AFFIDAVIT OF WAIVER

I, MARILOU PINE y TADEO, 44 years old, married, Filipino and a resident of Sta.
Maria, Moncada, Tarlac, Philippines, after having been sworn to in accordance with law,
hereby depose and say:

1. I am a sister of the late JUN MENDOZA TADEO who died at Koror, Palau on July
15, 2021;

2. My brother JUN MENDOZA TADEO died without issue meaning he died as single
without a spouse and children;

3. I, together with our sister, ALICIA PERALTA y TADEO, are his legal heirs
considering that our parents are already dead also, hence, we are his next of kin;

4. Since my brother is an OFW, his heirs are entitled to a death and burial benefit to be
given by OWWA;

5. I hereby waive, renounce and relinquish in favor of ALICIA PERALTA y TADEO


the full and total amount of the death and burial benefit claim for our late brother,
JUN MENDOZA TADEO; and

6. I am executing this affidavit to attest to the truth of the foregoing and for whatever
legal purpose it may serve.

IN WITNESS WHEREOF, I have set our hands this September 6, 2021, at San
Fernando City, La Union Philippines.
