CREATING ONLINE SURVEYS FEEDBACKS FORMS

Google offers one of the easiest ways to gather user’s feedback. Google
forms allow your audience to answer a set of questions you have set. These can
be used for surveys, feedback, online registrations, and customer care support.

1. Open your browser and go to drive.google.com

2. Sign in or create an account.
3. On the left-hand side of your My Drive page, click New>More>Google
Forms
4. You will be taken to Google Form’s interface
5. You may now fill out the form questions:
A. Question Title- Includes the question that will be answered by
your audience.
B. Help Text- Creates a subtext under the question to give more
information about a question.
C. Question Type- changes the type question according to your
preference. These questions include:
i. Text – can be answered in a short text.
ii. Paragraph Text – can be answered in a long text.
iii. Checkboxes – a question that can be answered with
multiple answers in a set of options.
iv. Choose from a list- similar to a multiple-choice question
but options are revealed in a drop-down list.
v. Scale- a question that can be answered with a numerical
range. vi. Grid- a question that contains sub questions with
similar options as shown;
• Date- a question that can be answered with a
specified date
• Time- a question that can be answered with a
specified time
D. Advanced Settings: Contains more options for the specified
question type.
6. Edit the Confirmation Page option at the bottom as you see fit.
A. Show link to submit another response- allows the user to answer
the same form again.
B. Publish and show a public link to form results- allows users to see
the summarized results for your survey.
C. Allow responders to edits responses after submitting- allows users
to back and edit their answers.
7. Click the Send Form button. The Send Form dialog box will appear these
options as you see fit.
a. Link to share- contains the URL that you can share on your
website.
b. Embed- contains an embed code to attach to your HTML.
c. Short URL- generates a shorter URL necessary for limited spaces
like sharing on twitter.
d. Share link via- allows you to share the link to Google+, Facebook,
and Twitter.
e. Send form via email- allows you to share the form via email.
f. Add collaborators- Allows others to edit your form, necessary
when working on groups.

1. The easiest way to view the result is viewing a summary of the responses. Click
on Responses> Summary of Responses.
2. You will be taken to a Summary of Responses page. This is particularly useful
for viewing your results from time to time.
However, after a set amount of time, you may want to use this data in a
spreadsheet and eventually create your own charts.
3. To use the data collected through responses, click View Responses button
found on the upper part of the page. Analyzing your Google Forms Result
4. Choose if you want to create a new Google Sheet as the destination of your
response results or if you want to put it on an existing Google Sheet.

What’s More

Create a feedback form and analyses viewers’ responses through

data analytics to evaluate the performance of the ICT project using available
monitoring tools and evaluating techniques.

Include the following to your existing web page:

Video presentation feedback form

Create a feedback form using any web-based survey data analysis
software. Insert a link below the video presentation.

Teacher may suggest:

Survey questions and answer styles to be used by the group
Selection of survey respondents

Data analysis of survey respondents

After a period of time or number of targeted respondents, the group needs
to generate a report on the response of your viewers. Interpret the gathered data
by using graphs and then discuss gathered results. Use a separate web page to
show the generated report.

At the end of the week, present data analysis to your teacher for evaluation.

REFLECTING ON THE NATURE OF ICT AND THE MANNERS BY WHICH

THE LEARNING PROCESS HAS CHANGED YOUR WORLDVIEW
REFLECT ON THE ICT LEARNING PROCESS COVERING THE TOPIC OF:

ICT the self and society.

We learn such things through the use of ICT.
Through it we can get news, and information or updates about the things
that are happening in our society.
We use ICT to interact and communicate to our relatives and friends,
even though we are far from each other.

It is an excellent program that helped students learn their competencies

and lessons.

Through the use of online learning tools, with this students acquire lecture
which was used in previous lessons, answers quizzes through online, and do
activities that isolated with the school work. The Integration of ICT in the Senior
High School program molded us students to be well guarded and equipped from
the continuous evolution of Technology.
LEARNING PROCESS OF ICT HAS CHANGED YOUR WORLDVIEW

Information and Communication technology or ICT is an essential in our

lives It helped us in our education as well us .This has introduced has the easiest
way of learning for both student and the teachers. If we observed are well educated
about the use of ICT We surely can lifted up our society, it could be a great way to
apply the things that we learn to improve our society.

THE POSITIVE AND NEGATIVE IMPACT OF ICT IN OUR LIVES:

As it is known from time immemorial that everything in life is like the two
side of a coin, there is always a positive and negative side of every phenomenon.

POSITIVE IMPACTS OF ICT CAN HAVE POSITIVE IMPACTS ON PEOPLE:

Access to information:

Possibly the greatest effect of ICT on individuals is the huge increase in

access to information and services that has accompanied the growth of the
Internet.
NEGATIVE IMPACTS OF ICT ON PEOPLE

Job loss: One of the largest negative impact of ICT can be the loss of the
person’s job.
Reduced personal interaction: Being able to work from home is usually
regarded as being a positive impact of using ICT but, there can be negative
impact as well.
Reduced physical activity: A third negative effect of ICT is that users may
adopt a more sedentary lifestyle.

NEGATIVE IMPACTS OF ICT ON ORGANIZATION

Cost: the cost of using ICT may cause a number of problems in

organizations. A lot of ICT hardware and software is expensive, both to
purchase and to maintain.
Competition: this is usually thought of as being a good thing, but for some
organizations being exposed to greater competition can be a problem.
Security: this is always a problem for any organization that uses ICT. Data
must be kept secure, Internet connections must be protected from attack,
new viruses and other forms of malware are released nearly every day.

ICT CAN HAVE POSITIVE EFFECT ON THE SOCIETY

Probably the largest impact that ICT use has on society is allowing members
of society to have greatly increased access to information. This can have
numerous positive effects, such as:
increasing opportunities for education
improving communication
allowing people to participate in a wider, even worldwide, society.

The positive impact of ICT on education

On the positive side, the use of ICT in education can provide opportunities that
might not otherwise exist, such as:
distance learning, where students can access teaching materials from all
over the world,
the ability to perform ‘impossible’ experiments’ by using simulations,
the possibility for students to have individual learning programs within a
topic, rather than everybody having to do the same thing at the same time
at the same pace. More able students can be given more challenging work,
less able students can access remedial lessons.
What is It
Instruct the students to make a reflection paper using an ICT platform or
application of their choice to explain their journey of the learning process in making
use of ICTs as an empowerment tool.

Guide Questions
1. How did this subject change the way you view ICT?
2. Are they tools to be used to achieve higher goals?
3. After going through the lessons and visiting online sites, reflect on how they are
currently being used. Do you agree or disagree with it?
4. Based on your reflection, do you have recommendations regarding ICT uses in
the country?

Following is an example of a Rubric that may be used for the reflection paper.
