Read and answer the following questions based on your prior knowledge about
ASSCAT.

1. What is community assessment?

Community assessment is a method or process in which used to provide information needed to aid
what the community lacks and to provide precise measures according to the community
necessities.

2. Why do we need to asses a community before conducting community service?

We need to assist the community first before conducting the community services needed to
provide the necessary measures and needs to what is needed. To properly solve the community
problem and to make sure to avoid any more issues with regards for assisting and providing aid.

3. How can you make a comprehensive community assessment?

We can make a comprehensive community assessment by having these important


information:

 Identify the essential resources that are already available within the organization.
 Identify the essential resources to be acquired.
 Determine how to use, develop, or obtain those resources.

Instructions:

The Civic Welfare Training Services (CWTS) component of the National Service
Training Program (NSTP) covers eight (8) major areas of concerns as stated in its
implementing rules and regulations. These are health, education, environment,
entrepreneurship, safety, recreation and moral of the citizenry and other social welfare
services. Using the guide format, create a Community Needs Assessment on your
locality that is focused on two (2) areas of concern. This can be on either of the scope
of your Purok, Barangay, or Municipality. The data gathering can be done using
technology or via face-to-face that is why everybody is reminded to strictly obey
minimum health protocols at all time and documentations are also needed in this
activity. Each Assessment is worth 25 points. This assessment will be the basis for
your Project Proposal in the next chapter. God Bless and Stay Safe .

COMMUNITY ASSESSMENT 1

Part I.
Introduction: (Steps 1, 2, 4 and 5)

Health and education

The city of Bayugan  is a 5th class component city in the province of Agusan del Sur,
Philippines. According to the 2015 census, it has a population of 103,202 people. Gained
cityhood through Republic Act No. 9405, it is only city in the province of Agusan del Sur.
Filled with green scenery and magnificent plaza and tourist spots. And in this city, number of
hospitals are located and being implemented as well as free services and programs related to
such organizations and facilities.

Step II: Decide on scope

This focuses on the children whose BMI is below normal at the age between 2-9 years
old in P-9B Camatsili Street Poblacion, Bayugan City. Children nowadays are more
prone to be infected by numerous cases of diseases and sickness. Our health is the
most important aspect we should be wary about. The best way to keep a healthy body,
is to practice healthy meals, exercises and health related activities. Through eating
nutritious foods like fruits and vegetables, it helps build up nourishment that our body
needs.

Step III: Identify Assets

The organizations or assets that is involve in this type of program will be the Baranggay
center, DSWD, Community members and organizations that helps and promote the health of
the citizens in this perspective area in Bayugan.
Step IV: Make connections

For this type of program and project, I would be asking the help of the council members in the
baranggay and to those in community health to be able to provide more accurate and guidance
for the necessary needs and procedures. Since, connections and help are needed to conduct
this type of project.

Step V: Collect Data

Upon implementing this activity, first collection of data or information with regards of the
district population of children and records of the BMI recorded are needed for selection of
participants and to know its numbers and the minimum to maximum estimated costs needed
to conduct and implement the said project. To avoid conflict and lack of data upon the
implementation of the project in the baranggay.

PART III: CONCLUSION

Children are important citizens with fragile physical, health and mental. That is why we
created this program to ensure their health and to be pro-active during this time of
year, especially now that we are facing such dilemma. Our group is proposing and
conducting a social project called Healthy Care for Children or HCC. A feeding
program for community children at age 2-9 years old. We decided to do this project
because we notice that some children in the specific area have low BMI and poor
physical looks. We notice that these factors might affect the children’s health, growth
and immunity. That is why this project is essential as it aims for the improvement and
progress of their health and growth.

RECOMMENDATIONS

In this type of project and programs there are certain factors needed to be improve and
attention. The implemented programs and organizations providing symposium and various
health related feeding menus and activities. The budget needed must be precise and consider
the people in the baranggay of their perspective needs.

Part I.

In Bayugan City, although it is now improving and growing as a city, there are still various
problems that rises that affects the beauty and its environment. One of it is the problem
related to waste disposal. As we all know, waste disposal has been number one issue every
city has been struggling to assess. Thus, implementing a project that is related about the
current and ever lasting issue will provide the necessary measurements, practices, values,
rules and regulations to minimize and may put stop of the arising issue at hand. Solid waste
management SWM is the action set to manage the arising issue faced by the City of Bayugan.

Step II: Decide on scope

This project will be focusing on the area of Bayugan and baranggays to make sure to be able
to address the problm stated above.

Step III: Identify Assets

The assets that may be helpful for this type of project will be the baranggay members,
CENRO, NGO’s, etc. Providing man power and guidance.

Step IV: Make connections

For this type of program and project, I would be asking the help of the council members in the
baranggay and to those in community health to be able to provide more accurate and guidance
for the necessary needs and procedures. Since, connections and help are needed to conduct
this type of project.
Step V: Collect Data

Collection of data is a must, to pinpoint the area in Bayugan that needed to be clean and to
provide the necessary man power, equipments, methods and process to be used and
transportation and to oversee the location, areas and security upon conducting the project.

PART III: CONCLUSION

Hence, with regards of the said project about solid waste management, actions and practices
still lacks in the City. The absence of awareness and responsibility leads to waste disposal
issues. And with this project, it identify the problems and methods needed to be done and the
possible strengths, weaknesses, opportunities and threats with regards to this project. That is
said, further more improvement and practices can an will solve this issue.

RECOMMENDATIONS

Thus, this leave us to further more set our attention on our waste management issues and
ways. For my recommendation upon this area of field, providing more man power and
attention, funds and equipment s necessary, it is not only for the sake of the people, but also
for the sake of our environment that we need this to be taken care of. Waste disposal issue is
simple problem yet complex that might bring danger to both humans and environment. That is
why, practices and our attention is needed to expand and introduce ways to avoid making it
more a bigger problem for future generation.
