Pre-Activity: Physical Readiness Questionnaire (PAR-Q) & You

PHYSICAL ACTIVITY READINESS QUESTIONNAIRE (PAR-Q) & YOU

Directions: Please read the questions carefully and answer each one honestly. Common sense is your
best guide when you answer these questions. Check YES or NO.

YES NO
1. Has your doctor ever said that you have a heart condition and that
you should only do physical activity recommended by a doctor?
2. Do you feel pain in your chest when you do physical activity?
3. In the past month, have you had chest pain when you were not
doing physical activity?

Activity 2: Self-assessment of Body Composition

A. Fill-up the table below with your own measurement

Body Composition

Test Measurement
Height in meter 1.63
Weight in kilogram 50kg
Waistline in centimeter 71cm
Hipline in centimeter 81cm

B. Based on your own measurement, compute your Body Mass Index and Waist to Hip Ratio
following the format below:

1. BMI = Weight (in Kilograms)/Height (in Meter)² =

50 (weight in kgs)

(1.63)² (height in meter)² =

50 (weight in kgs) 1.63 x 1.63 = 2.61 (height in meter x height in meter)

=50/2.61 = 19.16

(answer)

BMI = 19.16 Interpretation: NORMAL

2. WHR = Waist Circumference (cm)/Hip Circumference (cm) =

WHR= 71 cm/ 81 cm = 0.88

WHR = 0.88 Interpretation: HIGH RISK

C. Fill up the table below:

Body Composition

Test Score/Result Interpretation

Body Mass Index (BMI) 19.16 NORMAL
Waist to Hip Ratio (WHR) 0.88 High RISK

Activity 3: Suggested sports activities/skills Based on the results of your cardiovascular

endurance and body composition assessments, list down at least three (3) sports-related
activities for each component to improve the results.

3-minute step test – Cardiovascular Endurance
