The cost is $11 for the movie and bowling or $16 if also ordering a pizza lunch. Permission slips and payment are due by April 21st. Chaperones are needed and will pay $6 to attend the movie, with the option to also order a $5 pizza lunch.

    MMS 8th Grade Field Trip

    As part of the 8th grade science curriculum, we will be traveling to


    the Regal Lake Zurich Movie Theater to see Disney’s AFRICAN
    CATS. Afterwards, we will spend an afternoon bowling at
    Brunswick Zone Deer Park as an earned PBIS reward.
    WHEN: Tuesday, May 10th

    WHERE: Lake Zurich Movie Theater & Brunswick Zone


    Deer Park

    COST: $11.00 (which includes theater admission, 2 hrs


    of bowling and shoe rental)
    OR
    $16 if you choose to pre-order lunch (see details
    below)

    DUE: APRIL 21, 2011 Please return the permission


    slip and payment by this date.

    ATTENTION:
    Students can PRE-order a lunch for an additional $5,
    which includes 2 slices of pizza and unlimited soda,
    OR they must bring a brown bag lunch with a drink.

    Chaperones Needed!!
    We will need one chaperone for every ten students to accompany
    us on our field trip. Chaperones will need to pay $6 to cover
    the cost of the movie. You can also PRE-order a pizza
    lunch for an additional $5. Without your help, we will not be
    able to attend. Please contact your child’s homeroom teacher if

    My student will not be permitted to attend this student trip unless I have submitted a Parental Form for
    Emergency Treatment. If I wish for my student to receive any medication on this trip, I must either
    submit a Field Trip Medication Information Form or Extended Trip Medication Form to my student’s
    school.

    I hereby waive any claims against the Board of Education and its individual members, employees,
    agents, and assigns arising out of my student’s attendance on this student trip and agree to hold
    harmless and indemnify the Board of Education and its individual members, employees, agents, and
    assigns, either jointly or severally, from and against any and all liability, claims demands, damages, or
    causes of actions or injuries, costs, and expenses, including attorneys’ fees, resulting from or arising
    out of my student’s attendance on this student trip.
