This document lists acceptable forms of identity and address verification documents for KYC purposes. For proof of identity and permanent address, acceptable documents include a passport, driver's license, voter ID card, UIDAI letter/card, job card issued by NREGA, and PAN card. For proof of current address, acceptable documents include ration card, lease/sale agreement, insurance documents, utility bills no more than 3 months old, bank statements no more than 3 months old, declarations from judges, and identity documents issued by government, public sector, or professional bodies. Bank statements must be on official stationary or properly verified to be accepted.

List of Documents – Proof of Identity & Address

Proof of Identity & Address (Permanent Address)

Proof of Address (Correspondence Address)

Bank Statement (BS)


BS must be printed on the stationery of the bank, having logo and name of the bank, displaying the name
and address of the client Branch staff should verify the photocopy of the BS and put his / her signature on
it with the remarks “verified with original”.

OR

In case of Original BS on Plain Paper (Computer Generated) - BS must clearly mention the name and
address of the client. BS must be duly attested (signed and stamped) by the authorized official of the
issuing bank mentioning his name and designation.

General Guidelines for accepting Bank Passbook (BPB)

1) Must have the name, issuing branch name (preferably with address) and logo of the issuing bank.
2) Duly signed and stamped on original BPB by the authorized official of the issuing bank.
3) Photocopy of the covering page/booklet along with the pages having latest transactions must be
obtained.
4) BPB can be accepted as proof of address only for applicant/s whose name appears in BPB. Name in
the BPB can be in any order.

Common General Guidelines for BS & BPB: BS & BPB of a dormant / closed account should not be
accepted (dormant account means, account not having any transactions for the last 6 months).
6 Self-declaration by High Court and Supreme Court judges, giving the new address in respect of their own
accounts.
7 Proof of address issued by any of the following: Bank Managers of Scheduled Commercial

Banks/Scheduled Co-Operative Bank/Multinational Foreign Banks/Elected representatives to the
Legislative Assembly/Parliament/Documents issued by any Govt. or Statutory Authority.
8 Identity card/document with address, issued by any of the following:
 Central/State Government and its Departments
 Statutory/Regulatory Authorities
 Public Sector Undertakings
 Scheduled Commercial Banks
 Public Financial Institutions
 Colleges affiliated to Universities and Professional Bodies such as ICAI, ICWAI, ICSI, Bar Council
etc., to their Members
