1. Is the siren loud enough to be heard by all the drill participants?


YES/NO Definitely the siren is loud enough to be heard by all the participants.

2. Did the drill participants practice the, “Duck, Cover, and Hold” technique during the phase while the siren
was being sounded?
YES/NO The drill participants seriously executed the, “duck, cover, and hold” technique.
If NO, please elaborate.

3. Did the drill participants A] Evacuate during the Alarm Phase B] Waited for the alarm to stop before
evacuating? C] Others, please elaborate.
B. The drill participants waited for the alarm to stop before evacuating. They know that the
alarm signals the earthquake shake so they did not immediately evacuate.

4. Did the drill participants follow their designated routes to evacuation area?
YES/NO The drill participants seriously and correctly follow their designated routes.
If NO, please elaborate.

5. Did the drill participants (A) run, (B) walk casually or (C) walk faster than normal during the evacuation
phase? Please specify if A, B, or C.
C. The drill participants walked faster than normal during the evacuation phase. They walked
briskly without pushing each other.

6. Did the drill participants bring any first aid kit or any item noticeable during the evacuation phase?
YES/NO The participants went out without bringing their bags to avoid other problems to happen. First
aid kits were provided by the medical team.

7. Did the drill participants conduct the headcount during the headcount phase?
YES/NO The assigned HEAD COUNTERS immediately did the head count during the headcount
phase. They double checked whether their classmates are present at the evacuation site or not. Students who
were not found were immediately reported missing.

8. Is the evacuation area big enough to accommodate the evacuees? YES/NO


If NO, please elaborate.
The evacuation areas (both SHS and JHS) are spacious and big enough to accommodate the
students.
9. Are there any other problem observed in the drill that might need to be addressed? Please elaborate.
During the AM session, it tool time for the rescue team to search for the missing student
because they lacked in number. This will be addressed immediately.

10. How long did it take all drill participants to vacate the building and time to reach designated evacuation
area?
For the senior high school department, it took 2 minutes for the students to reach the
evacuation area while for the junior high school it took around 3 minutes to 4 minutes approximately. JHS
has greater number of students compared to the SHS department.

11. Did the drill participants (A) stay in the evacuation area, (B) move to the other place, (C) go back to the
building without instruction? Please specify if A,B, or C.
A. The drill participants stayed and waited for the instructions of the teachers before going to
other place.

12. What is the observed number of drill participants? (Estimation will do)
For the Senior High School, there were around 95 students.
For the AM session of the Junior High School, there were around 280 students.
For the PM session of the Junior High School, there were around 300 students.

13. Are there any untoward incident happened during the drill?

Confidently, nothing bad happened during our drill. Everything happened smoothly.
