1 SCOPE AND PURPOSE

This procedure outlines the requirements for practices and procedures to protect employees
from the hazards of entry into permit-required confined space.
To establish a comprehensive system that protects employees from a wide variety of
potential hazards while working in confined spaces.

3 ACRONYMS AND DEFINITIONS

The following terms used in this document have the meaning defined below:

CONTRACT The Form of Agreement together with the documents in order of priority,
including the exhibits, drawings, specifications and documents referred
to and in the order of precedence listed in the Form of Agreement.
CONTRACTOR Tecnimont S.p.a.
LICENSOR Process Licensoir selected by OWNER for the process of LICENSED
UNITS of the FACILITIES, here Lyondell Basell and Univation.
LICENSED UNITS The process units, designed and licensed by the LICENSOR to OWNER
for processing the feedstock and producing the blending components
and/or to make the final or finished product grades.
OWNER ORPIC PLASTIC LLC, a company duly registered under the laws of
Sultanate of Oman and having its principal office at Sohar Industrial Port,
Sohar, PO Box 336, Sultanate of Oman.
OWNER’s Means a Person in OWNER within the contract sponsor’s (end-user)
Representative division/department, which is responsible for managing all aspects of the
contract. It might be referred to as (OWNER Representative) as an
alternative.
PROJECT Means the development, financing, design, engineering, manufacturing,
procurement, installation, construction, testing, pre-commissioning and
commissioning of a petrochemical production facility (including offsite
facilities) to be located in Fahud and Sohar, in the Sultanate of Oman, as
more particularly described in the Owner’s Requirements.
SCOPE OF WORK The scope of work to be performed and services to be rendered by
CONTRACTOR under the CONTRACT in relation to the realization of
the PROJECT.
SUBCONTRACTOR Firm awarded with a subcontract by CONTRACTOR to provide services
for the PROJECT.
LOWER TIER Means any person or company of any tier, including but not limited to,
SUBCONTRACTOR suppliers or vendors of MATERIALS, suppliers or lessors of
SUBCONTRACTOR’s EQUIPMENT and erection contractors, having a
contract with SUBCONTRACTOR or a LOWER TIER
SUBCONTRACTOR for the performance of any part of the WORKS.
ACCEPTABLE The conditions that must exist in a permit space to allow entry and to

the bore and that is capable of withstanding the maximum pressure of


the pipe, line, or duct with no leakage beyond the plate
CONFINED SPACE A space that:
a. Is large enough and so configured that an employee can bodily
enter and perform assigned work, and
b. has limited or restricted means for entry or exit (e.g., tanks,
vessels, silos, storage bins, hoppers, vaults, and pits are spaces
that may have limited means of entry), and
c. is not designed for continuous employee occupancy.
(Examples: Any space where the access or egress is limited, where
oxygen deficiency may occur, 19.5% oxygen or below or an oxygen
enriched atmosphere in excess of 23.5% oxygen by volume, or where
toxic substances or other hazards may accumulate in a restricted work
area. Confined spaces include tanks, vessels, hoppers, bins, tank cars,
steam boilers, conveyor tunnels, coal bunkers, sumps, excavations
below 1.2m or 4 feet (according to OSHA) , ducts, scrubbers, manholes,
sewers etc. A confined space includes any chamber, tank, vat, pipe, or
similar confined space)
DOUBLE BLOCK The closure of a line, duct, or pipe by closing and locking or tagging two
AND BLEED in-line valves and by opening and locking or tagging a drain or vent valve
in the line between the two closed valves.
EMERGENCY Any occurrence (including any failure of hazard control or monitoring
equipment) or event internal or external to the permit space that could
endanger entrants.
ENGULFMENT The surrounding and effective capture of a person by a liquid or finely
divided solid substance that can be aspirated to cause death by filling or
plugging the respiratory system or that can exert enough force on the
body to cause death by strangulation, constriction, or crushing.
ENTRY The action by which a person passes through an opening into a permit-
required confined space. Entry includes ensuring work activities in that
space and is considered to have occurred as scan as any part of the
entrant's body breaks the plane of an opening into the space.
ENTRY PERMIT The written or printed document (refer to as "PERMIT") that is provided
by the employer to allow and control entry into a permit space and that
contains the information in the Permit System portion of this procedure
(refer to Paragraph D.23).
ENTRY The person responsible for determining if acceptable entry conditions are
SUPERVISOR present at a permit space where entry is planned, for authorizing entry
and overseeing entry operations, and for terminating entry.
He is competently trained in Confined Spaces Entry Course.
NOTE:
 An entry supervisor also may serve as an attendant or as an

