not saved as
evidence.

INFORMATION:
At approximately 21:07 hrs, KCSD responded to the Shell station in reference to a TWS. The suspect later identified as Kyle Goidosik was found
to be in possession of a handgun and a pursuit ensued. Sgt. Pittlekow (K-9 Sgt), PSO Corstange {K9) PSO Roddie (SWAT) and I were sent to
assist KCSD. As the pursuit continued deputies advised that they were taking gunfire from the suspect. The deputies eventually ended up in the
area of SB 44th at Q at a dead end where gunfire was exchanged between suspect and deputies. KCSD advised at 21 :37 hours that the suspect was
down and they were beginning CPR.
I made contact with KCSD command and they advised they were 10-14 and no longer needed our assistance besides taking in progress calls fur
service. I began to make my way back to the city when Sgt. Pittlekow called out with an officer down in the area ofMN/38th St where Deputy
Ryan Proxmyer was found with a single gunshot wound to the head in his vehicle. Proxmyer was loaded in vehicle # 1123. Sgt. Pittlekow drove
while PSO's Corstange and Roddie performed CPR in the back of the car.
I drove to Bronson Hospital to take command. Sgt. Mireles was on scene along with a number of PSO's, I directed Mireles to have two PSO's
ready to stand guard with Proxmyer and to have two PSO's ready to stand by with the possible suspect arrival. PSO Baldwin was tasked with
making sure the ambulance bay was clear. Bronson medical staff was continually updated with PSO Pittlekow's location and arrival time.
Upon Pittlekow's arrival, I assisted with removing Proxmyer from the vehicle. After this was completed, I had Sgt. Pittlekow, PSO Corstange and
PSO Roddie stand by and infonned them that they were to retum to HQ to take them out of the situation.
I was assigned to Bronson Hospital while monitoring county calls for service. At Bronson I updated Lt. Nielsen with updates on Proxmyer. Lt.
Nielsen, Sgt. Reidenbach and I additionally coordinated getting food for the County Deputies at the hospital. I stood by at the hospital until 630
hrs. I briefed day shift Sgt. Mason and Lt. Smith before leaving for shift.

INFORMATION:
On 8/14/2021 at approximately 2106 hours, I was conducting a Directed Patrol at the Bronson Hospital Emergency Department, at 601 John St, in
the City of Kalamazoo, when dispatch relayed information regarding a Kalamazoo County Sheriffs Department vehicle pursuit and eventual
officer involved shooting that originated from the Shell Gas Station in Galesburg, Michigan. I assisted in the investigation by helping remove the
involved officer, later identified as Deputy Ryan Proxmire, from the patrol vehicle on arrival at the hospital and transporting him to Trauma Room
C. I remained with Deputy Proxmire until he arrived at TCU Room 143, and provided security outside of the ambulance bay entrance to the
emergency department until approximately 0630hrs. This concludes my involvement in this investigation.
SCENE SECURlTY:
I remained with Deputy Proxmire while he was receiving treatment in Trauma Room C, when he was brought for a CT Scan, and finally, when he
was transferred to Trauma Care Unit Room 143. I left Room 143 when SherriffFuller and another KCSD deputy arrived at the room.
WATCHGUARD:
WFClRl 10788 was activated during this investigation.

