READ THIS DOCUMENT THOROUGHLY
STEP 1: SF-181
Print the SF-181 https://www.opm.gov/forms/pdf_fill/sf181.pdf
Fill in the name (LAST, FIRST, Middle Initial) AS IT APPEARS ON THE SOCIAL
SECURITY CARD in the top left box of the SF-181. DO NOT ADD ANYTHING TO
THE DEBTOR NAME!!! Add your numerical birth MONTH & YEAR only.
It is MANDATORY TO MARK THE BOXES FOR WHITE and
AMERICAN INDIAN. Also mark whatever applies to your lineage.
Fax: Fax the cover letters below, the SF-181 and the following attachments (in
order listed) to the Office of Management and Budget (OMB) 1-202-395-3888
AND to Social Security Administration (SSA) Office of International Affairs 1-877-
385-0645. Depending on how your computer prints the Pope’s Apology, you will
have a total of 12 or 13 pages. Wait for the fax confirmation page. That IS
your record for publishing!
On the Fax Cover Sheet, provided below, do NOT ask for confirmation of receipt
for your fax!! We have made fax cover sheets for you; add the name AS IT
APPEARS ON THE SOCIAL SECURITY CARD, your fax #, the # of pages and
the date. You can also record a video of yourself faxing your SF-181 to the OMB
and SSA of International Affairs and publish it.
Children: If you have children under age 18, you must do each child’s SF-181
individually. Include the 5 supporting documents. You must have fax
confirmations for each one of your children.
It is Mandatory YOU Publish Your Documents After confirmed fax:
To publish you need a fax confirmation results page or a video. Publish your fax
confirmation, cover letters, SF-181, and attachments video on your Facebook
page, Scribd, Twitter, Instagram, Linkedin, email it to family, friends, associates
and all other forms of electronic tracking and media outlets, as evidence of your
commercial status for the record. Make sure the Human Resources Department
at your job & Union representative, physicians, attending schools/universities for
you and your children also receive what you published on the internet.
Notify DMV:
Send a copy of everything you faxed to the OMB and SSA Office of International
Affairs, including the confirmation pages, to the local DMV via Registered Mail.
Notify Voter Registration:
If you have registered to vote, you need to send everything you faxed to the
OMB and SSA Office of International Affairs, including the confirmation pages, to
the local Voter Registration Office via Registered Mail. Take a photo of the
envelope and contents and publish it along with the proof of delivery. This is
mandatory as Voter Registration relays your race and ethnicity to law
enforcement.
How to Send Registered Mail: When sending Registered Mail, write “$55.00”
in the “Value” section on the white Registered Mail paper. It does NOT COST
$55.00 to send Registered Mail! You will receive a green signature return
receipt card as proof of delivery once your mail is delivered.
DO NOT LET THE POSTAL CLERKS BULLY YOU INTO
SENDING CERTIFIED MAIL!
Certified Mail = Domestic
Registered Mail = International
Disability: If you collect income from Disability, add the SF-256
(https://www.opm.gov/forms/pdf_fill/sf256.pdf) to the SF-181, and the 5
supporting documents, and fax it to OMB and SSA Office of International Affairs.
Then, take everything you faxed along with the fax confirmation pages into Social
Security Insurance (SSI) and have them update your file. THIS WILL NOT
AFFECT NOR CAUSE YOUR SSI BENEFITS TO BE AFFECTED
NEGATIVELY.
Welfare/Food Stamps/Housing: If you receive any of these benefits give a
copy of your SF-181 to your case worker and have them add it to your file.
