    WAIVER OF RIGHTS

    Know all men by these presents

    We, ZAIDA D. FLORES and HERNANE T. FLORES of legal age and and senior citizen, a
    Filipino the mother and father of the deceased, and residents OF 35 Quartz St., Crystal Meadows
    Subd. Sasa Davao City, Philippines, after being sworn in accordance with law, hereby declare that:

    1. That we are the surviving beneficiary of the deceased - insured ALVA ROSE D. FLORES
    who died on December 26, 2020 at SPMC, Davao City
    2. That We, ZAIDA D. FLORES and HERNANE T. FLORES the undersigned, do hereby
    waive, beneficiary / claimant over said SSS Claim left by my above deceased daughter;
    3. That this waiver of rights is being executed in favor of my son/brother of the deceased,
    name: HAZEL D. FLORES as, manifested and acceptance of the same and hereby
    assumes the death claim of the said company.

    IN WITNESS WHEREOF, we have hereunto set our hands this  in June 7, 2021, 2021 at
    Davao City, Philippines.

    That I execute this waiver, freely and voluntarily to attest to the foregoing facts and
    statements and for whatever legal purpose it may serve.
