NARRATIVE REPORT
Causes of Student’s Failure
S.Y 2020-2021

This is to report the poor academic performance of Harol S. Negro, a Grade 9- Mabini
student. It is sad to note that he has ‘retained’ status for the current school year due to the reason that
he received failing grades in six of his academic subjects, namely; Filipino, English, Mathematics,
Science, AP and ESP.
During the start of this school year with modular distance learning modality, it seemed to
favor his side as we can see how engrossed he was but as the grading period advances, he started to
show lack of interest in answering his modules. In fact, he failed to submit his performance tasks and
summative tests in the aforementioned subjects in the second half of the year.

In response to his passive action, we conducted home visitation during our vacant time. We
spent some time in consulting his parent regarding with the behavior of Harold. We discovered that he
went to Babak before the start of third grading period that’s why we were wondering why his SLMs
were not claimed. We tried to contact him thru his messenger account but he did not reply yet we still
encourage him to continue schooling until he finishes this school year but we didn’t see him
responding.

We have extended our patience not just once but many.

NARRATIVE REPORT
Causes of Student’s Failure
S.Y 2020-2021

Along with the utilization of a new learning modality, learners also have to adjust especially
that there is no face to face interaction for this school year due to the emergence of an unprecedented
crisis. Being in the field, we have to likely devise a strategy adaptive to this new modality. We see it
most difficult for learners who need more guidance in the instruction phase.

We tried our very best to extend efforts in reaching out these learners but due to the
overlapping paper works, we seem to lack time for this but we really inserted few of our time in
conducting home visitation, consultation or even for dialogue. We tend to adjust and extend our
foremost effort to Normie Jr. G. Dinopol with immense patience. As their subject teachers, we could
recognize that he has needs to be addressed. In order to cope up with this, we conducted
dialogue/consultation with his parents. He was adaptive from the start but in the midst of this school
year, he started to show lax behavior. In fact, he never claimed his modules on time and as a result, he
had limited time in answering. And so, he got low scores in his summative tests and performance
tasks. In most case, he failed to comply his summative tests and performance tasks that is why he had
failing grades in four of his academic subjects.

We can say that we have already done our part. We have shown our concern to him but he
remained unresponsive.

NARRATIVE REPORT
Causes of Student’s Failure
S.Y 2020-2021

Not all learners have the ability to cope up with this new normal. Some are driven by their
indolence attitude. This new learning modality may have been a favor on their side only if they are
willing to conform.

This report is about the poor academic performance of Adolfo Jr. M. Maglasang whose
grades in five of his academic subjects were failed, to wit; English, Mathematics, Science, EsP and
TLE. In every grading period, we didn’t fail to monitor this student. We were very happy to see his
outputs in the first half of this school year but in the second half, he eventually gave up. As part of our
duty, we still encourage him to continue his studies. We conducted dialog with his parents several
times just to uplift him. But he was never responsive to our efforts. In effect, he will be retained in
grade 8 level.

NARRATIVE REPORT
Causes of Student’s Failure
S.Y 2020-2021

This is to report the failure of Regine G. Calamba in three of her academic subjects, to wit;

English, Science and EsP. She was consistent in answering her modules in the first and second

grading but when she went to Davao prior to the start of the third grading, she failed to comply her

summative tests and performance tasks. We didn’t fail to do our part as teachers. In fact, we tried to

reach out her via messenger and we also conducted a dialog with her aunt. She remained

noncompliant.

We may have extended our consideration to her only if she’s able to show interest in her

academics. She reached us out when the fourth grading almost ended. We gave her chances yet she

never showed willingness.
