For your DENTAL and MEDICAL clearance:


1. Download the CG MEDICAL PROCESS FLOW here. Follow the instructions
stated therein.
2. Join the CG Medical Group specified in their process flow. Instead of PCG ABT,
write CGWCEISC ENTRY EXAM PASSER and the date of your exam.

How to PACKAGE your documents PROPERLY:
1. Prepare one (1) expandable envelope and eight (8) sliding folders. Blue for
males, yellow for females.
2. Tabbings should be placed on a clean, blank paper and not pasted directly on
your documents.
3. All pictures MUST be COLORED in all your folders.
4. Place your folders, complete with your documents, in the expandable folder.
Your COVER PAGE should be a clean paper with your printed name on it. Format:
FIRST NAME MIDDLE INITIAL LAST NAME.
5. PRINT your complete name on a clean sheet of paper and paste it behind the
envelope (the flat side of the envelope).
6. ORIGINAL copies of the aforementioned requirements should be placed in one
(1) folder. Seven (7) folders of authenticated or certified true copy shall be
prepared.
