1. Main Hazard & Control In Your Job

H2S gas, moving vehicles, open trench, underground facility, working at height, moving
heavy equipment, loading, unloading, temperature, dust, confined space, working near flow
line, excavation, slipping etc.

3. Process facility & hydrocarbon area

Process facility

The area within the boundary fence of any hydrocarbon processing facility, including:
• Gathering and Pumping stations.
• Terminals.
• Other fenced hydrocarbon storage or processing areas.
In addition the level area around a hydrocarbon production or DWD wellhead (well
location) is classed as a Process Facilities for permit classification purposes, when the
well is flowing

Hydrocarbon area

Areas outside the boundaries of Process Facilities where hydrocarbons are, or have been
present. These include:

• 10m area of inter field gas line


• 25m area of main oil line or SOGL gas line
• 50m area of outside of fenced process facility
• 50m area around well pad
• 50m area around flow line which has flanged or joints
• Areas within 100m of a drilling rig for work by non drilling personnel.
• Area around a well site (well location) when the well is not classed as a Process Area

4. H2S emergency escape procedure

❖ Stop work
❖ Stop breathing and wear breathing apparatus
❖ Kick ground to find wind direction
❖ Go to near assembly point against the wind direction
❖ Call emergency number for help

5. HEMP for job HSE plan

HEMP stands for hazard and effects management process it includes the steps of identify,
assess, control & recovery

IDENTIFY
Understand the hazards and threats associated with the work & check
❖ What are the impacts to people, assets, environment or reputation?
❖ Are there 'process' hazards or hazards associated with the material being
handled involved in the work?
ASSESS
Recognise the risks & check:
❖ What is the likelihood to cause harm?
❖ Is the risk involved as low as reasonably practicable -ALARP?
CONTROL
Decide upon the barriers and control necessary to manage the risk thus providing a
safe working environment. Check
❖ What is required to prevent the likelihood of causing harm from carrying out
the work being realised?
❖ Are there any safer alternatives either in terms of timing, or the intended
method of performance of the work?
RECOVER
Decide upon which necessary measure will be taken to bring the situation normal
after occurring the consequences.

A permit holder can hold 4 Class “B” permit at a time in one location or only
one Class “A” permit.

9. How many permit an area authority can validate?

Total 14 permit and limit of 6 Class “A” permit

10. Why authorization period for class “A” permit is 3 days?

Authorization period for Class “A” permit is not 3 days

11. If your friend falls by H2S what will you do?

I will go to nearest assembly point and call emergency number for help.

12. What is NORM and where it found?

NORM stands for natural occurring radioactive material. It is usually found in


the sludge in tank, scaling in the pipeline.

13. Validation & revalidation

Validation:
This is the process by which area authority signs the green copy of the permit for
work after examining the work site and allowing the permit holder to perform the
work that day.
Revalidation:

❖ The Re-validation process is exactly the same as the Validation and acceptance
process.
❖ If the Area Authority changes during the work period the new Area Authority is to Re-
Validate all Permits in his area. This is done by signing ALL the green copies at the
Permit Issue Point.
❖ For Class ‘A’ Permits he must examine the worksites and sign the Worksite Copy of
each Permit.
❖ For Class ‘B’ Permits the Worksite Copy is to be signed by the new Area Authority on
the return of the Worksite Copy to the Permit Issue Point.
❖ After Re-validation of the Permit, the Permit Holder is to satisfy himself that the
Worksite is safe, and sign to accept the Permit.
❖ Once all spaces for Re-validation have been used, a new Permit is required.

14. Working procedure in process facility

1. 1st I need a validated permit, accept permit & put permit in the permit rack.
2. Keep tag marker on the location board
3. Enter the name of the worker in the entry register
4. Keep H2S card in H2S rack
5. Make sure that nobody has the source of ignition with him
6. Keep H2S monitor in on condition
7. Identify the assembly point & emergency exit gate
8. Arrange proper TBT & explain jobs to crew

15. How many zones are there & types?

Four zones 0, 1, 2 & non-hydrocarbon

Zone 0:

Areas where hydrocarbons (explosive atmosphere) have been continuously present for a
long time.

Zone 1:

Areas in which hydrocarbon (explosive atmosphere) occur regularly in normal operation.

Zone 2:

Areas in which hydrocarbon (explosive atmosphere) not likely occur regularly in normal
operation.

Non hydrocarbon

An area where hydro carbon never be present.


16. How to convert zone 0, 1 to zone 2?

The process system(s) which resulted in the area being classified as Zone 0 or 1 must
be shut down, isolated and de-pressurised, so that the hazard which caused the area
to be classified as Zone 0 or Zone 1 is removed for the period of work.
If specifically approved by the Responsible Supervisor, a pressurized habitat may be built
to create a sub-environment within an otherwise Zone 1 area where Hot Work may be
done without creating a fire hazard. The habitat must be examined before use and
approved by the Area Authority, Responsible Supervisor, and Production Coordinator.
Continuous gas monitoring shall be required

Isolation means separation or immobilization of process and equipment.

21. What is gas testing, who will carryout & what is the procedure?
Gass testing is the process to make sure that the worksite is free from any explosive
and toxic gass.
An authorize gass tester perform this, usually by area authority.
In this we use a multi gass detector by placing the detector the place by which gass
is to be tested. Present gasses there will be appear on the screen of the detector.
Take the sample different locations.

It is permission to enter in the confined space

27. What is permit?

The permit is a written document which authorises certain people to carry out
specific work at a certain time and place. The permit sets out the precautions needed
complete the work safely.

29. Changeover of permit holder?


Within a Process Facility, the new Permit Holder must obtain the Area
Authority’s approval to change and sign both copies of the permit. At remote
locations, where he is unable to visit the Area Authority, he must speak to the
Area Authority, and sign the worksite copy when he takes over. The Area
Authority will change the name of the Permit Holder on the green copy, which
will be signed when the Permit Holder returns to the station. The outgoing
Holder is not to leave the worksite until he has fully briefed the new Holder and
the new Holder has signed to accept the Permit in his presence.

30. If your contractor asks you to hold too many permits what will you say?
Sorry I cannot accept more than 1 CLASS”A” permit or 4 CLASS”B” permit at a
time.