INFORMATION:
On 08- 14-21 after 2100 hours, I responded with other KDPS units to assist KCSO Deputies who were in pursuit of a suspect who was actively
shooting at them. Prior to arriving on scene, KDPS was advised we could clear after the suspect was shot and there was no longer a need for us.
While driving back to the city, SGT Pittelkow called out an officer down as she had located Deputy Proxmire in his patrol car suffering from a
gunshot wound to head.
I arrived on scene shortly after this and loaded Deputy Proxmire in the backseat of my patrol vehicle (1123); SGT Pittelkow drove to Bronson
Hospital while PSO Corstange and myselfperfonned life saving efforts in the backseat.
OBSERVATIONS/ACTIONS:
While enroute back to the city, SGT Pittelkow got on the radio and advised she was out with a deputy who had been shot at MN/38th St. PSO
Corstange and I were on MN heading WB just down the road. Upon arrival, I observed SGT Pittelkow performing CPR on a deputy in the road; I
positioned my patrol vehicle directly next to her and prepped the backseat for a "load and go. 11
I approached the deputy and observed a gunshot wound to the head; at this time, I assisted in lifting the deputy into my backseat where PSO
Corstange was waiting on the passenger side to assist. I then climbed in on top of him and continued chest compressions. I continued to perfo1m
chest compressions until we arrived at Bronson ER. I then assisted in lifting Deputy Proxmire out of the car and onto the stretcher. I stopped chest
compressions once he was turned over to staff in a trauma room.

PRIMARY INFORMATION:
On 08/13/21 at approximately 0130 HRS, I was on patrol in the city of Portage. My patrol vehicle was
parked in a public parking lot off Portage Road near Lansing. I observed a Ford Econoline van turn south on
Portage Road from Ramona pulling a utility trailer with a motorcycle strapped to it. As the van passed, I could
hear it had an extremely loud muffler. I also observed there were no taillights or any lights at all on the trailer it
was pulling, and I did not observe a license plate on the trailer either. I followed the vehicle SB on Portage
Road, and in the passenger section of Romance, I activated my emergency lights and initiated a traffic stop.
TRAFFIC STOP:
The vehicle pulled over to the right side of the road, and as I approached the vehicle on foot on the
passenger side, there was a license plate affixed by a single screw and hanging vertically on the utility trailer's
rear gate. However, it was an extremely old blue Ml plate and was unreadable. I also observed there was a
Colorado license plate affixed to the rear of the Econoline van of 835QFV. The van was originally white.
However, it appeared someone attempted to cover up some writing on the side of the van with black spray
paint.
LEIN/SOS:
At the time of my stop, my computer was not functioning, and I ran all LEIN/SOS through Dispatch and
had to issue a paper citation. According to Dispatch,

CITATION ISSUED:
I did issue a paper citation to GOIDOSIK for the improper plates, no proof of insurance and no taillights
on the trailer. Because of the misdemeanor citation and the fact the title was not in his name and no
insurance, I was going to impound the vehicle.
SUMMARY:
On 08/13/21 I observed a 2003 Ford Econoline van traveling SB on Portage Road pulling a utility trailer
with no lights and an unreadable license plate. I initiated a traffic stop on Portage and Romance and identified
the driver by his Ml DL as KYLE ANTHONY GOIDOSIK - I initially issued him a misdemeanor
citation for improper plates and ordered him out of the vehicle as I was going to impound the car. He refused
my lawful order to step out of the vehicle and fled SB on Portage Road where I lost sight of him. The initial
ticket I wrote him will be void, and I will be requesting charges for flee & elude.

SUPPLEMENTAL INFORMATION:
On 08/13/21 at 0130 HRS, I assisted OFC REYNHOUT with a traffic stop he conducted on Portage
~ Road south of Romence Parkway. I initially assisted him by obtaining a VIN number from a mid-to-late 70s
Suzuki motorcycle on a trailer being towed by the vehicle he conducted the traffic stop on.
Additionally, I assisted by carrying on a conversation with the driver, KYLE ANTHONY GOIDOSIK
During my conversation with GOIDOSIK, he was wearing a green shirt with black pants.
Throughout my conversation with him, I noted he appeared paranoid and often spoke about conspiracy
theories he had.
R&O:
While standing at the passenger's window of GOIDOSIK's vehicle, I observed OFC REYNHOUT step
into the driver's window and instruct GOIDOSIK to step out of the vehicle. He did not comply with OFC
REYNHOUT's orders to step out of the vehicle and began an argument with him. At one point during the
argument, I specifically recall GOIDOSIK asking why the vehicle was being impounded, and OFC REYNHOUT
informed him the vehicle was not legal to be on the roadway. After pausing for only a brief second, GOIDOSIK
shouted "No," started the vehicle and pressed the accelerator, simultaneously putting the transmission into
drive and speeding away SB on Portage Road. After confirming all officers were uninjured, we began following
the vehicle SB along Portage Road. It should be noted no emergency lights or sirens were activated at this
point. We did not pursue the suspect per department policy. I specifically set the cruise control of my vehicle
at 44 mph in a 45 mph speed limit. I later set the cruise control at 52 mph in a 55 mph speed limit area.
GOIDOSIK was last seen driving his vehicle SB on Portage Road from Bacon Avenue and was unable to be
located after that.

