This procedure outlines the requirements for practices and procedures to protect employees
from the hazards of entry into permit-required confined space.
To establish a comprehensive system that protects employees from a wide variety of
potential hazards while working in confined spaces.

The following terms used in this document have the meaning defined below:
CONTRACT The Form of Agreement together with the documents in order of priority,
including the exhibits, drawings, specifications and documents referred
to and in the order of precedence listed in the Form of Agreement.
CONTRACTOR Tecnimont S.p.a.
LICENSOR Process Licensoir selected by OWNER for the process of LICENSED
UNITS of the FACILITIES, here Lyondell Basell and Univation.
LICENSED UNITS The process units, designed and licensed by the LICENSOR to OWNER
for processing the feedstock and producing the blending components
and/or to make the final or finished product grades.
OWNER ORPIC PLASTIC LLC, a company duly registered under the laws of
Sultanate of Oman and having its principal office at Sohar Industrial Port,
Sohar, PO Box 336, Sultanate of Oman.
OWNER’s Means a Person in OWNER within the contract sponsor’s (end-user)
Representative division/department, which is responsible for managing all aspects of the
contract. It might be referred to as (OWNER Representative) as an
alternative.
PROJECT Means the development, financing, design, engineering, manufacturing,
procurement, installation, construction, testing, pre-commissioning and
commissioning of a petrochemical production facility (including offsite
facilities) to be located in Fahud and Sohar, in the Sultanate of Oman, as
more particularly described in the Owner’s Requirements.
SCOPE OF WORK The scope of work to be performed and services to be rendered by
CONTRACTOR under the CONTRACT in relation to the realization of
the PROJECT.
SUBCONTRACTOR Firm awarded with a subcontract by CONTRACTOR to provide services
for the PROJECT.
LOWER TIER Means any person or company of any tier, including but not limited to,
SUBCONTRACTOR suppliers or vendors of MATERIALS, suppliers or lessors of
SUBCONTRACTOR’s EQUIPMENT and erection contractors, having a
contract with SUBCONTRACTOR or a LOWER TIER
SUBCONTRACTOR for the performance of any part of the WORKS.
ACCEPTABLE The conditions that must exist in a permit space to allow entry and to
ENTRY ensure that employees involved with a permit-required confined space

LINE BREAKING The intentional opening of a pipe, line, or duct that is or has been
carrying flammable, corrosive, or toxic material, an inert gas, or any fluid
at a volume, pressure, or temperature capable o. causing injury.

DEFICIENT
ATMOSPHERE
OXYGEN An atmosphere containing more than 23.5% oxygen by volume.
ENRICHED
ATMOSPHERE
PERMIT- A confined space that has one or more of the following characteristics:
REQUIRED a. Contains or has a potential to contain a hazardous atmosphere.
CONFINED SPACE b. Contains a material that has the potential for engulfing an entrant.
(PERMIT SPACE) c. Has an internal configuration such that an entrant could be
trapped or asphyxiated by inwardly converging walls or by a floor,
which slopes downward and tapers to a smaller cross section.
d. d. Contains any other recognized serious safety or health hazard.
PERMIT- Contractor's overall program for controlling, and where appropriate, for
REQUIRED protecting employees from, permit space hazards and for regulating
CONFINED SPACE employee entry into permit spaces.
PROGRAM
PERMIT SYSTEM The employer's written procedure for preparing and issuing permits for
entry and for returning the permit space to service following termination
of entry.
PROHIBITED Any condition in a permit space that is not allowed by the permit during
CONDITION the period when entry is authorized.
RESCUE SERVICE The personnel designated to rescue employees from permit spaces.
RETRIEVAL The equipment (including a retrieval line, chest or full-body harness,
SYSTEM wristlets, if appropriate, and a lifting device or anchor) used for non-entry
rescue of persons from permit spaces.
TESTING The process by which the hazards that may confront entrants of a permit
space are identified and evaluated. Testing includes specifying the tests
that are to be performed in the permit space. [Note: Testing enables
employers both to devise and implement adequate control measures for
the protection of authorized entrants and to determine if acceptable entry
conditions are present immediately prior to, and during, entry].

implementation and adherence to this Procedure and report violations and/or noncompliance
issues to project management for immediate corrective action. In addition, the

