    CONTRACT OF SERVICE

    (On-Call)

    KNOW ALL MEN BY THESE PRESENTS:

    This Contract made and entered into this ____ day of ________, 2021 by and
    between:

    SOROSORO IBABA DEVELOPMENT COOPERATIVE (SIDC), a duly registered cooperative


    under the Cooperative Development Authority and with business address at Sorosoro Ibaba, Batangas City
    represented by its Chief Executive Officer, HON. RICO B. GERON hereinafter referred to as the “First
    Party”.
    -and-

    ______________________________ of legal age, Filipino, and a resident of


    _______________________, hereinafter referred to as the “Second Party”.

    -WITNESSETH-

    1. That the FIRST PARTY is in need of Registered Nurse (On-Call) to be assigned in its Office for
    _________________;

    2. That the SECOND PARTY hereby possesses the education, experience and skills required to
    perform the job as a registered nurse or as described in the job description;

    3. That the SECOND PARTY has signified his/her intention, to which the FIRST PARTY has
    accepted, to provide the service needed by the latter.

    4. That the SECOND PARTY hereby attests that he has not been previously dismissed from
    government service or private institutions by reason of an administrative offense;

    5. That in view hereof, the Second Party is hereby contracted as an on-call REGISTERED NURSE
    for the period from ______________________________, to work _________ (____) hours per day
    preferably during Saturdays, Sundays, Holidays and upon the exigency of her service is required
    by the company (especially during the leave of absence of the regular Nurse), in consideration of
    the daily rate of Php ____________/day to be paid in ___________ basis. This is to ensure
    availability of registered nurse during this time of global health crisis;

    6. That the Second Party shall perform work at a time and schedule to be agreed upon by both parties; and

    7. That it is understood that this contract does not create an employer-employee relationship between the
    First Party and the Second Party, that the services rendered hereunder are not considered and will not be
    accredited as company’s service; and the latter is not entitled to any benefits enjoyed by the regular
    personnel of the First Party in which case the terminating party shall serve the other party a notice of
    termination at least fifteen (15) days prior to the termination;
