Login your DepEd Email
Click Authorize
Click Site Home
Click Professional Development
1. Look for Facility for the submission
of application…..
2. Click Access
3. Input the code: PNPK2_Sorsogon
1. This message will prompt if you
have successfully entered the key.
2. Scroll down to see the course
overview.
Click Download PNPKI FORM
1. Make sure that you are
using Adobe Acrobat
Reader. Otherwise, you
cannot affix your photo.
2. Information encoded
must be in UPPERCASE.
3. In Organization / Agency /
Company, please encode:
DEPED – SCHOOLS
DIVISION OF SORSOGON
and the Name of your
School in Organizational
Unit.
1. Photo size is 35 mm x 45
mm (w x h).
2. After encoding save the
file.
1. On course overview, click
SDO Sorsogon.
1. Click PNPKPI Questionnaire
1. Click Attempt quiz now.
2. Input all the required
information.
1. Click Submit all and finish.
2. A confirmation message
will show, click submit all
and finish.
After submitting the quiz,
click Upload PNPKPI Form
Here.
Click Add submission.
1. Click this file icon to
upload your PNPKPI Form
OR:
2. You can Drag and Drop
your file here.
1. If you click the File Icon,
this file picker window
will pop up, click Upload a
file.
1. Click Choose File Button.
1. Locate and click your
PNPKI PDF Form.
2. Click Open.
1. Make sure that you have
selected the correct file.
2. Then Click Upload this
file.
1. Click Save changes.
Congratulations! You have
successfully uploaded your
PNPKPI Form. The progress
bar should be at 50%
because your uploaded file
will be verified by personnel
section.