authorized entrant, as long as that person is trained and


equipped as required by this procedure for each role he or she
fills.
 Also, the duties of entry supervisor may be passed from one
individual to another during the course of an entry operation
providing that person has adequate training.

HAZARDOUS An atmosphere that may expose employees to the risk of death,


ATMOSPHERE incapacitation, impairment of ability to self-rescue (i.e., escape unaided
from a permit space), injury, or acute illness from one or more of the
following causes:
a. Flammable gas, vapor, or mist in excess of 10% of its lower
flammable limit (LFL).
b. Airborne combustible dust at a concentration that meets or
exceeds its LFL [Note: This concentration may be approximated
as a condition in which the dust obscures vision at a distance of 5
feet (1.52 m) or less].
c. Atmospheric oxygen concentration below 19.5% or above 23.5%.
d. Atmospheric concentration of any substance which could result in
employee exposure in excess of its dose or permissible exposure
limit. [Note: An atmospheric concentration of any substance that
is not capable of causing death, incapacitation, impairment of
ability to self-rescue, injury, or acute illness due to its health
effects is not covered by this provision].
e) e. Any other atmospheric condition that is immediately dangerous
to life or health.
HOT WORK The employer's written authorization to perform operations (e.g., riveting,
PERMIT welding, cutting, burning, and heating) capable of providing a source of
ignition.
IDLH IMMEDIATELY DANGEROUS TO LIFE AND HEALTH: Any condition
that poses an immediate or delayed threat to life or that would cause
irreversible adverse health effects or that would interfere wilt an
individual's ability to escape unaided from a permit space. [Note: Some
materials (hydrogen fluoride gas and cadmium vapor, for example) may
produce immediate transient effects or that, even if severe, may pass
without medical attention, but are followed by sudden, possibly fatal
collapse 12-72 hours after exposure. The victim "feels normal" from
recovery from transient effects until collapse. Such materials in
hazardous quantities are considered to be "immediately" dangerous to
life and health].
INERTING The displacement of the atmosphere in a permit space by a non-
combustible gas (such as nitrogen) to such an extent that the resulting
atmosphere is non combustible. [Note: This procedure produces an IDLH
oxygen-deficient atmosphere].
ISOLATION The process by which a permit space is removed from service and
completely protected against the release of energy and material into the
space by such means as: blanking or blinding; misaligning or removing
sections of lines, pipes, or ducts; a double block and bleed system;
lockout or tagout of all sources of energy; or blocking or disconnecting all

mechanical linkages.
LINE BREAKING The intentional opening of a pipe, line, or duct that is or has been
carrying flammable, corrosive, or toxic material, an inert gas, or any fluid
at a volume, pressure, or temperature capable o. causing injury.

OXYGEN An atmosphere containing less than 19.5% oxygen by volume.