5 INSTRUCTION
Only air must be used to drive pneumatic tools in confined spaces (except when the
atmosphere is inert).

The confined space must be prepared and placed in a safe condition for entry prior to the
Permit Issuer completing the Hot Work Permit. The actual preparation of an individual confined
space will vary according to its design, construction and use. However, in all cases, adequate
lighting, ventilation and safe means of access and egress must be provided. Ensure that
torches are available for emergency lighting in case of generator failure.
Evaluations for heat stress must be carried out by the Subcontractor Supervisor and Contractor
Supervisor prior to start of work and at regular intervals, and appropriate action taken.
Whenever more than one group of workers is working simultaneously inside a confined space a
proper coordination will be organized by CONTRACTOR and relevant SUBCONTRACTORS,
the coordination measures shall be reported in the WMS/JSA.
No hot work will be allowed inside a confined space until appropriate control measures are
introduced, based on the Method Statement and JSA.
The ventilation system, preferably air driven, must be arranged such that only fresh air is drawn
or blown into the confined space.
In all cases Oxygen and Gas tests must be carried out by Site HSE Department prior to initial
entry, and within working by a Competent Person at intervals determined by the Permit Issuer.
(Note: For certain activities continuous monitoring may be required).
The Safety Watcher must be briefed by the Permit Issuer on his/her responsibilities.
An agreed emergency response/rescue plan must be prepared and explained to all persons
involved in the confined space entry. The complexity of the plan will be dependent upon the
type of confined space; however, all persons entering must wear adequate PPE, including
safety harness.
Cylinders containing oxygen, acetylene, nitrogen or other gases shall not be taken into confined
or enclosed spaces.
Internal combustion powered engines shall not be operated in confined spaces.
Low voltage transformers must not be taken into confined spaces.
When all preparation work is complete the Permit Issuer will sign the Permit to Work and
forward it to Contractor Site HSE Manager for review and authorization signature.
When issued the Permit to Work must be displayed at the entrance to the confined space.
Each person entering the confined space will attach his/her ID badge to the display board and
sign the log sheet, both of which will hang at the entrance to the confined space. Each person
leaving the confined space, irrespective of for how long, must sign the log sheet and remove
his/her ID badge.
Safety Watcher will strictly control entry to the confined space. When there is no one inside, and
before he leaves, he/she must either shut the door or place a warning sign to prevent
unauthorized entry in his absence.
Prior to entering a permit required confined space a permit must be properly completed by the
competent person (Appendix 1).
When an individual passes through an opening he is entering the confined space area. Entry
includes working in the confined space and is considered to have taken place as soon as any
part of the entrant's body breaks the plane of an opening.

The proper protective clothing, respiratory equipment, emergency stand-by equipment, and fire
precautions shall be taken before the permitted confined space is entered.
Anyone entering a permitted confined space area should wear a harness attached to a lifeline
leading out of the confined space opening.
When the confined space to be entered is a tank, vessel, vat container or similar, an
investigation will be performed to identify the container’s previous contents. Appropriate safety
precautions, such as issuing chemical protective clothing, will be taken.
The entry permit that records compliance with this section and entitles entry to a permitted
confined space shall be posted at the point of the access/entry and shall identify the following:
Name of permit space to be entered, authorized entrant(s), eligible attendants and
individuals authorized to be Entry Supervisors;
The date and the authorized duration of the entry permit (the duration of entry permits

must not exceed the time required to complete an assignment or one shift, whichever
comes first);
Name and signature of supervisor who authorizes entry;
Purpose of entry and known space hazards;
Measures to be taken to isolate permit spaces and to eliminate or control space hazards;

Name and telephone numbers of rescue and emergency services and means to be used
to contact them;
Acceptable entry conditions;
Communication procedures and equipment to maintain contact during entry;
Additional permits, such as for hot work, that have been issued authorizing work in the
permit space;
Special equipment and procedures, including personal protective equipment and alarm
systems; and
Any other information needed to ensure employee safety;
The authorized personnel, by name, currently serving as Standby person.
Circumstances may require more than one person posted at the entry point or at
different access/entry points. The requirement for additional standby persons should be
carefully considered during the planning of the job depending on the specific JSA
contents.
5.5 Training
CONTRACTOR and SUBCONTRACTORs will provide training so that all employees whose
work is regulated by either permit required Confined Space Entry Program, must obtain the
understanding, knowledge, and skills necessary for the safe performance of the responsibilities
assigned under this procedure.
Training shall be provided to each employee before the employee is assigned responsibilities. If
there is a change in assigned duties employees must undergo additional training or orientation.
If there is a change in either permit required confined space operation that presents a hazard
which concerns an employee who has not previously been trained in that area, he must be
given training concerning the proper operations and the required procedures.
The training shall establish employee competence in the responsibilities required by this section
and shall present new or revised operations, as necessary, for compliance with this program.
The Site HSSE Department will document and certify that the training for these procedures has
been accomplished. The certification will include each employee's name, the signatures or
initials of the trainers, and the dates of training.
If the entrant recognizes any warning sign or symptom of exposure to a dangerous situation,
immediate evacuation of the permitted confined space is required.
If the entrant notices or detects a dangerous situation outside the confined space, then
the entrant must evacuate immediately.

Entrants must maintain communication with the watcher, either by radio, verbal, or visual
contact.
Entrants must wear a full body harness with tag line secured to the “D” ring on the back
of the harness. In case of an emergency where the entrant is unable to physically
remove himself from the confined space, an extraction device will assist in removing the
entrant.
An extraction device will be made available for all confined space work.
