3-Level of MDM

This document provides guidelines for assigning codes to patient encounters based on three categories: 1) Number and complexity of problems addressed, 2) Amount and complexity of data reviewed, and 3) Risk of complications. It lists codes 99211 through 99215 and describes the criteria for each level in terms of the three categories, such as the number of chronic illnesses or tests reviewed. The highest codes (99205 and 99215) require extensive data review and a high risk of complications, while the lowest codes (99211 and 99212) involve minimal data and low risk.