ATMOSPHERE
OXYGEN An atmosphere containing more than 23.5% oxygen by volume.
ENRICHED
ATMOSPHERE
PERMIT- A confined space that has one or more of the following characteristics:
REQUIRED a. Contains or has a potential to contain a hazardous atmosphere.
CONFINED SPACE b. Contains a material that has the potential for engulfing an entrant.
(PERMIT SPACE) c. Has an internal configuration such that an entrant could be
trapped or asphyxiated by inwardly converging walls or by a floor,
which slopes downward and tapers to a smaller cross section.
d. d. Contains any other recognized serious safety or health hazard.
PERMIT- Contractor's overall program for controlling, and where appropriate, for
REQUIRED protecting employees from, permit space hazards and for regulating
CONFINED SPACE employee entry into permit spaces.
PROGRAM
PERMIT SYSTEM The employer's written procedure for preparing and issuing permits for
entry and for returning the permit space to service following termination
of entry.
PROHIBITED Any condition in a permit space that is not allowed by the permit during
CONDITION the period when entry is authorized.
RESCUE SERVICE The personnel designated to rescue employees from permit spaces.
RETRIEVAL The equipment (including a retrieval line, chest or full-body harness,
SYSTEM wristlets, if appropriate, and a lifting device or anchor) used for non-entry
rescue of persons from permit spaces.
TESTING The process by which the hazards that may confront entrants of a permit
space are identified and evaluated. Testing includes specifying the tests
that are to be performed in the permit space. [Note: Testing enables
employers both to devise and implement adequate control measures for
the protection of authorized entrants and to determine if acceptable entry
conditions are present immediately prior to, and during, entry].

4 ROLES AND RESPONSIBILITIES

CONTRACTOR Site Manager


It shall be the responsibility of the CONTRACTOR Site Manager to ensure that the requirements
of this Procedure are adhered to.

SUBCONTRACTOR Site Manager


It shall be the responsibility of the SUBCONTRACTOR Site Managers to ensure that the
requirements of this Procedure are adhered to.
In addition, the SUBCONTRACTOR Site Manager:
 Evaluates the facilities and assigned site areas within scope of work for confined space
hazards and installs warning signs at all entrances.
 Ensures that safe working condition requirements have been achieved for the confined
space entry permit.
 Ensures that all personnel involved with the confined space entry have been trained in
accordance with confined space entry requirements.
 Ensures that all employees who enter a confined space have received the necessary
information on respect to the work to be performed.
 Provide prescribed protective equipment for confined space entry operations.

CONTRACTOR Site HSSE Manager


It shall be the responsibility of the CONTRACTOR Site HSSE Departments to monitor the
implementation and adherence to this Procedure and report violations and/or noncompliance
issues to project management for immediate corrective action.
In addition, the CONTRACTOR HSE Manager shall:
 Verify the correctness and completeness of the confined spaces risk assessment in all
the site area
 Define and verify the confined space pre-entry testing and atmospheric monitoring.
 Verify that safe working condition requirements have been achieved for the confined
space entry permit
 Verify that all personnel involved with the confined space entry have been trained in
accordance with confined space entry requirements
 Plan and organize periodically emergency and evacuation “mock drills”

SUBCONTRACTOR Site HSSE Manager


It shall be the responsibility of the SUBCONTRACTOR Site HSE Department to monitor the

implementation and adherence to this Procedure and report violations and/or noncompliance
issues to project management for immediate corrective action. In addition, the
SUBCONTRACTOR HSE Manager shall:

 Perform the confined space pre-entry testing and atmospheric monitoring.


 Verify that safe working condition requirements have been achieved for the confined
space entry permit
 Verify that all personnel involved with the confined space entry have been trained in
accordance with confined space entry requirements
 Assist in the organization of emergency and evacuation “mock drills”

CONTRACTOR Entry Supervisor (ES)


