INFORMED CONSENT

PART I: Information Sheet

I, ____________________ , a student of Master of Science in Economics of the Holy Cross of Davao College
is inviting you to participate in my research study entitled, ___________________________________________.
The purpose of this study is to find out and describe the experiences felt by the resort owners in the
island with regards to the pandemic and its effects and coping strategies of the resort owners. You
can take time to reflect on whether or not you want to participate. I assure you that I am willing and
open to whatever clarification and questions you would like to ask at any time.

You have been chosen to participate in the study because you qualify the participants’ criteria set
for this study. These criteria include the following: _______________________________________________________.

If you decide to participate, you will be asked questions. Your decision to participate is voluntary.
Rest assured that all information gathered will be kept in utmost confidentiality and would be used
only for academic purposes. Your name will not appear in any research publications resulting from
this study.

Procedure

The research will strongly observe the health protocol in conducting the interview and focus group
discussion such as: social distancing, use of alcohol, wearing of face mask and face shield and
washing of hands before and after the interview. The study involved data collection through in
depth interview with the use of semi structured guide questions. I will explain to you what it
entails to participate in this research. I have arranged schedule for your interview. If you are not
available, then I will arrange the time convenient to your schedule.

It is a chance for you to tell your story and experiences concerning the effect of the pandemic in
your business operation and your strategies to survive despite of the pandemic. The local
government unit of this city will also benefit from this study to make them informed decisions
when it comes to problems and solutions brought by the pandemic.

Confidentiality

Your responses to the interview questions will be kept confidential. At no time will your actual
identity be revealed. You will be assigned a random numerical code. Whoever will transcribe the
data, the responses will only know you by this code. The recording will be destroyed as soon as it
has been transcribed. The transcript, without your name, will be kept until the research is complete.

Sharing the results

Please be aware that upon completion of the research, the research findings will be shared more
broadly through publications and conferences.

Participation and Withdrawal

Your participation in this study is completely voluntary, and you may refuse to participate or
withdraw from the study without any legal implication.
