work place and it is also referred to as nursing handover, has been defined by Tobiano et al.,
(2018) as a point in care where the transfer of responsibility and accountability for patient care
moves from one nurse to another nurse. However, there are some situation wherein nurses
encountered problems concerning communication in the situation where the nurse doing
handover or endorsement to another nurse have forgotten to include some of tiny but important
details to note down. It is a one of the communication problems encountered by both of the nurse
for the reason that they do not thoroughly check the patient’s medical chart. The prescription
ordered by attending physician also pose as a problem that nurse may encounter in their work.
We have a slight knowledge to a doctors illegible handwriting and it pose as a problem to a nurse
if they are ordered to give a kind of medication to certain patient but unable to decipher the
doctor’s prescription. Laughable as it may sound but this is a common problem some nurses have
encountered and it is a serious issue because nurses deal with life, death, and everything in
between. Therefore, nurses need effective and good communication skills to sincerely
comprehend the patient’s overall health in order to provide the best possible care to the patients.