The Entry Supervisor is the person responsible for determining if acceptable entry conditions
are present at a permit space where entry is planned, for authorizing entry and overseeing entry
operations, and for terminating entry. He is competently trained in Confined Spaces Entry
Course.
In addition to the CONTRACTOR Issuing Authority duties stated in the “Permit To Work
Procedure” (3911-LZ-PC-AA000107), the Entry Supervisor performs the following tasks:
 Confirms, by checking, that the appropriate accesses have been made available as
stated on the PTW, that all tests indicated in the permit have been conducted and that all
procedures and equipment indicated by the permit are in place before issuing the permit
and allowing entry to begin.
 Verifies that all pre-entry tests specified by the permit have been conducted and that
acceptable entry conditions exist.
 Ensures that all procedures and equipment specified by the permit are in place prior to
entry.
 Endorses the permit and authorizes entry.
 Terminates the entry and cancels the permit as necessary.
 Verifies that rescue services are available and that the means for summoning them are
in place and operable.
 Removes unauthorized individuals who enter or attempt to enter the space during entry
operations.
 Determines that entry operations remain consistent with terms of the permit and that
acceptable entry conditions are maintained.
 Ensures safe closure and maintains control of the confined space.
 Prevents unproved individuals to enter the permit space during entry operations
 If an entrant has the potential to be exposed to a substance for which a Material Safety
Data Sheet or COSHH sheet is required to be kept at the worksheet, the written
information shall be made available to the medical facility treating the exposed entrant.
NOTE: the CONTRACTOR Site Management shall ensure that each Entry Supervisor is
aware of the hazards that may be encountered during entry, including information on the
mode, signs or symptoms and consequences of exposure.

Confined Space Watcher/Standby Person (SUBCONTRACTOR)


The Confined Space Watcher/Standby performs the following tasks:

 Continuously maintains an accurate record of authorized entrants.


 Remains outside the permit space during entry and/or rescue operations until relieved by
another attendant.
 Communicates with authorized entrants as necessary to monitor entrant status and to
alert entrants of the need to evacuate the space.
 Monitors activities inside and outside the space to determine if it is safe for entrants to
remain inside the space or non-entry personnel to remain outside the space nearby.
 Orders evacuation under the following conditions:
o Upon detection of a prohibited condition
o Upon detection of the behavioral effects of hazard exposure to authorized
entrants
o Upon detection of a situation outside the space that could endanger the entrants
o Upon noting that the attendant cannot effectively or safely perform required
duties
 Summons rescue/emergency services upon determination that authorized entrants may
require assistance to escape from the space.
 Takes the following actions when unauthorized persons approach or enter the space:
o Warns the unauthorized person to stay away
o Advises an unauthorized person to exit the space
o Advises authorized entrants and the entry supervisor that an unauthorized
person has entered the space
 Performs non-entry rescues as per this project instruction.
 Performs no duties that might interfere with the primary duty – monitoring and protecting
authorized entrants.
 Is competently trained in Confined Spaces Entry Course.

Employees / Authorized Entrants


All employees who enter a confined space will:
 Review and sign the confined space entry permit
 Know space hazards, including information on the mode of exposure (e.g., inhalation or
dermal absorption), signs or symptoms, and consequences of the exposure;
 Use appropriate personal protective equipment properly (e.g., such as gloves, aprons,
and coveralls);
 Correctly use all of the required equipment (i.e., ventilation, communication, lighting,
PPE);
 Maintain communication with the attendant to enable the attendant to monitor entrant
status and alert entrants of the need to evacuate the space;
 Alert the attendant when:
o Entrant recognizes any warning sign, symptom of exposure, or the presence of a
dangerous situation;
o A prohibited condition is detected;

 Exit the space as quickly as possible when:


o An order to evacuate is given;
o The entrant recognizes a warning sign or symptom of exposure to a dangerous
situation
o The entrant detects a prohibited condition

o An evacuation alarm is activated

5 INSTRUCTION

5.1 General Requirements


Only persons who have attended the Safety Training course and competently trained in
Confined Spaces Entry Course, who have been issued with a confined space authorization
certificate by the Contractor HSSE Department can enter into a Confined Space, or act as a
Safety Watcher.
A Work Method Statement/JSA must be approved for each confined space entry and should

