Accomplish the PNPKI Application Form


 Open the PNPKI Application Form using the Adobe Acrobat Reader DC App/Software
 Individually accomplish said form by clicking in text boxes. Please note that some items with either an
asterisk (*) or “red-colored” boxes are REQUIRED.
 After accomplishing the Form, save it in this file naming convention format:

LastName_FirstName_EmployeeNumber_(your affiliation or current position)

Sample: Balucan_Lauriel_0914344_EPS

Note: PNPKI will not accept handwritten application forms.

Submission of requirements through the DepEd LMS link; r9.lms.deped.gov.ph

click; Deped Commons Teacher

Deretso na po kayo maglogin sa Deped Commons or Sign in with Google or Sign in with Microsoft

Click Home then go to Professional Development


Click PNPKI

Provide Enrolment Key: PNPKI_r9dapitan

click Enroll Me

Click DOWNLOAD PNPKI Form Click Dapitan City Answer all (Personal) Questions Click Submit all and
Finish

Next Upload PNPKI Form Here click Add Submission. You can drag and drop files here to add them. Then
click Save changes.

Note: Wait for the Notifications from the Central Office, Regional Office or Division PNPKI Team, thru an
advisory or memoranda, will be issued for updates and next steps.
Things to prepare
Please prepare the following items.

1. Passport-Sized ID Picture
 Secure a soft copy of clear and decent passport-sized ID picture.
 4.5 cm x 3.5 cm or 1.8 inches x 1.4 inches.
 Pictures should show the whole head up to the shoulders.
 Clear (not blurred) and high-resolution picture (min. of 600 dpi)
 When taking the picture using a smartphone, ensure that the resolution is set at the
highest setting and with ample lighting.
 Image should be in either JPG or PNG format.
 Taken within the last six (6) months
 Stretched images will NOT be considered.
You can download your pictures here, already set to passport-sized with 600dpi resolution. If you are not
comfortable in using this picture, you may provide your own photo following the guidelines above.

2. Secure soft copy of electronic signature


 Using a black permanent marker or any writing instrument with a broad and continuous even ink
flow, write a big sample of your signature on a piece of white paper.
 Ensure that the sample signature is very clear and matches your official signature.
 After making the sample signature, have it scanned in high resolution (minimum of 600 dpi) and
saved in JPG or PNG format.
 White background.

3. Download the free Adobe Acrobat Reader DC


1. To download the free Adobe Acrobat DC, go to this site: https://get.adobe.com/reader/?

2. Before downloading the free Adobe Acrobat DC application, make sure that all checkboxes in the
“Optional Offers” sections are clear or unchecked to avoid ads.
3. Next, click “Download Acrobat Reader” and follow instructions until the application is installed on
computer.

DepEd LMS
1. Login to DepEd Region 9 LMS through this link:

3. Once logged in, go to Site Home.
4. Under Course Categories, select Professional Development.
5. Select Facility for the Submission of the Application Requirement for the PNPKI Digital
Certificate of DepEd Personnel in the Field Offices.
6. Enter enrollment key: PNPK1_r9dapitan
7. Once successfully enrolled, you have now access to the course. You can view video tutorials on how
to upload your PNPKI Form.

How to fill-out the PNPKI Form


1. Click Picture Area to insert new photo
2. ALL CAPS / UPPERCASE
3. Textbox in Red are REQUIRED
4. Use DepEd Email for Email Address
5. Organization: DEPARTMENT OF EDUCATION – REGION 9 – DAPITAN CITY
6. Organizational Unit: DAPITAN CITY DIVISION – DAPITAN CITY DAPITANCITY NATIONAL HIGH
SCHOOL
7. Click signature area to insert signature (2 Signature Areas)
8. Save as PDF File
 Format: Lastname_Firstname_Employee Number_affiliation
 Sample: JUAN_MARIA ANA_6070456_DAPITANCITY.pdf
Put a space on your first name if ever it has 2 or more words
 To those employees who do not have employee number yet, please reach out Division
Personnel/HRMO.
