AFFIDAVIT OF DENIAL
I, GENER ABAGA y GALVEZ, 27 years old, married, and resident of Barangay San
Vicente, San Fernando City, La Union, Philippines after having been duly sworn to in
accordance with law, hereby depose and say:

1. The NBI Regional Office 1 informed that I could not be issued an NBI clearance
because my name is similar to a person who has a pending case appearing in their
records;

2. I hereby categorically state for a fact that I am not GENER ABAGA who was charged
under Criminal Case No. 829 for Grave Coercion pending before Branch 02A of the
Municipal Trial Court of Masinloc, Zambales;

3. We do not have the same middle name and that I have never been to Masinloc,
Zambales, all my life; and

4. I am executing this affidavit to attest to the truth of the foregoing, to deny


categorically under oath that I am not the person charged with a criminal offense at
the aforementioned court of justice, and to request the NBI to issue the appropriate
clearance in my favor.

IN WITNESS WHEREOF, I hereby affixed my signature this August 12, 2021 at


San Fernando City, La Union, Philippines.