The following activities are considered to be hazardous when carried out in confined spaces
 Paint spraying
 Welding
 Gouging
 Preheating and Burning
 Grit blasting
 Use of Solvents
The hazards involved include Fire, explosion, asphyxiation, dust, fumes, noise, electrocution
and oxygen deficiency/enrichment.
The confined space must be positively isolated from all piping connections, either by spades or
the removal of valves or spool pieces.
All electrical-driven equipment connected to, or normally associated with the confined space,
must be locked out.
The use of electrical equipment inside confined spaces must be strongly discouraged, however,
where this is not possible the use of the low-voltage (12 V) or GFCI-equipped (or ELCB, as
applicable) electrical tools may be allowed. Additionally, any air-powered tool used within a
confined space will be powered by Grade-D breather air or its equivalent (i.e., properly

equipped compressor with a CO monitor).

Only air must be used to drive pneumatic tools in confined spaces (except when the
atmosphere is inert).

The confined space must be prepared and placed in a safe condition for entry prior to the
Permit Issuer completing the Hot Work Permit. The actual preparation of an individual confined
space will vary according to its design, construction and use. However, in all cases, adequate
lighting, ventilation and safe means of access and egress must be provided. Ensure that
torches are available for emergency lighting in case of generator failure.
Evaluations for heat stress must be carried out by the Subcontractor Supervisor and Contractor
Supervisor prior to start of work and at regular intervals, and appropriate action taken.
Whenever more than one group of workers is working simultaneously inside a confined space a
proper coordination will be organized by CONTRACTOR and relevant SUBCONTRACTORS,
the coordination measures shall be reported in the WMS/JSA.
No hot work will be allowed inside a confined space until appropriate control measures are
introduced, based on the Method Statement and JSA.
The ventilation system, preferably air driven, must be arranged such that only fresh air is drawn
or blown into the confined space.
In all cases Oxygen and Gas tests must be carried out by Site HSE Department prior to initial
entry, and within working by a Competent Person at intervals determined by the Permit Issuer.
(Note: For certain activities continuous monitoring may be required).
The Safety Watcher must be briefed by the Permit Issuer on his/her responsibilities.
An agreed emergency response/rescue plan must be prepared and explained to all persons
involved in the confined space entry. The complexity of the plan will be dependent upon the
type of confined space; however, all persons entering must wear adequate PPE, including
safety harness.
Cylinders containing oxygen, acetylene, nitrogen or other gases shall not be taken into confined
or enclosed spaces.
Internal combustion powered engines shall not be operated in confined spaces.
Low voltage transformers must not be taken into confined spaces.
When all preparation work is complete the Permit Issuer will sign the Permit to Work and
forward it to Contractor Site HSE Manager for review and authorization signature.
When issued the Permit to Work must be displayed at the entrance to the confined space.
Each person entering the confined space will attach his/her ID badge to the display board and
sign the log sheet, both of which will hang at the entrance to the confined space. Each person
leaving the confined space, irrespective of for how long, must sign the log sheet and remove
his/her ID badge.
Safety Watcher will strictly control entry to the confined space. When there is no one inside, and
before he leaves, he/she must either shut the door or place a warning sign to prevent
unauthorized entry in his absence.

5.2 Prior to Entry

Prior to entering a permit required confined space a permit must be properly completed by the
competent person (Appendix 1).
When an individual passes through an opening he is entering the confined space area. Entry
includes working in the confined space and is considered to have taken place as soon as any
part of the entrant's body breaks the plane of an opening.

The proper protective clothing, respiratory equipment, emergency stand-by equipment, and fire
precautions shall be taken before the permitted confined space is entered.
Anyone entering a permitted confined space area should wear a harness attached to a lifeline
leading out of the confined space opening.
When the confined space to be entered is a tank, vessel, vat container or similar, an
investigation will be performed to identify the container’s previous contents. Appropriate safety
precautions, such as issuing chemical protective clothing, will be taken.

