AFFIDAVIT OF INSERTION

I, NAME OF BUYER, of legal age, Filipino, present status, a resident of present


address, after having duly sworn to in accordance with law do hereby depose, certify and
state that:

1. That, I am the BUYER in a DEED OF ABSOLUTE SALE (the “deed”) involving a


parcel of land with exisitng improvement, situated in location, containing an area
of Seventy Five (75) square meters more or less, covered under Transfer
Certificate of Title No. 0000 of the Registry of Deeds for Branch;

2. That, the said deed has been executed and signed on DATE OF NOTARY before
ATTY. VIRGILIO R. BATALLA, a Notary Public for and in the City of Makati, and
was entered into his Notarial Registry Book as Doc. No. 00; Page No. 00; Book
No. 00; Series of 2019;

3. That, however, marital status of SELLER and my present civil status being the
BUYER were not indicated therein; therefore, I intend to insert the same in the
said deed subject to execution of the foregoing and proof for marital consent of
spouse in the sale and/or purchase of property;

4. That, in view thereof, I hereby attest that our civil status as indicated hereunder
are the same to the civil status inserted in the said deed:

Seller: NAME, MARRIED to NAME OF SPOUSE


Buyer: NAME, SINGLE

5. That, I further attest that the above information are true and correct to the best
of our knowledge and belief;

6. That, I am executing this affidavit to attest to the truth of foregoing facts, for
information and guidance of proper authorities concerned and for whatever
legal purpose this may serve.
