    AFFIDAVIT OF LOSS

    I, LESLIE T. DANGA, married, Filipino citizen, and a residentof Maamot, San


    Jose, Tarlac, after having been duly sworn to in accordance with law, hereby depose
    and state THAT:

    1. My son, JAKE IVAN DANGA, is a Grade 4 student at Mababanaba


    Elementary School, Mababanaba, San Jose, Tarlac;

    2. He was issued an English Book and a Filipino Book, which he always bring
    going to and from the school;

    3. Last February 2019, I have found out that the abovementioned books were
    lost;

    4. It is my belief that the said books were lost while he was riding a tricycle
    on his way home from school;

    5. I exerted earnest efforts to trace the books in our residence house but to no
    avail, and are now considered lost beyond recovery;

    6. I am executing this affidavit to attest to the veracity of the foregoing


    statements and for whatever legal purposes it may serve.

    I, MICHAELLE KING R. MANIEGO, married, Filipino citizen, and a resident


    of San Rafael, Tarlac City, after having been duly sworn to in accordance with law,
    hereby depose and state THAT:

    1. I am a holder of a Professional Driver’s License issued at Land


    Transportation Office (LTO);

    2. That sometime on November 2019 the above-said Identification Card was


    unfortunately lost or misplaced by me and diligent search proved futile;

    3. That despite diligent efforts exerted to locate the same, it could no longer
    be found and is now deemed lost and beyond recovery;

    4. That I am executing this affidavit to attest to the truth of the foregoing


    statements and facts.
