4. PAST AND PRESENT COMPLAIN:-


According to patient:-
He said that he is having some physical illness, family members have brought him
here and he wants to go home.
According to informant:-
According to his mother, he was suffering from this illness from 8 months back and
was admitted in Mental Health institute, S.C.B.M.C.H, Cuttack. He took medications
and was discharged from hospital after getting well. He discontinued medication and
the illness aroused again. There was decreased sleep, laughing without reason. He was
always in an aggressive mood, beating and biting other individuals.
5. PROVISIONAL DIAGNOSIS:- Schizophrenia
6. AIMS AND OBJECTIVES:-

client’s behaviour as he co-operated me. I shall again interview the patient in same time on
next day.

TECHNIQUE USED:-

I first developed good interaction and rapport with the client and his family members by
conversation method.
