Calories (kcal): 25 x 74 kg = 1850 kcal/day
Enteral Formula: Glucerna 1.2 cal (cal/mL =1.2)
1850 kcal/1.2 = 1542 estimated volume | 1542 mL/22 hr feed = 70 mL/hr
Check Work: 70 mL/hr x 22 hr feed = 1540 mL Glucerna x 1.2 = 1848 kcal/day
Free water: 1 L Glucerna 1.2 = 805 mL free water
1540 mL x 0.001 = 1.54 L x 805 mL/L = 1240 mL free water
(1 kcal = 1 mL H20) 1848 mL total – 1240 mL free water = 608 free water required
608 mL/22 hr feed = 28 mL/hr water

multiple snacks daily
BMI: kg/m 2 → (202 lbs/2.2 =92 kg) | 6’2’’ = 74 in x 2.54 = 188 cm x 0.01 = 1.88| (1.88)2= 3.53
m 2| BMI = 92 kg /3.53 m 2= 26 kg/m 2
Evaluate: 26 falls within overweight category and since no weight loss, lower end of range (25-
30 kcal/kg)
Calories (kcal): 25 x 92 kg = 2300 kcal/day
Protein: 1 g x 92 kg = 92 g protein/day
Enteral Formula: Jevity 1.2 cal (cal/mL =1.2)
2300 kcal/1.2 = 1917 estimated volume | 1917 mL/22 hr feed = 87 mL/hr

Calories (kcal): 20+ lb wt loss therefore higher range (25-30 kcal/day) 144/2.2 = 65 kg
65 kg x 30 = 1950 kcal/day
Protein: (1.25 – 1.5 g/kg) 1.5 g x 65 kg = 98 g protein
98 g protein/1950 mL x 100 = 5% protein (98 x 4 =392 kcal, 1950 – 392 = 1558 kcal remaining)
Lipids: 65 kg x 9 = 585 kcal/2 =293 mL (20% IL) (1558 kcal – 585 = 973 kcal remaining)
Dextrose: (3.4 kcal/g dextrose) 973 kcal/ 3.4 = 286 kcal/g dextrose
286/1950 mL x 100 = 15% dextrose
Summary: 5% protein, 15% dextrose, 293 mL 20% IL TPN
4. TPN Patient: age 47, 5’11”, SBO w/ UBW 175 lb 5 days, current wt 173 lbs
Calories (kcal): decreased nutrient intake over 5 days, therefore higher range (25-30 kcal/day)
173/2.2 = 79 kg x 30 = 2370 kcal/day
Protein: (0.8– 1 g/kg) 0.8 g x 79 kg = 63 g protein
63 g protein/2370 mL x 100 = 2.7% protein (63 x 4 =252 kcal, 2370 – 252 = 2118 kcal
remaining)
Lipids: 79 kg x 9 = 711 kcal/2 =355.5 mL (20% IL) (2118 kcal – 711 = 1407 kcal remaining)
Dextrose: (3.4 kcal/g dextrose) 1407 kcal/ 3.4 = 413 kcal/g dextrose
413/2370 mL x 100 = 17% dextrose
Summary: 2.7% protein, 17% dextrose, 356 mL 20% IL TPN
