2. Mild Anxiety related to emotional abuse as evidenced by belittlement from significant other (husband)
3. Decisional conflict related to emotional abuse as manifested by client’s verbalization “Lagi niya akong sinasabihan na walang kwenta. Hindi ko na alam ang gagawin ko”
4. Hopelessness related to stressful life events as evidenced by inability to manage situation
5. Risk for disturbed personal identity related to dysfunctional family situation as manifested by client’s verbalization “parati niyang sinasabi na wala akong ginagawang
tama, and he makes me feel useless”

NURSING DIAGNOSIS: Chronic Low Self-Esteem related to repeated negative inputs as evidenced by belittlement from significant other (husband)

CUES:
S: “Noong pandemic po, nagbago ang lahat pati pagtrato sa akin ng aking asawa. Hindi ko napo alam ang gagawin ko.” “Lumaki ako na mahiyain” “Hindi ako sanay sa tao”
“Noong una, masaya kami….hanggang sa nagbago siya.” “Di kasi nakikita ng asawa ko ang lahat ng ginagawa ko.” “Hindi rin ako nagsasabi sa mga kamag-anak at mga
kaibigan ko dahil baka magalit ang asawa ko” as verbalized by client.

O: The client starts to doubt her strengths. She was confused when asked why his husband keeps saying negative words to her or what alternative action she does. Hesitant to
share problems with family and friends. Excessive seeking of reassurance.

LTO: After 3 days of nursing intervention,


STO: After 8 hours of nursing intervention, patient will be able to:

EXPLANATION TO THE PROBLEM:
NURSING INTERVENTIONS RATIONALE
Assess the self-esteem level of the patient. Signs of a low self-esteem includes withdrawal from social relationships, feeling of inadequacy, neglect of
personal hygiene and dress, and rejecting self which all may indicate a negative thought pattern.
Assess content of negative self-talk. Note client’s Constant repetition of negative words and thoughts reinforces idea that individual is worthless and belief that
perceptions of how others view him or her. others view him or her in a negative manner.
Determine availability and quality of family/ significant The development of a positive sense of self depends on how the person relates to members of the family, as they
