APPRAISAL SUMMARY

This appraisal summary covers the month of December 2019. This appraisal is for Gerry Franceslio- IT
Iloilo, Roel Laspoňa-IT Davao, Juamar Tres Reyes- IT Cebu, Aleks Pariolo-IT Quezon City, and Lloyd Gadoy
–IT Alabang.

This appraisal covers the area of every individual’s Attitude towards work, Initiative to provide solution,

SUMMARY FOR IT TEAM :

Gerry Franceslio- IT Iloilo has an overall score of 98% which means he got an excellent appraisal.

Roel Laspoňa-IT Davao has an overall score of 98% which means he got an excellent appraisal.

Juamar Tres Reyes- IT Cebu has an overall score of 75% which means he got an average score.

Aleks Pariolo-IT Quezon City has an overall score of 75% which means he got an average score.

Lloyd Gadoy –IT Alabang has an overall score of 100% which means he got an excellent appraisal.

REMARKS :

Gerry Franceslio- IT Iloilo is an excellent team player. He always sees to it that his task is done. A very
hard working team member, who always adhere to the rules. He has the initiative to propose changes or
solutions to any given challenge. An always dependable member of the team.

RECOMMENDATIONS :

I therefore recommend to increase Gerry’s salary from 14,000 to 20,000 since he deserve this increase
for all the hard work. I believe with the increase Gerry would be more inspired to work hard and
contribute more to the progress of our IT team which will result to successful projects in the future.

Roel Laspoňa-IT Davao is a very dependable team member. He always submit any requested documents
on time and always heed our request for support. A very hard working team member who always share
his time to all of us. Though he needs to sometimes actively participate in our discussions or meetings.
Roel is has an advance technical knowledge when it comes to application which he reserves to share and
according to him this is because he hesitates to suggest out of respect to other team members. I already
advised him to not hesitate and propose any solution that he has. Roel is an asset to the team, with his
advance technical knowledge we can utilize him in future projects.

RECOMMENDATION:

I recommend to increase his salary from 18,000 to 21,000 as a token for his contribution to our team
and company. According to him he was promised before that his salary would be increased after 1year. I
recommend to the management to grant this increase as he well deserved it for the hard work and
contribution.
Juamar Tres Reyes- IT Cebu has recently been failing our appraisal. He is not focus in doing his task or
job as an IT member. He neglects to input important data in our helpdesk system which is very
important as Lloyd Gadoy gather these data from this system for further analysis on how can we
improve our IM support. I already informed Ms. Diane, our Cebu admin to talk to him and ask him to
improve his situation. I also talked to him about his appraisal and what areas he needs to improve. So far
he promised that he will now be focus on his job and will reform.

RECOMMENDATION:

Since Juamar is a senior technical guy, I recommend that we give him some technical task that could
motivate him to utilize his skill.

Aleks Pariolo-IT Quezon City is a poor team player. He always has excuses for not doing his job or task.
Worse you can see no cooperation or the initiative to do his job. He would always work around our
rules to his advantage. He does not do reports when required. Does not input his tickets which is crucial
for our IM support.

RECOMMENDATIONS:

I recommend to suspend him if he will not improve next month. And I would recommend to terminate
his services if his attitude persist.

Lloyd Gadoy –IT Alabang is a very dependable and a very good leader to the team.
