You are currently in your pediatric rotation. At the end of your first clinical day, you
already have all information about your patients whom you need to provide the necessary
nursing care. You were assigned to three children who were diagnosed with Acute
Gastroenteritis. Use the following data for planning appropriate nursing interventions for
each patients.

PATIENT A

Name: J.P. Age: 2 years old Gender: Female Birthday: November 26, 2017
Address: San Juan Accfa, Cabanatuan City Citizenship: Filipino Religion: Catholic
Date of Admission: March 4, 2020

Vital Signs upon Admission: BP: 100/70mmHg Respiratory Rate: 35cpm


Pulse Rate: 120 bpm Temperature: 37.2 C

Chief Complaint: LBM and Vomiting

Admitting Diagnosis: AGE with some dehydration

History of Present Illness : 2 days PTA, the patient had vomiting for 3 times and passage of
watery stool.

Past Medical History: Patient gets seasonal cough and colds at times but never serious.
Family Medical History : (+) Hypertension- Paternal side

Personal and Social History : Pedia Patient A. Chuchay, a 4 year old, lives with her Lola . Her
grandmother is present only during afternoon visiting hours because she must take care also
herhusband who suffers also from numerous health problems.

Medications: Paracetamol, Erceflora, Metronidazole

PATIENT B

Name: L.D Age: 1 year & 7mos. Gender: Male Birthday: August 24,2018
Address: Aliaga, Nueva Ecija Citizenship: Filipino Religion: Roman Catholic

Respiratory Rate: 32 Pulse Rate: 116 Temperature:36.4’C

Chief Complain: LBM Admitting Diagnosis: Acute Gastroenteritis

History of Present Illness : Three days prior to admission, according to his mother the patient
experienced 3 days watery stool and 2 episodes of vomiting. The patient immediately rushed
to the hospital.

Past Medical History : According to the mother of the patient, the patient was first hospitalized
due to Acute Gastroenteritis when he was 8mos old while on formula milk.

Family Health History :(+) Hypertension- Father side.

Medications : Zinc Oxide, Cefuroxime, Racecadotril

Personal and Social History : Pedia Patient B. Boknoy, a 3 year old, lives with his mother.
Since his admission yesterday, Boknoy’s mother has stayed at his bedside and provided most
of her son’s care.

PATIENT C

Name: R.J.M Age: 3 !/2 yrs old Gender: MALE Birthday: April-26-2017
Address: Talavera, Nueva ecija Citizenship: Filipino Religion: Roman Catholic

Date of Admission: 2-24-2020


Vital Signs upon Admission: Respiratory Rate: 25 Pulse Rate: 90 Temperature: 36.1’C

Chief Complain: Vomiting, LBM

Admitting Diagnosis: AGE with moderate dehydration


History of Present Illness: Prior to admission, the client was vomiting 8x and lbm x10. His stool
was watery at first they go to the barangay health center and gave them a medication for lbm
but after taking the medication the client was still vomiting and still experiencing lbm. Tthe
family decided to rush the client to this hospital.
Pedia Patient C. , a 3 and 1/2 year old, lives with her parents. Both parents are able to visit
only during afternoon visiting hours when they leave work.

On your first clinical day exposure, you were assigned to care for these patients with
Acute Gastroenteritis. Each of these patients is unique. Although they have same medical
diagnosis, their needs and and your nursing care will differ dramatically. These differences
stem from a variety of factors, one of which is each patient’s stage of growth and
development.

GUIDE QUESTIONS :

1. Identify the factors affecting the nutritional needs of each patients. At specific age of each
patients, how are their nutrional needs and eating patterns vary? How will you attain
rehydration with the advised diet given to them?

2. What are the risk factors you can identify with the above given scenario?

3. Formulate at least 3 NCP for each patient according to its most priority. Give rationale for
each nursing intervention.

4. How will you measure the INTAKE and OUPUT of each patient accurately?

5. Make discharge plans for each patient including health teachings to the mother or guardian
or any relatives of patient.
