At 1932:46 hours ambulance 254 was dispatched code one to the above address for a
traumatic injury code one. C250 (report writer) was in-service and traveling on 17th
Ave NW when the call for service went out. Based on CAD notes C250 upgraded the call
to code three response and asked for E251 to be dispatched. C250 went in route to
the above address code three responding to gate two (2) for access. C269 was working
at Adventureland on an extra duty assignment. C269 asked for information from
dispatch since his crew, the extra duty APD officers and Adventureland security have
not been made aware of an incident at Adventureland. C269 was informed by dispatch
of the incident on the raging river ride and responded code three with APD and
Adventureland security. C250 arrived at gate two (2) to find the gate closed and
appeared to be secured with a heavy chain and lock. C250 called for
security/Adventureland to open gate two (2) for access of all responding units. A254
and C250 arrived inside the park through gate two (2). C250 advised dispatch that he
would be in command with a mobile command. C269 was on scene advising the location
of the incident and possible number of patients. C269 advised that CPR was in
progress on one patient at this time. C250 made the scene after a lengthy run to the
far east side of the park along the man made river through un even terrain to find
two patients lying on the shore of

the north side of a man made river. A raft was upside down on the south side on the
inside corner in the man made river. The water was not moving and appeared to be
about two (2) feet deep. C250 spoke to C269 face to face, C269 advised that all
patients were out of the raft and we had a total of 4 to 6 patients. A patient was
still in the water on his feet resting against the side of the man made river. Two
bystanders were supporting the patient at the side of the river. AFD, APD and
Adventureland security were performing life safety measures on patient #1. AFD
member was performing life safety measures on patient #2. Patient number three was
being supported while standing in the water on the side of the man made river with
bystanders providing comforting and support measures. Patient #4 was standing and
talking north of patient #1 and 2 and was standing with other family members who
were on the raft but did not have injuries. C250 made the determination that the
event was an MCI and requested an ambulance strike team to be dispatched to our
location code three. C260 was assigned the operations sector and managed the staging
and deployment of resources needed for the remote location of the incident.
Ambulance 254 took over care of patient #2 along with E251 upon their arrival.
Ankeny ambulance crew took over patient care of patient #3 and with the assistance
of Des Moines medic three and truck three patient #3 was removed from the water in a
controlled manner. Pleasant Hill ambulance crew took over care of patient #4 and
advised that the other two family members were not injured and did not need medical
assistance at this time. All patients were secured to long boards except patient #4,
to be removed from the scene. All patients were secured to cots and many first
responders and Adventureland employees were utilized to remove the cots containing
patients from the remote location. All patients were transported to Methodist
Medical center in Des Moines.

After all patients were removed from the scene we had two fire department members
with proper PPE enter the water and perform a search of the raft and surrounding
area with nothing found.

At this time, command reported that all of the patients had been transported and we
could start the systematic release of units that there still on scene. The release
of units was coordinated with Staging. Command was terminated. While beginning to
exit the incident site, we spoke with several folks who identified themselves as the
fireworks crew that was working to set up fireworks in the area of the overturned
raft when the incident occurred. See C250 report for contact information. The
fireworks employees reported that while setting up fireworks near the Raging River,
they began to hear screams and yells coming from the Raging River. Initially this
was thought to be normal activity from people as they rode the attraction. However,
as other rafts began to pass through the area, they were able to convey to the
fireworks

workers that there was an emergency and that a raft had overturned. They stated that
they did not believe that the Adventureland staff were aware of what was happening
yet. The fireworks workers called 911, proceeded to the overturned raft site, and
entered the water and assisted with victim removal from the water. They then
assisted with CPR along with first-arriving Adventureland staff and other first-
arriving AFD and APD staff.

C250 and C260 remained on scene for media inquires with C260 handling all on camera
requests.

C250 and C260 returned to service.

C260 was dispatched to the above location by C250 to assist with a medical call at
Adventureland Park. C260 was on a previous call (see 21-1297) and diverted to
Adventureland, Response 3. While I was en route, C250 had arrived, established
command, and was reporting that a raft had overturned at the Raging River Ride and
that there were now two CPRs in progress. There were also additional patients. C250
requested an ambulance strike team to be dispatched Response 3.

E251 and A254 arrived, entered Gate 2 per command, and reported to the site of the
overturned boat. They began to administer care; see patient care reports. AFD had
staffed a Gator Response unit at Adventureland Park during the day, and this unit
was on duty at the park at the time of the call. The Gator unit also responded to
the scene and began to administer patient care; see patient care reports. A253
cleared from a previous call, and responded to the scene Response 3 from the VA
Hospital.

