1.0 GENERAL
A permit to work is a document that specifies the necessary arrangements to be carried out and the
necessary control measures to be taken. It allows work to start only after safe work procedure has
been identified and it provides a clear record that all foreseeable hazards have been considered.
2.0 OBJECTIVE
To provide proactive guidance to control hazardous activities. It is also to identify all work activities,
hazard and risk involved in order to take a safety precaution prior to the execution of the work such as
HIRADC and SOP before commencement of work.
3.0 SCOPE
The scope of this procedure applies for Head Office, Building and Civil Engineering Projects.

5.0 DEFINITIONS
Permit to work system (PTW) is a control mechanism imposed to certain type of work that is
potentially hazardous.
6.1 RESPONSIBILITES
6.2 The line of responsibility for the establishment and implementation of PTW is as follows:

Verification of application
Safety & Health Officer/Personnel Record Keeping
6.3 All site supervisory shall be responsible to ensure that precautions specified in PTW are
followed and complied by the personnel / workers (performing the work).
