WRITTEN WARNING NOTICE

The following incident has been brought to the company’s attention for appropriate disposition.

We received a report that allegedly you failed to attend the mandatory Toolbox Meeting today at
7:00A.M.

Please be reminded that it is your utmost duty as a QC/QA Engineer to attend the mandatory
toolbox meeting as mandated by DOLE for you to be well informed of the policy being imposed
currently at site especially with regards to the health and safety concerns which will be headed
by the safety department.

Below is the policy which concerns the incident you are involved in:

Article C. Gross and habitual neglect by the employee of his duties.


Section 63. Simple negligence in the performance of assigned duties or failure to observe the
degree of diligence demanded by the situation.

First offense for the abovementioned violation is tantamount to written reprimand or 8 days work
suspension.

In addition, such acts also fall under “Section B. Gross and habitual neglect by the employee of
his duties”, under Article 297 (As amended) of the Labor Code which fall under Just Causes of
“Termination by Employer.”

Due to the abovementioned infraction, you are hereby given a Written Warning. Any
Repetition of the same shall be strongly dealt with.
