II. CERTIFICATION
This is to certify to the best of our knowledge that Our Lady of Piat High School has IT

infrastructure with technology support for the following:

a. PLDT as the school internet provider with 30 Mbps. bandwidth.

b. Web hosting service provider.

c. School’s ICT Coordinator and Quipper Support Officer as technical support for

learners, teachers and other users. (parents/guardians)

d. QUIPPER APP as the School’s Learning Management System.

III. The school make use of other social media/social networking apps/website like Zoom,

Messenger Room, Google Meet, Facebook and other third party app whenever needed.

This certification is issued in compliance with the provision of DepEd Order No. 013, S.

2020.

STATEMENT OF UNDERTAKING

I, Ernestina P. Aquino, School Principal of Our Lady of Piat High School with office

address, Poblacion 02, Piat, Cagayan, after having been duly sworn, depose and state that:

1. A courseware for each learning area in different grade levels are available and ready for

the school year. A self-learning package in digital format is provided to learners for

independent study, and a set of educational materials in digital format too for the use of

the teachers in the delivery of instruction.

2. A e-learning platform for users (Admin, teachers, learners and parents) is accessible.
