    RELEASE, WAIVER, QUITCLAIM

    AGREEMENT
    I, _________________________-, of legal age, Filipino, with postal address at
    ______________________, after being sworn to in accordance with law, depose and state:

    1. That I hereby acknowledge the receipt of my full refund amounting to


    ____________________(PhP ________________) which is in full and final
    satisfaction of my refund claim from the Corporation. I hereby further waive
    and relinquish all my rights that I or my heirs, assigns and/or successor-in-
    interest/insurer may have to any further amounts/claims that may be due
    me for my purchase; That I hereby declare that I have no further claims
    whatsoever against the Corporation and its directors, officers, employees,
    subsidiaries, affiliates, stockholders, successors, assigns, agents and
    representatives; That I hereby irrevocably release and forever discharge all of
    them from any and all claims, demands, cause of action of whatever nature
    arising out of my employment with the Corporation and hereby withdraw
    any claim or lawsuit, if I have any, now or in the future, against the
    Corporation and its directors, officers, employees, subsidiaries, affiliates,
    stockholders, successors, assigns, agents and representatives.

    2. That I warrant that I will not at any time, in any manner whatsoever, directly
    or indirectly commit any act prejudicial to the interest of the Corporation and
    its directors, officers, employees, subsidiaries, affiliates, stockholders,
    successors, assigns, agents and representatives arising out of or related to my
    purchase from the Corporation.

    3. That I also acknowledge that in case I violate any of the undertakings


    contained herein, in any manner and for any reason whatsoever, I will hold
    the Corporation and its directors, officers, employees, subsidiaries, affiliates,
    stockholders, successors, assigns, agents and representatives, and owners,
    and legal counsels free and harmless from any and all liabilities, damages or
    injuries and shall fully indemnify and compensate the Corporation for any
    and all liabilities, damages or injuries incurred or caused by such violation or
    in any manner connected or related thereto.

    4. That I further acknowledge that I shall exert and exhaust all efforts to enforce
    compliance of the foregoing obligations and to prevent or cure any breach or
    threat of breach.

    5. Finally, that I declare that I have read this Release, Waiver, Quitclaim
    Disclosure Agreement, and that I execute the same willingly and voluntarily
    and with full knowledge of my rights under the law and its consequences.
