Electronic Health Record Management System

The document outlines the design of an electronic health record management system (EHRMS) to address issues with the current manual healthcare system by creating a centralized digital patient database accessible to doctors. A needs assessment was conducted through interviews with patients, doctors, and hospital management to identify key system requirements. Various analyses including feasibility, data modeling, and interface designs were presented to develop the proposed EHRMS.

1.Problem Identification and Opportunity Analysis :
In the process of problem identification the problem should be either real or
anticipated, that require corrective action and opportunities to improve a situation
despite the absence of complaints .A key element of this analysis will ensure that “root
causes,” not just the symptoms of the problem, are identified and subsequently
addressed in the project design. Here we are identifying our problem with existing
health care system and work on find out the best opportunities.

3.Description of Electronic Health Record Management System
The rapidly advancing field of information technology has provided many excellent
innovations, but it has also put pressure on the healthcare field to keep up. Health
information technology, which involves the management of patient health records, has
struggled to implement necessary changes in a timely manner. The lack of proper
implementation of electronic health record systems can cause errors in patient care
management and medical billing, along with other issues. So, the need to manage
health records safely and efficiently is important for many healthcare organizations.
4.Current Situation(As is System):
1. Manual reporting of patient
2. No centralized database of patients

Future Situation(To be System):

1. Automated Reporting
2. Centralized database of patients

Root Definition
A Electronic Health Record Management System is a system funded by Private
Sector and efficiently run by its administrators to give doctors and other staff the
medical and other facilities that enable them to provide patients with treatment
and essential health care services that meet the needs ,quality and cost
expectations of society.

5.Feasibility Analysis :Feasibility analysis is the process of confirming that a strategy,

plan or design is possible and makes sense. This can be used to validate assumptions,
constraints, decisions, approaches and business cases. Hospital Management
Business Case is aimed to provide a self-diagnosis for user staying far from the
hospital. The software product to be produced is a Hospital Management System which
will automate the major hospital operations.

Technical Feasibility:
This is concerned with specifying equipment and software and hardware that will
successfully satisfy the user requirement. The technical needs off the system may vary
considerably, but might include:

a) The facility to produce output in a given time.

b) Response time under certain conditions.

c)Ability to process a certain volume of transaction at a particular speed.

d)Facility to communicate data to distant location.

I have selected PhpMyAdmin because of the following number of reasons.

· Able to handle large data.

· Security.

· Robust RDBMS.

· Backup & recovery.

. Easy to Operate.

Operational Feasibility: It is mainly related to human organizational and political

aspects. The points to be considered are:

· What changes will be brought with the system?

· What organization structures are disturbed?

What new skills will be required? Do the existing staff members have these skills? If not,
can they be trained in due course of time?
At present stage all the work is done by manual. So, throughput and response
time is too much. So we are making sure that the our project has to be reliable
and user friendly

Economic feasibility:

Economic analysis is the most frequently used technique for evaluating the
effectiveness of a proposed system. More commonly known as cost/benefit
analysis: the procedure is to determine the benefits and saving that are expected
from a proposed system and compare them with cost. If benefits outweigh cost, a
decision is taken to design and implement the system. Otherwise, further
justification or alternative in the proposed system will have to be made if it is to
have a chance of being approved. This is an ongoing effort that improves in
accuracy at each phase of the system life cycle.

Hence the engineer will not find any difficulty at the installation time and after
installation user also newer find difficulty i.e. hang, slow speed or slow response
time. One project is compulsory for each person this project is either dummy or
lives. If I am developing a live project then it gives a lot of confidence. It is better
for me and for company because, I am developing a system with out any money.
So every thing is in favor now, I can say the cost of this software is I think
negligible.

8.Requirement Analysis for Electronic Health Record Management System:

Requirements for Dr
Information of patients will only be visible to Drs when one patient data is registered
Requirements of Patients :
Patient can divide into two group :
a) New patients
b) Already under observation/registered patient
Requirements for new patient :
Registration process with unique id(given by EHR system) . Then starting with some
requirements
a) New patient with Present complaints.
b) elaborate history and suffering problem ( History of presenting illness)
c) Past history( Like immunization, travel, family history egulo)
d) Drug history must. Because most of the patients are consuming various
medicines.
e) Previous diagnostic reports.
Then refer him/her to the particular Dr.
g) after Dr checkup all the reports, provisional diagnosis then the Treatment will be
continued.
h)When the patient visits again with his/her reports, those diagnostic reports are also
needed to include in the database for future treatment.

Requirements of under observation patients :

a) whenever they will come with new problems, they will provide their id so the
authority can help them and guide them to particular Dr .

The method of data collection:

Interview Process:
Patients’ Questions : 1..Is it helpful to find your previous health record easily?
2.How would you like to access this system and have you feel any security issues?
Doctors’ Questions:
1. As a doctor, what are you thinking? Is it helpful for Patient?
2. Would you be cooperative with such system?
Hospital Management Questions:
1. From managerial point of view, would this system be of any help to your hospital?
2. Will the hospital’s general work process be disturbed by the system?
3. Is there any internal database in your hospital?
4. Would the hospital like to add some features to this system?
