    RELEASE, WAIVER AND QUITCLAIM

    I, [nam], a Filipino, of legal age, resident of [address] and formerly employed as [position] with [name of
    company] (the “Company”), do state that:

    1) I acknowledge receipt of the sum of [amount in words] (PHP amount in figures) from the Company. I
    further acknowledge that the foregoing sum is in full payment and final settlement of all salaries, wages
    and benefits in whatever form due me under Philippine law and under my employment contract with the
    Company, as well as any and all claims of whatever kind and nature which I have or may have against the
    Company, its directors, officers, employees, agents and/or clients arising from, or in relation to, my
    employment with, and my voluntary resignation from, the Company.

    2)  In consideration of said payment, I do hereby release, discharge and waive any and all actions of whatever
    nature, expected, real or apparent, which I may have against the Company, its directors, officers,
    employees, agents and/or clients by reason of or arising from my employment with the Company. 

    3) I will institute no action, whether civil, criminal, labor or administrative against the Company, its directors,
    officers, employees, agents and/or clients.  Any and all actions which I may have commenced either solely
    in my name or jointly with others before any office, board, bureau, court, or tribunal against the Company,
    its directors, officers, employees, agents and/or clients are hereby deemed and considered voluntary
    withdrawn by me and I will no longer testify or continue to prosecute said action(s).

    4) I shall not:

    a) Directly or indirectly, divulge any financial information relating to the Company, its affiliates or
    clients;
    b) For a period of three (3) years following the cessation of my employment, solicit the business of the
    Company’s clients for my own benefit or that of a third person,
    c) For a period of three (3) years following the cessation of my employment, disclose any information,
    which may be of a confidential or proprietary nature, belonging or relating to the Company.

    5) I declare that I have read this document and have fully understood its contents.  I further declare that I
    voluntarily and willingly executed this Release, Waiver and Quitclaim with full knowledge of my rights
    under the law.