PRIMARY INFORMATION:
On 08/13/2021 at shift change, Officer REYNHOUT told me that he had stopped KYLE GOIDOSIK at
approximately 0130 hours on Portage Road near Romence for several equipment violations. He said
GOIDOSIK was driving a white Ford Econoline van that had been spray painted black and that it had a
Colorado license plate on it. GOIDOSIK provided Officer REYNHOUT with his Michigan driver's license but
when Officer REYNHOUT went to serve him with a citation and tell him that his vehicle was being impounded,
GOIDOSIK fled in the vehicle southbound on Portage Road. Due to this department's pursuit policy, Officer
REYNHOUT did not pursue and lost sight of him.
On this date (08/14/2021), at approximately 1324 hours, I was northbound on Lovers Lane approaching
E. Milham when I observed the same white Ford Econoline van that was spray painted black travel eastbound
on E. Milham through the Lovers Lane intersection. The driver of the van, whom I eventually was able to
identify as KYLE GOIDOSIK, was towing a boat on a trailer and there was no trailer plate visible. GOIDOSIK
continued eastbound on E. Milham toward Portage Road and by the time my light changed and I was able to
catch up with him, he was turning southbound on Portage Road from E. Milham. I immediately activated my
emergency lights to stop GOIDOSIK because I couldn't see the license plate on his boat trailer and I also knew
that he had fled from Officer REYNHOUT. In an addition, the van had an improper Colorado license plate on it.
GOIDOSIK saw my emergency lights and brought his vehicle to a stop on the right side of the road at the north
entrance to the Stryker property.

Dispatch put out a BOL to county agencies which included information that GOIDOSIK may be armed
(due to my history with him) and eventually asked Vicksburg Police Officer MARK PETERSON to check
GOIDOSIK's last known address, I was told by Officer
PETERSON that he was not there.

VEHICLE INFORMATION:
Dispatch was able to determine that the vehicle GOIDOSIK was operating was a white with black 2003
Ford Econoline van with VIN 1FTSS34S63HA55588. The license plate that was assigned to the van was
DC15324 expiring in February of this year. At the time of this report, I have not yet had a chance to review my
body worn camera to see if I could see what the boat trailer plate was. According to dispatch, GOIDOSIK was
the suspect in a B&E in Kalamazoo earlier in the week where some tools were stolen. It is highly likely that the
boat GOIDOSIK was pulling when he fled from me was stolen, as well.
EVIDENCE:
My body worn camera and patrol vehicle MVR were recording during the entire incident.
CASE SUMMARY:
On 08/14/2021 at approximately 1324 hours, I stopped KYLE GOIDOSIK for a license plate violation in
the 6200 block of Portage Road. I was also acting on information Officer REYNHOUT had provided me on
08/13/2021 when he said that GOIDOSIK fled from him on a traffic stop. Initially, GOIDOSIK stopped his 2003
Ford Econoline van that was white with black spray paint over it but as I approached him, he fled southbound
on Portage Road accelerating well above the posted 45mph speed limit. GOIOOSIK was passing other
southbound vehicles using the turn lane and also drove into oncoming traffic to pass a vehicle at the
intersection of Portage Road and Romence. GOIOOSIK continued to flee eastbound on Romence at above the
posted 40 and 45mph speed limits and then continued eastbound on Romence through Sprinkle out into the
county where I lost sight of him. At the time I stopped GOIDOSIK, I was driving a fully marked Portage Police
Department patrol vehicle and in full uniform. Due to departmental policy, I was unable to pursue him.
This case was going to be sent to the Prosecutor's Office for review, however later in the day,
GOIDOSIK fled from KCSD deputies and was killed in what was thought to be an officer involved shooting.

