STUDENT ACKNOWLEDGEMENT FORM

I, the undersigned Student-Grantee, is enrolling or currently enrolled in the Pamantasan ng Lungsod ng


Marikina (PLMAR) and qualified under its existing admission & retention policies and hereby availing the
MARIKINA-FHE Scholarship, do hereby acknowledges its understanding and agrees to the following
implementing guidelines;

1) Coverage of the Scholarship Grant- The grant of MARIKINA-FHE Scholarship covers the tuition fee
and other school fees (TOSF), namely the Registration Fee, Library Fee, Identification Fee, Laboratory
Fee, Athletic/Cultural Fee, Computer Fee, Medical/Dental Fee, School Publication Fee, Examination
Fee, Developmental Fee, Insurance Fee, and Student Organization Fee, and shall be entitled to a
maximum grant per semester;

2) Eligibility- The Student-Grantee must meet the admission and retention policies of PLMAR, with no
previous undergraduate degree and overstayed at the college level (e.g., maximum residency rule plus
one-year grace period as provided by law) in order to be eligible to the grant. Students who are not
eligible shall be charged tuition and other school fees;

3) Good Academic Standing- Students should maintain good academic standing and should fulfill the
return service requirements set by PLMAR. The return service requirements should be met within the
prescribed period of completing the degree program.

4) Voluntary Opt-Out- Students who voluntarily opt-out shall be exempt from the return service
requirement. Students who voluntarily contribute to the institution will still be covered by the return
service requirement.

5) Third to Fourth-Year Students- Incoming third to fourth-year students with previous balances in
tuition and miscellaneous fees need to settle their accounts in order to avail of the Free Tuition under
this program;

6) Shiftees and Transferees- Shiftees and transferees, shall pass all the subjects taken from the former
school and shall qualify to the admission and retention policy of PLMAR;

7) Returning Students- Returning students and other students who went on a leave of absence and passed
all the subjects taken shall undergo the re-admission and retention policy of the college;

8) Effect of Dropping- Students who will be covered in the free tuition and will decide to drop within the
semester due to reasons unexcusable to the PLMAR, have to pay the existing tuition, miscellaneous
fees, and other accounts left as per agreement between parents, guardians, students, and school;

9) Other expenses- Other education-related expenses such as cost of books, school supplies, uniforms,
reproduction of materials, electronic devices necessary for education, and other expenses as
determined by the Institution are not included in the free tuition of the college, including the conduct
of drug test and chest x-ray examination once every year as mandated by CHED MEMO NO. 64 s 2017.
Only the first copies/printouts of school ID, library ID and student handbook shall be free; any
subsequent request by the student will be charges ;

10) Continuing compliance- The Student Grantee understands that the grant of the scholarship shall be
conditioned on his/her compliance to the relevant policies of PLMAR, and the CHED;
