Good day!
We’re from ABM 11 Y2-11 - Group 5 and we will conduct a survey for our research
entitled, “A Correlation Study of Anxiety to the Academic Performance of Senior High
School Students.”
Please read some information that we provided below for an enhanced understanding of
our research and answer our questions easily. In this regard, we prepared this questionnaire to
gather the data needed for our research. Kindly answer the following items in all honesty. We
guarantee that your answers will be confidential information and can only be used for our
research. We will keep your identity hidden for your privacy as well.
Anxiety is a normal reaction of our body especially if there’s an occasion, random thoughts, or
unexpected event that will make you feel nervous or anxious. Academic anxiety is one of the
problems that many students are facing nowadays. A lot of students experience nervousness
because of the sudden recitation, reporting, taking exams from difficult subjects, deadlines of
tasks, and such.