19. Property Left on Premises: Tenant shall not commit or permit any damage or waste to the

1. Your right to occupy Space 6127 has been tenninated, and you no longer have access to the stored
property.
2. The stored property is subject to a lien. The amount of the lien is currently $105.95. Attached you will
find an itemized statement of your charges. This amount will increase in accordance with the terms of
your rental agreement until paid, or until stored property is sold at a lien sale.
3. You must pay this amount within 14 days of the date of mailing this notice.
4. Unless this claim is paid within 14 days of the date of mailing of this notice, the stored property will be
advertised for sale or other disposition, on September 9, 2021 , at StorageTreasures.com, commencing

The video recordings from Sergeant Ryan Proxmire and Sergeant cruisers have been
retrieved. The recording retrieval was delayed due to corruption of the video files on the drives.
While the files appear to have lost an unknown amount of time from the end of each file from that
corruption, the central events of Saturday, August 14, 2021 were recovered, and the files manually
transferred to USB.
Background
The ICOP mobile video recorders (MVRs) in Sergeant Proxmire and Sergeant- cruisers i

record onto removable hard drives. Following the events of Saturday, August 14, 2021, the drives for
Sgt. Proxmire and Sgt.-were recovered and Sgt Schmaltz and Sgt Stryd attempted to upload

them to the Kalamazoo County Sheriff's Office ICOP iVault server. The iVault software uploaded
previous files on the drives but refused to recognize the files from the event and would not import
them into the iVault file repository. The files could not be viewed on the hard drives directly due to

By digging into the server's directories, I was able to discover a temporary file cache containing the
video files from the events in question. While the files weren't labeled and some metadata

(date/time on file) was missing, I was able to find the events and export the appropriate
files.
I've added the MVR footage and Body Worn Camera (BWC) footage to USB (three USB drives total)
for Michigan State Police Detective/Sergeant Gorham, and I provided Lt. Sandlin with a final drive.

I first want to extend my condolences to your department. I am not sure if you will be obtaining any surveillance footage
from the Shell station but my husband and I were inside Subway in the Shell Station on Saturday August 14th. During
this time is when the gun was pulled on one of your deputies. I am unsure who the deputy was, but I wanted to reach
out and explain what I witnessed during this situation.
The deputy came in and warned us to get down for our safety because the suspect had just pulled a gun on him. Even
during this highly stressful moment he did what most couldna€™t and kept level headed as he backed in never taking his

eyes off the door towards the suspects position to ensure we were down and in the safest position we could be. He then
ran right back out to this situation and thata€™s when the suspect took off and he pursued.
My heart is heavy and I hope our community knows how much pressure these officers are under daily. In this situation,
most wouldna€'01t be able to handle a gun being pointed towards them, he did. Then he came inside to help protect
others and went back out to ensure this suspect was caught.
I havena€™t been able to clearly identify him as it happened so quick, I am not sure of formality for security footage but
I would like to know if it was indeed the same heroic deputy that unfortunately had his life taken too soon. If it was, I
wanted to inform you of his heroic actions leading up to the pursuit.

SUMMARY:
On the listed date and time, I was contacted by MSP Operations and requested to assist with a shell casing
search following an Officer Involved Shooting. Operations advised Kalamazoo County was involved in a
shooting and an officer had been hit I responded to the scene and made contact with Sgt Davis and D/Lt.
Criger.
I was given several locations that were to be searched with Canine Bo locating shell casings at two of the three
locations. All shell casings were photographed and collected by MSP Lab Techs. The locations the casings
were located at are listed below. Following the searches nothing further was conducted and the scene was
cleared.