5.3 Issuing Entry Permit


The permit issuing authority shall:
 Enforce the required measures to prevent prohibited entry.
 Distinguish and classify the hazards of a permit required confined space before
employees enter.
 Produce and co-ordinate the procedures and operations necessary for a safe permit
confined space entry operations.
 Provide the necessary equipment, at no cost to employees, sustain that equipment
properly and ensure that the employees are using the equipment properly. This includes
testing and monitoring equipment, communications equipment, personal protective
equipment, lighting equipment, barriers and shields. The employer must also make
available all necessary rescue equipment.
 When there is a change of task a new Permit is required.

5.4 During the activity


When an unsafe condition arises in or near the permit required confined space the entry
supervisor shall stop work, remove the workers, and revoke the entry permit. The Entry
Supervisor must cancel entry permits when an assignment is completed or when new conditions
exist. New conditions must be noted on the cancelled permit and used in revising the permit
space program. The standard requires that the SUBCONTRACTOR keep all cancelled entry
permits for at least one year.
Any problems confronted during an entry operation shall be marked on the appropriate permit
so that proper revisions to the permit can be made.

The entry permit that records compliance with this section and entitles entry to a permitted
confined space shall be posted at the point of the access/entry and shall identify the following:
 Name of permit space to be entered, authorized entrant(s), eligible attendants and
individuals authorized to be Entry Supervisors;
 The date and the authorized duration of the entry permit (the duration of entry permits

must not exceed the time required to complete an assignment or one shift, whichever
comes first);
 Name and signature of supervisor who authorizes entry;
 Purpose of entry and known space hazards;
 Measures to be taken to isolate permit spaces and to eliminate or control space hazards;

 Name and telephone numbers of rescue and emergency services and means to be used
to contact them;
 Acceptable entry conditions;
 Communication procedures and equipment to maintain contact during entry;
 Additional permits, such as for hot work, that have been issued authorizing work in the
permit space;
 Special equipment and procedures, including personal protective equipment and alarm
systems; and
 Any other information needed to ensure employee safety;
 The authorized personnel, by name, currently serving as Standby person.
Circumstances may require more than one person posted at the entry point or at
different access/entry points. The requirement for additional standby persons should be
carefully considered during the planning of the job depending on the specific JSA
contents.

5.5 Training
CONTRACTOR and SUBCONTRACTORs will provide training so that all employees whose
work is regulated by either permit required Confined Space Entry Program, must obtain the
understanding, knowledge, and skills necessary for the safe performance of the responsibilities
assigned under this procedure.
Training shall be provided to each employee before the employee is assigned responsibilities. If
there is a change in assigned duties employees must undergo additional training or orientation.
If there is a change in either permit required confined space operation that presents a hazard
which concerns an employee who has not previously been trained in that area, he must be
given training concerning the proper operations and the required procedures.
The training shall establish employee competence in the responsibilities required by this section
and shall present new or revised operations, as necessary, for compliance with this program.
The Site HSSE Department will document and certify that the training for these procedures has
been accomplished. The certification will include each employee's name, the signatures or
initials of the trainers, and the dates of training.

5.6 Emergency Situations


Emergency situations exist when there is immediate or imminent danger to life and health of the
entrants.
Emergency procedure will be activated in particular in the following occur:
 Failure of monitoring equipment
 An emergency occurs inside or outside the confined space
The attendant must be notified immediately whenever the entrant notices any warning sign or
symptom of exposure to a dangerous situation, or he detects a prohibited condition.

If the entrant recognizes any warning sign or symptom of exposure to a dangerous situation,
immediate evacuation of the permitted confined space is required.
 If the entrant notices or detects a dangerous situation outside the confined space, then
the entrant must evacuate immediately.

 Entrants must maintain communication with the watcher, either by radio, verbal, or visual
contact.
 Entrants must wear a full body harness with tag line secured to the “D” ring on the back
of the harness. In case of an emergency where the entrant is unable to physically
remove himself from the confined space, an extraction device will assist in removing the
entrant.
 An extraction device will be made available for all confined space work.
