This is to certify that Mr. Jhun Dave R. De Vera and Carissa L. Delos Santos, both

residents of Tococ West Bayambang, Pangasinan are allowing to apply Marriage License at Civil

Registrar Office, if ever limited their guests to avoid Mass Gathering in the reception and

maintain social distancing protocols on their Wedding Ceremony to be scheduled on January 3,

2021.

This certification is being issued upon the request of the above-named persons for