While still en route, C250 asked dispatch which ambulances were on the way to the
scene. Dispatch reported that the only units en route were A253 and A254; command

in asked for an ambulance strike team, Response 3, as the initially requested strike
team still had not been dispatched.

I arrived and checked in with Command via radio. The decision was made that C260
would assume the Operations role, as Command was in a mobile mode and was needed to
participate in patient care due to limited resource availability.

The entrance for all inbound units was declared to be at Gate #2, along the west
side of the park along 34th Avenue SW. As I began planning operations, I parked my
vehicle just inside Gate 2 and out of the way of traffic. Here, I encountered a
teenage male Adventureland employee and asked him if he was in charge or if he could
direct me to a supervisor. The employee stated that no, he was not in charge and he
didn't know where a supervisor was or where to get ahold of one. I then encountered
two teenage or low 20's-aged female Adventureland employees that appeared to be
clearing out of the Adventure Bay water park area. They also could not direct me to
a supervisor, and none of the Adventureland employees appeared to be aware of what
had happened at the Raging River. I instructed the employees to make sure the Gate 2
driveway remained clear of obstructions and that absolutely no one was to block it.

I proceeded from my vehicle down a path eastbound towards the incident scene. I met
APD 266 here, and directed him to control traffic at the Gate 2 entrance. I then met
APD Detective Houser and asked her to maintain traffic control in the Outlaw Gulch
area. APD personnel assisted with moving A254 from the Outlaw Gulch area back past
the Sidewinder as close to the overturned raft as possible. The closest they could
get was about 100 yards from the raft, in the area where July 4th Fireworks had been
set up and near the loading shack for the Raging River ride. This became the
ambulance staging area. This area of the park (Outlaw Gulch and Raging River area)
seemed to be shut down, although the other areas of the park appeared to remain open
for normal business.

I checked with dispatch and verified that the ambulance strike team was en route; we
verified that the ambulances were on the way. Command reported that there was an
emergent need for additional manpower, ambulance crews, and personnel equipped with
PFD's to enter the water to perform a rescue. In addition to the ambulance strike
team already en route, I had dispatch send an additional engine company from the Des
Moines Fire Department, a District Chief from the Des Moines Fire Department, and an

engine company from the Delaware Township Fire Department. Engine 3 and DC 2
responded from DMFD; E331 contacted me via radio and stated that they had PFD's on
L335 and not E331. DTFD asked if we wanted L335 instead and I said yes. L335
responded.

Strike team ambulances began to arrive. Pleasant Hill ambulance was able to drive
back to the staging area. Their crew reported to Command at the overturned raft to
assist. Ankeny ambulance arrived and proceeded to the staging area, and then moved
to the raft site to assist. Mercy Ambulance arrived and proceeded to the staging
area. Mercy ambulance crew staged at their ambulance. DMFD Engine 3, Medic 3, and
DC2 arrived. Engine 3 and DC2 were parked south of the Tornado near E251 to leave
way for ambulances. Medic 3 and and two Bondurant ambulances arrived and were
ordered to park near the Sidewinder, without blocking the patch back to the staging
area. Engine 3, Medic 3 crew, and DC2 all ran back to the overturned raft with their
PFD's and reported to Command to make the water rescue. Bondurant EMS crews were
sent on foot back to the raft site to assist with lifting and patient movement to
ambulances. The terrain was rough and uneven, this created major lifting
difficulties and required additional lifting assistance.

A254 and A253 initiated patient transports. L335 arrived and remained on 34th at
Gate 2 to assist with traffic control. A333 arrived and remained at Gate 2. Staging
Officer duties were assigned to L335 Officer. A333 assisted. Ankeny ambulance
initiated patient transport and Pleasant Hill ambulance transported family members
to the hospital. L335 crew was ordered to reposition E251 to ensure adequate egress
routes for the transporting ambulances, and then L335 ran back and reported to the
site of the overturned raft to provide lifting assistance.

At this time, command reported that all of the patients had been transported and we
could start the systematic release of units that there still on scene. The release
of units was coordinated with Staging. Command was terminated. While beginning to

A254 dispatched initially to above location for a response 1 traumatic injury. When
they went en route it was updated this was a CPR in progress. E251 responded
response 3 with A254. C250 was also en route. C250 established command. E251 crew of
2 arrived and responded to area of the overturned raft of the raging river ride.

later transported to the hospital, see EHR. E251 crew assisted in clean up of
equipment and disposable pt care materials. E251 was released by command, returned
to service and returned to station 25.
