PHOTOS CONSENT AND RELEASE FORM

For good and valuable consideration, the receipt of which is hereby acknowledged, I,
_____________________________________[name], voluntarily hereby grant Focus Data Analysis and its
agents, permission to use my photography in any and all of its collections. I understand and agree that
any photograph/s using my likeness will become property of Focus Data Analysis and its agents. I hereby
irrevocably authorize Focus Data Analysis to edit, alter, copy, exhibit, or distribute photos and waive the
right to inspect or approve the finished product. Additionally, I waive any right to royalties or other
compensation arising or related to the use of the photograph/s.

Terms of use: The general purpose of this collection is to generate unbiased AI Face recognition models,
hence contributing for ethical AI. The images collected for this project, will be used with the sole purpose
of training data for AI and Machine Learning models in the Computer Vision area; no publicity/advertising
(i.e., public exposure) usage will be given to these images.

I hereby hold harmless and release and forever discharge Focus Data Analysis and its agents from all
claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or
any other persons acting on my behalf or on behalf of my estate have or may have by reason of this
authorization.
